100万人以上のアメリカ人が脳卒中後に下垂足を患っています。
米国および世界中で人口の高齢化が進むにつれ、年齢が重要なリスク要因となるため脳卒中の発生率も増加するため、適切な治療を行うための低コストで使いやすく、拡張性の高いソリューションが必要となります。回復を促進する治療法。
この研究では、在宅環境における下垂足に対する新しい低コスト医療機器である可変摩擦シューズ (VF シューズ) を評価します。

詳細な説明
私たちは、足首の完全な意志制御を可能にしながらもつまずきの危険に対処する代替戦略、つまり安価な可変摩擦 (VF) シューズを提案します。 そのアウトソールは、歩行の立脚段階では高摩擦となり、スイング中は低摩擦になります。さらに、擦り傷が発生すると「カチッ」という音が発生します。
私たちの中心的な仮説は、つまずきの危険を軽減しながら足首の自発的な動きを可能にし、歩行段階の聴覚バイオフィードバックと組み合わせることで、下垂足の被験者の歩行が改善されるというものです。 具体的には、VF シューズは AFO よりも大幅に優れた治療効果を示し、しかも AFO の望ましい矯正効果を維持すると仮説を立てています。 私たちは 2 つの目標に基づいて仕事を行っています。
具体的な目的 1: 使用期間全体にわたる VF シューズの擦り傷力の軽減を特徴づけます。 VF シューズの効果を理解する上で重要なのは、擦り傷力の軽減レベルの特性評価です。
具体的な目的 2: 慢性脳卒中および下垂足を患う患者の歩行に対する VF シューズの効果を評価する。 AB-BA 臨床試験の 12 週間の各段階で、参加者は自宅で少なくとも週 5 日間、1 日あたり少なくとも 30 ~ 45 分間ウォーキングします。

- 脳卒中後3か月~1年(通常、患者に下垂足用の装置が装着される期間)
- 18歳以上
- 所定の AFO を所有している、または AFO の使用の潜在的候補者である
- 杖や歩行器などの補助具の有無にかかわらず、少なくとも10m歩行できる
- 医師の医療許可により医学的に安定していると判断される
- 少なくとも 3 か月間は薬剤の変更が予想されない
- 立脚時の足首の適切な安定性
- VFシューによって発せられるカチッという音を聞くことができる
- 医師の承認
- インフォームドコンセントを与える能力
- 支えを受けずに座ることができ、3 段階の指示に従うことができる
- 上肢または下肢に治癒していない/未解決の整形外科的損傷がなく、重度の腰痛の病歴もない
- 英語を話す
- 両方のデバイスを使用して、プログラムの全期間 (6 か月間) 1 日あたり少なくとも 30 分、週に少なくとも 5 日のエクササイズ プログラムに従う意欲がある
除外基準:
- 脳卒中前に週に1回以上転倒した歴
- 歩行速度: 1.2 m/s を超える自己選択速度 (SSV)
- 装置内で安全に操作できず、介護者の支援も受けられない
- 病的肥満(BMI > 40 kg/m2)
- 重篤な心臓病、心筋梗塞、うっ血性心不全、デマンドペースメーカー、発作、過度の知覚異常痛、靴のフィット感や使用を妨げる重篤な下肢の病状などの既存の症状。
- 認知またはコミュニケーションにおける重度の欠陥
- 妊婦(自己申告によりステータスが決定されます)。
- 研究を妨げる併存疾患(例: 重大な関節炎または関節の問題、背中の損傷の病歴、神経筋障害、てんかんなど)。
- 重度の骨粗鬆症(状態は自己申告によって決定されます)。

SIS は、全体的な身体機能および認知機能に対する脳卒中の影響を評価する検証済みの尺度です。
この 59 項目の患者ベースのアンケートは、脳卒中回復の 8 つの領域、すなわち筋力、可動性、コミュニケーション、感情、記憶と思考、参加、日常生活活動/日常生活手段活動 (ADL/IADL)、および手の機能を評価します。
追加の質問では、患者が脳卒中からの回復を 0 から 100 のスケールで評価する必要があります。
この措置は、アンケートの 24 週間前の期間に基づいて被験者に質問に回答するよう指示します。
