칸디다혈증 및/또는 침습성 칸디다증이 있는 성인 참여자에서 경구 FOSMANOGEPIX(PF-07842805)에 이어 IV 카스포펀진에 이어 경구 플루코나졸에 따라 IV를 조사하기 위한 중재적 효능 및 안전성 3상 이중 맹검, 2군 연구.
이 임상 시험의 목적은 칸디다증 및/또는 침습성 칸디다증(칸디다라고 하는 여러 종의 효모에 의해 발생하는 생명을 위협하는 진균 감염)의 잠재적 치료를 위한 연구 의약품(Fosmanogepix라고 함)의 안전성과 효과에 대해 알아보는 것입니다.
이 연구는 칸디다혈증 또는 침습성 칸디다증 진단을 받은 참가자를 찾고 있습니다.
모든 참가자의 2/3는 연구 약물인 fosmanogepix 정맥 주사(IV) 주입과 선택적 fosmanogepix 정제를 받게 됩니다. 모든 참가자의 1/3은 카스포펑진 정맥 주사(IV) 주입과 선택적 플루코나졸 캡슐의 표준 치료 요법을 받게 됩니다.
Fosmaogepix 또는 caspofungin은 먼저 연구 클리닉에서 매일 팔의 정맥에 직접 정맥(IV) 주입으로 제공됩니다. Fosmaogepix 정제 또는 플루코나졸 캡슐은 연구 클리닉에서 또는 참가자가 병원에서 퇴원할 수 있을 만큼 건강할 경우 집에서 매일 경구로 복용합니다.
우리는 fosmanogepix를 받는 사람들의 경험을 caspofungin/fluconazole을 받는 사람들과 비교할 것입니다. 이는 fosmanogepix가 안전하고 효과적인지 판단하는 데 도움이 됩니다.
참가자는 감염이 제거되었는지 여부와 감염과 관련된 증상이 호전되었는지 여부에 따라 최대 6주 동안 치료를 계속합니다. 이 기간 동안 최대 10회까지 연구 방문을 하게 됩니다. 또한 연구 치료가 중단된 후 6주 후에 후속 방문이 있을 것입니다.

- 효모 또는 칸디다 종에 대해 양성인 배양, 혈액 또는 일반적으로 무균 부위에서 채취한 검체로부터 효모에 대해 양성 그람 염색(또는 기타 직접 현미경 검사 방법) 또는 혈액으로부터 후원자가 승인한 신속 진단 테스트를 사용하여 칸디다에 대해 양성 결과 견본.
- 칸디다혈증/침습성 칸디다증에 기인할 수 있는 하나 이상의 임상 기준의 존재: 열, 저체온증, 저혈압, 빈호흡, 빈맥 및 칸디다에 감염된 기관/부위와 관련된 국소 징후 및/또는 침습성 칸디다증을 암시하는 방사선 소견.
제외 기준:
- 다음을 포함한 기존 감염: Candida krusei로 인한 감염(혈액 또는 기타 정상적인 무균 부위), 부적절한 진균 감염원 제어, 특정 심층 칸디다 감염 진단 또는 동일하거나 인접한 감염 부위에서 일시적으로 관련된 입증된 세균 감염.
- 혈액 투석에 대한 요구 사항 또는 예상 요구 사항
- 연구 등록 전 96시간 이내에 칸디다혈증의 현재 에피소드를 치료하기 위해 이전의 전신 항진균제 치료에 상응하는 2일 이상을 받았다.
