Gedetailleerde beschrijving
In deze studie zullen er 2 procedures worden uitgevoerd. De eerste procedure zal P1 heten en bestaat uit subcutane injectie van een placebo of Botulinum Toxine A. De tweede procedure zal de cross-over procedure genaamd P2 zijn. Voor de cross-over-procedure krijgen de proefpersonen die aanvankelijk Botulinetoxine A hadden gekregen een placebo en de proefpersonen die aanvankelijk een placebo hadden gekregen, Botulinetoxine A. Dit is een gerandomiseerde, dubbelblinde, placebogecontroleerde studie. Gerekruteerde proefpersonen zullen onmiddellijk voorafgaand aan P1 (of tijdens een bezoek voorafgaand aan het bezoek voor P1) toestemming krijgen, worden ingeschreven en geëvalueerd. Na de initiële evaluatie voorafgaand aan de behandeling krijgen proefpersonen willekeurig placebo of botulinumtoxine A subcutaan (P1). Een telefonische follow-up (of e-mail follow-up) vindt plaats 2 weken en 8 weken na P1. Een follow-up ter plaatse zal worden uitgevoerd 4 weken na P1 en 12 weken na P1.
Cross-over-onderzoek: na de evaluatie ter plaatse in de derde maand (12 weken na P1), tijdens hetzelfde bezoek, gaat de proefpersoon verder met het cross-over-onderzoek. Op dit moment heeft de patiënt de mogelijkheid om een herhaalde subcutane injectie van het cross-over-middel te krijgen. Als ze er een willen, zal tijdens datzelfde bezoek een onderhuidse injectie van het cross-over-middel worden uitgevoerd. Als ze de herhaalde injectie willen uitstellen, wordt er elke 4 weken - tussen 12 weken en 24 weken na P1 (geen enkele proefpersoon krijgt P2 na week 24) - gevraagd of ze de subcutane injectie van de kruisinjectie willen hebben. over agent. Als ze er een willen, wordt er de volgende week een herhalingsinjectie gepland.
De grondgedachte voor een variabele tijdsduur na de initiële Botulinetoxine A/Placebo-injectie (P1) is het documenteren van de variabiliteit van de pijnrespons van individuen na Botulinetoxine A. In de literatuur is melding gemaakt van de proefpersonen die reageren op subcutane Botulinetoxine A-injectie. Toxine A-injecties tegen pijn, de meesten zullen binnen 12 tot 24 weken terugkeren naar hun basislijn pijnscore.

Beschrijving
Inclusiecriteria:
- Tussen de 18 en 80 jaar
- Gediagnosticeerd met traumatische dwarslaesie
- Doelpijn wordt door de arts beschouwd als SCI op niveau met een hoge mate van zekerheid (4 of 5 op een Likert-betrouwbaarheidsschaal van 0-5, waarbij 0 "puur een gok" is en 5 "absoluut zeker")
- In staat om schriftelijke geïnformeerde toestemming te geven
- Target pijn die continu aanwezig is gedurende ten minste een maand
- Doelpijn is van ten minste matige gemiddelde intensiteit gedurende de afgelopen week, bijv. groter dan of gelijk aan 4/10 op een numerieke beoordelingsschaal, het afkappunt voor matige pijn in een SCI-populatie.
- Doelpijn is gelokaliseerd in het dermatoom dat de NLI identificeert of binnen 3 niveaus onder de NLI
- Proefpersoon heeft gedurende ten minste 3 weken een stabiele dosis analgetische bemiddeling gebruikt (of geen analgetische medicatie) en is het ermee eens om op het huidige regime te blijven voor de duur van het onderzoek (eerder voorgeschreven doorbraakanalgetica zijn toegestaan)
Uitsluitingscriteria:
- Zwangerschap
- Geschiedenis van intolerantie, overgevoeligheid of bekende allergie voor botulinumtoxine of zijn conserveermiddelen
- Geschiedenis van intolerantie, overgevoeligheid of bekende allergie voor EMLA-crème (lignocaïne / prilocaïne eutectisch mengsel) dat wordt gebruikt als analgeticum tijdens BoNT-injectie
- Recente geschiedenis van toediening van botulinetoxine (binnen de afgelopen 6 maanden)
- Contra-indicaties voor botulinumtoxine (myasthenia gravis of andere ziekte van de neuromusculaire junctie)
- Stollingsstoornis
- Huidige infectie
- Onvoldoende beheersing van het Engels om zelfrapportage-instrumenten in te vullen.

ACTIVE_COMPARATOR: Botulinetoxine A
Elke injectieflacon botulinetoxine (100E, BOTOX, Allergan) wordt gereconstitueerd met 4 ml niet-geconserveerde zoutoplossing (0,9%) zoals aanbevolen door de fabrikant (concentratie van 5 eenheden Botulinetoxine A/0,2 ml).
Elke injectie is 0,2 ml (BOTOX, 5 eenheden), toegediend via een naald van 25 gauge.
Het gemarkeerde gebied krijgt subcutane injecties, elk gescheiden door een straal van 1 cm, van de andere injecties in het gemarkeerde gebied (maximaal 80 injecties, 400 eenheden).

De proefpersonen krijgen subcutane injecties met botulinumtoxine A in de gemarkeerde pijnlijke regio.
De spuiten worden voorafgaand aan de injectie door een derde partij voorbereid en de beheerder van de procedure zal blind zijn voor de inhoud van de spuit.
Deze arts zal de injecties onder steriele omstandigheden uitvoeren.
Lokale anesthesie, EMLA (lignocaïne / prilocaïne eutectisch mengsel) crème, tot 4 gram, zal topisch worden aangebracht voor lokale anesthesie.
Na 50 minuten wordt de crème verwijderd.
De bovenliggende huid wordt gesteriliseerd met een betadine- of alcoholoplossing.

Mechanische allodynie is een kenmerk van opgewekte pijn bij personen met neuropathische pijn.
Statische allodynie voor mechanische prikkels zal worden gedefinieerd als een gevoel van pijn dat wordt opgeroepen door de druk van het uiteinde van een houten stok.
Het uiteinde van een houten stok raakt het aangetaste gebied gedurende 10 seconden met voldoende druk om de huid in te drukken.
Daarna wordt de proefpersoon gevraagd de waargenomen pijn te beoordelen op een 11-punts NRS.
