미숙아에서 루테인/제아잔틴의 조기 투여

루테인은 인체의 특정 조직, 주로 망막, 황반(따라서 이름) 및 수정체 수준에 선택적으로 존재하는 가장 중요한 카로티노이드입니다. 조직과 혈청에서 루테인은 카로티노이드 이수산화물, 그 이성체인 제아잔틴과 함께 발견됩니다. 루테인과 제아잔틴은 탯줄 수준에 존재하고 태반 장벽을 통과하며 혈장, 모유, 특히 초유에도 존재합니다. 투여 방법과 관련하여, 루테인은 경구 투여 후 생체이용률이 증가하는 특정 특성을 나타냅니다. 카로티노이드가 풍부한 영양분을 제공한 후 루테인의 혈류 수준은 베타카로틴에서 관찰된 14%에서 67%로 증가합니다. 체외 및 생체 내에서 수행된 학제간 및 조정 연구를 통해 다양한 작용 메커니즘이 확인되었습니다. 특히 일중항산소와 활성산소종(ROS)의 중화(quench) 현상을 통해 생성되는 루테인에 의한 조직기능의 방어기전을 규명하였다. 이 작용은 항산화 기능, 항염증 특성, 항종양 효과를 촉진하는 특성, 해독 효소 유도 및 관절 사이의 통신을 촉진하는 단백질에 대한 긍정적 효과(상향 조절)와 같은 다양한 활동을 분자에 제공합니다. 최근 ROS에 의해 결정되는 산화 스트레스와 유해 작용이 알츠하이머, 성인의 파킨슨, 신생아의 ROP 및 NEC와 같은 많은 신경계 질환의 발병에 중요한 역할을 할 수 있음을 입증하는 실험적 및 화학적 데이터가 나타났습니다.
이는 신경계가 지질 과산화를 통한 ROS 공격의 영향을 받는 최초의 세포 화합물인 다중불포화 지방이 풍부한 막이 특징이라는 사실 때문입니다. 유사한 메커니즘이 특정 안구 조직(황반, 수정체, 망막)에 나타날 수 있으며, 다량의 고도불포화 지방산을 함유하고 ROS에 의해 유도된 산화적 분해가 있는 다른 구조보다 더 취약합니다.
카로티노이드가 자연계에 존재하는 가장 강력한 항산화제라는 사실 때문에 신생아의 신경퇴행성 질환을 예방하는 이러한 물질의 기능적 역할에 관한 새로운 연구가 개발되고 있습니다.
이러한 다중불포화 지방산은 산화에 매우 민감하기 때문에 혈장 수준의 변화는 산모와 태아의 항산화 시스템 상태에 영향을 미칩니다. 많은 연구에서 임산부의 고도불포화 지방산의 과산화에 대한 감수성이 증가하면 출생 직후 급격히 감소하는 토코페롤 혈장 농도의 동등한 증가가 동반된다는 것이 입증되었습니다.
신생아의 항산화제의 혈장 농도는 어머니의 농도보다 낮았습니다. 탯줄에서 토코페롤과 카로티노이드 수치는 모체 혈장에 등록된 수치보다 현저히 낮으며 신생아의 고도불포화 지방산 농도는 모체보다 상당히 높고 훨씬 더 증가합니다.
또한 특정 연구에서는 출생 후 축적되는 것으로 추정되는 산화 스트레스 및 산소 반응성 종에 대한 관심이 증가하고 있음을 보여주었습니다. 분만실에서 일반적으로 사용되는 많은 관행(예: 임산부에게 통증을 완화하기 위해 투여하는 약물, 신생아의 적출 방법, 체온 감소를 최소화하는 기술, 탯줄을 차단하고 특히 산소를 100% 사용하거나 질식 징후가 있는 신생아를 위한 통풍실)이 항상 효율적인 것으로 입증되지는 않으며 활성산소의 상당한 증가로 인해 신생아의 건강을 해칠 수도 있습니다.
