연구 개요
상세 설명
이 연구에서는 2가지 절차가 수행됩니다. 첫 번째 절차는 P1으로 명명되며 위약 또는 보툴리눔 독소 A의 피하 주사로 구성됩니다. 두 번째 절차는 P2로 명명된 교차 절차입니다. 교차 절차의 경우, 처음에 보툴리눔 독소 A를 투여받은 대상자는 위약을 투여받게 되고, 처음에 위약을 투여받은 대상자는 보툴리눔 독소 A를 투여받게 됩니다. 이것은 무작위 이중 맹검 위약 대조 시험입니다. 모집된 피험자는 P1 직전에(또는 P1을 방문하기 전에 방문하는 동안) 동의, 등록 및 평가됩니다. 초기 치료 전 평가 후 피험자는 위약 또는 보툴리눔 독소 A를 무작위로 피하 투여합니다(P1). 전화 후속 조치(또는 이메일 후속 조치)는 P1 이후 2주 및 8주에 수행됩니다. 현장 후속 조치는 P1 후 4주 및 P1 후 12주에 수행됩니다.
교차 연구: 3개월 현장 평가(P1 후 12주) 후, 동일한 방문 동안 피험자는 교차 연구를 진행합니다. 이 시점에서 환자는 크로스오버 제제를 반복적으로 피하 주사할 수 있는 선택권을 갖게 됩니다. 원하는 경우 동일한 방문에서 교차 제제의 피하 주사가 수행됩니다. 반복 주사를 연기하고자 하는 경우, P1 후 12주에서 24주 사이(24주 후 피험자는 P2를 받지 않음)에 4주마다 연락하여 교차 피하 주사를 원하는지 묻습니다. 오버 에이전트. 원하는 경우 다음 주에 반복 주사가 예정되어 있습니다.
초기 보툴리눔 독소 A/위약 주사(P1) 후 시간의 가변성에 대한 이론적 근거는 보툴리눔 독소 A 후 개인의 통증 반응의 가변성을 문서화하는 것입니다. 문헌에 보고된 바와 같이, 피하 보툴리눔에 반응하는 피험자 통증에 대한 독소 A 주사, 대부분은 12-24주 내에 기본 통증 점수로 돌아갑니다.

포함 기준:
- 18세에서 80세 사이
- 외상성 척수 손상으로 진단
- 표적 통증은 의사가 높은 확실성(0은 "순전히 추측"이고 5는 "완전히 확실함"인 0-5 범위의 리커트 신뢰 척도를 사용하여 4 또는 5)에 있는 수준의 SCI로 간주합니다.
- 서면 동의서를 제공할 수 있음
- 최소 1개월 이상 지속적으로 나타나는 표적 통증
- 표적 통증은 지난 주에 걸쳐 적어도 중등도 평균 강도이고, 예를 들어 SCI 모집단에서 중등도 통증에 대한 컷오프 포인트인 수치 평가 척도에서 4/10 이상입니다.
- 표적 통증은 NLI를 식별하는 피부 분절 내 또는 NLI 아래 3 수준 내에 국한됩니다.
- 피험자는 최소 3주 동안 안정한 용량의 진통제(또는 진통제를 복용하지 않음)를 복용했으며 연구 기간 동안 현재 요법을 유지하는 데 동의합니다(이전에 처방된 획기적인 진통제가 허용됨).

- 현재 감염

ACTIVE_COMPARATOR: 보툴리눔 독소 A
보툴리눔 독소(100U, BOTOX, Allergan)의 각 바이알은 제조자가 권장하는 4ml 비보존 식염수(0.9%)로 재구성됩니다(5 단위 보툴리눔 독소 A/0.2ml의 농도).
각 주사는 0.2mL(BOTOX, 5단위)이며, 25 게이지 바늘을 통해 투여됩니다.
표시된 부위는 표시된 부위에 다른 주사와 반경 1cm 간격으로 각각 피하주사합니다(최대 80회, 400단위).

기계적 자극에 대한 정적 이질통은 나무 막대기 끝의 압력에 의해 유발되는 통증 감각으로 정의됩니다.
