상세 설명
우리는 발목을 완전히 의지적으로 제어하면서도 넘어질 위험을 해결하는 대체 전략인 저렴한 가변 마찰(VF) 신발을 제안합니다. 밑창은 보행 중 입각기 동안 마찰이 크고 스윙 중 마찰이 적습니다. 또한 흠집이 발생할 때 "클릭" 소리가 발생합니다.
우리의 중심 가설은 보행 단계 청각 바이오피드백과 결합되어 걸려 넘어질 위험을 완화하면서 발목의 의지적 움직임을 허용하면 발이 떨어지는 피험자의 보행이 개선될 것이라는 것입니다. 특히, 우리는 VF 신발이 AFO보다 훨씬 더 큰 치료 효과를 나타내면서도 AFO의 바람직한 교정 효과를 유지할 것이라는 가설을 세웠습니다. 우리는 두 가지 목표로 작업을 준비합니다.
구체적인 목표 1: 사용 기간 동안 VF 신발의 마찰력 감소를 특성화합니다. VF 신발의 효과를 이해하는 데 중요한 것은 스커프 포스 감소 수준을 특성화하는 것입니다.
구체적인 목표 2: 만성 뇌졸중 및 하퇴족 환자의 보행에 VF 신발이 미치는 영향을 평가합니다. AB-BA 임상 시험의 각 12주 단계 동안 참가자는 집에서 일주일에 최소 5일 동안 하루에 최소 30~45분 동안 걷게 됩니다.

포함 기준:
- 3개월 - 뇌졸중 후 1년(환자가 일반적으로 족하수 장치를 착용하는 기간)
- 18세 이상
- 규정된 AFO를 보유하고 있거나 AFO를 사용할 가능성이 있는 후보자
- 지팡이나 보행기 등 보조기구를 사용하거나 사용하지 않고 최소 10m를 걸을 수 있습니다.
- 의사의 의료 승인에 따라 의학적으로 안정적이라고 판단됨
- 최소 3개월 동안 예상되는 약물 변화 없음
- 자세를 취하는 동안 발목의 적절한 안정성
- VF 슈에서 발생하는 딸깍거리는 소리를 듣는 기능
- 의사의 승인
- 사전 동의를 제공하는 능력
- 지지 없이 앉을 수 있고 3단계 명령을 따를 수 있습니다.
- 상지 또는 하지에 치유되지 않은/해결되지 않은 정형외과적 부상이 없고 심한 허리 통증의 병력이 없습니다.
- 영어로 말하기
- 프로그램 전체 기간(6개월) 동안 하루에 최소 30분, 주당 최소 5일 동안 두 장치를 모두 사용하여 운동 프로그램을 따르려는 의향이 있는 사람
제외 기준:
- 뇌졸중 발생 전 일주일에 1회 이상 넘어진 병력
- 보행 속도: 1.2m/s보다 큰 자체 선택 속도(SSV)
- 장치를 안전하게 작동할 수 없고 간병인의 도움을 받을 수 없음
- 병적 비만(체질량지수 >40kg/m2)
- 심각한 심장 질환, 심근경색, 울혈성 심부전, 심박조율기 요구, 발작, 과도한 감각이상 통증, 신발의 핏이나 사용을 방해할 수 있는 심각한 하지 병리와 같은 기존 질환.
- 인지 또는 의사소통의 심각한 결함
- 임산부(자기 보고에 따라 결정되는 상태)
- 연구를 방해하는 동반질환(예: 심각한 관절염 또는 관절 문제, 허리 부상 병력, 신경근 장애, 간질 등).
- 중증 골다공증(자가 보고에 의해 결정된 상태).

SIS는 뇌졸중이 전반적인 신체 및 인지 기능에 미치는 영향을 검증된 척도입니다.
59개 항목으로 구성된 이 환자 기반 설문지는 뇌졸중 회복의 8가지 영역, 즉 근력, 이동성, 의사소통, 감정, 기억력 및 사고, 참여, 일상 생활 활동/일상 생활 도구 활동(ADL/IADL) 및 손 기능을 평가합니다.
추가 질문은 환자가 뇌졸중 회복을 0에서 100까지 평가하도록 요구합니다.
이 측정은 피험자에게 설문지 24주 전의 기간을 기준으로 질문에 답하도록 지시합니다.