일부 특정 연구에서는 산소로 100% 또는 산소로 21% 처리된 질식이 있는 신생아의 탯줄 혈장에서 마커로 강조 표시된 자유 라디칼 수준을 질식이 없는 어린이의 대조군과 비교했습니다. 자유 라디칼 수치는 세 그룹 모두에서 출생 직후 유의미하게 증가했으며 질식이 있는 신생아의 두 그룹에서 증가했습니다. 산소로 21% 치료를 받은 그룹에서는 이러한 값이 감소하여 출생 후 28일에 질식이 없는 신생아와 동일한 수준에 도달한 반면, 산소로 100% 치료를 받은 그룹에서는 자유 라디칼 수치가 매우 높게 유지되었습니다.
따라서 신생아가 100% 산소에 잠깐 노출되는 것은 확장된 산화 스트레스 상태와 자유 라디칼의 지속적인 증가의 원인이 되며, 이는 생후 첫 달 동안, 특히 미숙아에서 다양한 질병과 병리에 관여하는 것으로 보입니다. ROP, IVH, BPD, NEC 및 감염의 발생률이 크게 증가하는 유아.
이러한 결과는 신생아가 산화환원 균형을 확립하고 높은 수준의 자유 라디칼 및 산소 반응성 종에 장기간 노출되어 발생하는 문제를 방지하기 위해 항산화 보호 수준을 높일 필요가 있음을 보여줍니다.
조산은 사망, 이환율 및 장애의 가장 흔한 원인입니다. 미숙아는 안구 또는 신경학적 병변이 발생할 위험이 매우 높습니다. 나타날 수 있는 시각 수준의 주요 합병증은 미숙아 망막병증, 소위 ROP입니다. 산화 스트레스는 이 질병의 병인에 관여합니다. 실제로 미숙아는 호흡기 문제로 인해 잠재적으로 유해한 산소 농도에 노출되거나 청색광 강도가 높은 광선 요법에 노출되는 경우가 많습니다. 이러한 치료 관행은 자유 라디칼의 원천입니다.

문헌에는 신생아에서 루테인/제아잔틴을 사용한 연구와 결과가 이미 나와 있습니다.
최근 Gong의 연구는 Romagnoli, Dani 및 Manzoni를 포함한 다양한 연구에서 얻은 데이터를 비교하여 루테인/제아잔틴의 역할을 평가했습니다. 또한 주제에 대한 Rubin의 RCT 분석 덕분에 연구자들은 루테인/제아잔틴이 내약성이 우수하고 미숙아의 경구 투여 후에도 잘 흡수된다는 결론을 내렸습니다.
통계적으로 유의하지는 않지만(아마도 작은 샘플로 인해) 나타난 매우 흥미로운 결과는 루테인/제아잔틴 보충이 ROP의 발생률과 중증도를 감소시켰다는 것입니다.
이 프로토콜은 이전 작업의 흥미로운 결과가 주어지면 0.5mg의 루테인 및 0.05mg의 제아잔틴과 동일한 최소 1ml/kg의 용량을 심화하는 것이 중요하다고 간주된다는 생각에서 탄생했습니다.
산화 스트레스에 대한 주요 마커의 평가는 치료 중 및 치료 후 생물학적 항산화 잠재력(BPT) 및 총 하이드로퍼옥사이드(TH) 연구와 함께 필요합니다.
이미 이전 연구에서 S. Perrone과 M. Longini는 BTP와 TH를 측정하여 루테인/제아잔틴 투여 도중과 투여 후 만삭아의 자유 라디칼 감소를 입증했습니다.
루테인과 제아잔틴을 기반으로 한 제제는 투여 후 또는 위장이나 전신 수준에서 사람에게 부정적이거나 유해한 영향을 나타내지 않았습니다. 최근 연구에서 루테인 또는 제아잔틴 20mg/일을 6개월 동안 투여한 후 부작용이나 다른 지용성 영양 요소와의 상호작용이 보고되지 않았습니다.
