상세 설명
이것은 무작위, 단일 센터 이중 맹검 연구입니다.
이 연구의 주요 목적은 심장-순환 사망 결정(DCD) 후 신장 수혜자에서 위약과 비교하여 지연된 이식 기능의 발생률 및 중증도를 감소시키는 rhC1INH의 능력을 결정하는 것입니다.
이 시험은 DGF 발병 위험이 높은 환자에서 rhC1INH 치료의 보호 효과를 평가하기 위해 특별히 설계되었습니다. 이 연구의 일부가 될 잠재적 기증자의 선택은 역사적으로 40-55% 범위의 DGF 발생 위험을 보여준 DCD 기증자 집단으로 제한됩니다. 각 그룹의 참여는 무작위로 지정됩니다. 위약 또는 rhC1INH(100 Units/kg) IV를 수술 중 주입한 후 48시간 동안 하루에 두 번 50 Units/Kg IV를 주입하여 치료합니다.
총 20개의 과목이 2개 그룹으로 나뉩니다.

최대 용량은 초기 용량의 경우 8400 단위이고 두 번째 및 세 번째 용량의 경우 최대 4200 단위입니다.

이식 수혜자의 포함 기준:
이식된 신장을 받는 성인 환자는 연구의 일부로 간주되기 위해 다음을 충족해야 합니다.
- 연구의 요구 사항을 이해할 수 있는 능력이 있고 서면 동의서를 제공할 수 있습니다(연구 관련 건강 정보의 사용 및 공개에 대한 동의 포함).
- 18세 이상의 남성 또는 여성.
- 사망한 기증자로부터 이식을 받는 사람(심장 순환계의 사망 기준 결정 후 기증).
- 토끼 다클론 항-흉선 세포 글로불린, 항-CD25(항-IL2R) 또는 항-CD52를 사용한 항체 유도 요법과 같은 치료 유도 요법 요구 사항의 표준을 준수할 수 있습니다.
여성 피험자는 다음과 같은 경우 연구에 참여할 수 있습니다.
- 임신 또는 수유 중이 아님

- 다장기 이식을 받을 예정입니다.
- 한꺼번에 이식되는 계획된 신장 이식(이중 신장 이식)이 있습니다.
- 한꺼번에 이식하지 않고 (동일한 기증자로부터) 이중 신장 이식을 계획했습니다.
- 이식 혈전증으로 인해 첫 번째 신장 이식을 잃었습니다.
- 장기 회복 직전에 허혈성/재관류 손상에 대해 다른 IND/CTA에 따라 연구 요법을 받은 것으로 알려진 기증자의 신장 이식이 예정되어 있습니다.
- 인간 단클론 항체 또는 연구 약물 부형제에 대해 알려진 과민성.
- 바실리시맙, Campath-1H 또는 항흉선세포 글로불린(ATG)에 대한 이전의 과민성.

- HIV 양성 수혜자.
- B형 간염 표면 항원 양성 신장 이식 수혜자.
- B형 간염 핵심 항체 양성 신장 이식 수혜자.
- 치료를 받지 않았거나 항바이러스 치료 후 12개월 이상 지속적인 바이러스 관해를 입증하지 못한 C형 간염 바이러스 양성(HCV+) 환자.
- 지속적인 치료가 필요한 임상적으로 중요한 감염의 존재.
- 활동성 결핵에 대한 양성 선별검사.
- 조사자의 의견에 따라 연구 약물의 분포, 대사 또는 배설을 크게 변경할 수 있는 현재 이식 이외의 모든 외과적 또는 의학적 상태의 존재.
- 수행된 경우 NIH 항글로불린 림프세포독성 방법 또는 CDC 교차 일치 방법에 의해 양성 T- 또는 B-세포 교차 일치가 있습니다.
