Karakterisering en klinische proef van een schoen met variabele wrijving, een nieuw paradigma van bewegingstherapie met verminderde beperkingen voor mensen die last hebben van klapvoet als gevolg van een beroerte

Wij stellen een alternatieve strategie voor die het struikelgevaar aanpakt, zelfs terwijl volledige controle over de enkel mogelijk is: een goedkope Variable Friction (VF)-schoen. De buitenzool biedt hoge wrijving tijdens de standfase van het lopen en lage wrijving tijdens het zwaaien; verder produceert het een "klik" wanneer er sprake is van slijtage.
Onze centrale hypothese is dat het toestaan van vrijwillige beweging van de enkel en tegelijkertijd het risico op struikelen verkleint, in combinatie met loopgefaseerde auditieve biofeedback, zal resulteren in een verbeterd looppatroon voor proefpersonen met een klapvoet. Concreet veronderstellen wij dat de VF-schoen aanzienlijk grotere therapeutische effecten zal vertonen dan een EVO, maar toch het gewenste orthetische effect van de EVO zal behouden. Wij verdelen ons werk in twee doelstellingen.
Specifiek doel 1: Karakteriseren van de vermindering van de schuurkracht van de VF-schoen gedurende de gebruiksduur. Cruciaal voor het begrijpen van de effecten van de VF-schoen is een karakterisering van de mate van vermindering van de schuurkracht.
Specifiek doel 2: Evalueer de effecten van de VF-schoen op het looppatroon bij personen met een chronische beroerte en klapvoet. Tijdens elke fase van 12 weken van een klinisch onderzoek met AB-BA lopen de deelnemers thuis minimaal 30-45 minuten per dag gedurende minimaal 5 dagen per week.

Beschrijving
Inclusiecriteria:
- 3 maanden - 1 jaar na een beroerte (de periode waarin patiënten doorgaans een apparaat voor klapvoet krijgen)
- Leeftijd 18 jaar of ouder
- In het bezit zijn van een voorgeschreven AFO of een potentiële kandidaat zijn voor het gebruik van een AFO
- Kan minimaal 10 meter lopen, met of zonder hulpmiddel zoals een wandelstok of rollator
- Medisch stabiel zoals bepaald door de medische goedkeuring van een arts
- Geen verwachte verandering in medicatie gedurende minimaal 3 maanden
- Voldoende stabiliteit aan de enkel tijdens stand
- Mogelijkheid om het klikgeluid van de VF-schoen te horen
- Goedkeuring van de arts
- Mogelijkheid om geïnformeerde toestemming te geven
- Kan zonder ondersteuning zitten en kan een driestapscommando volgen
- Geen ongenezen/onopgelost orthopedisch letsel aan de bovenste of onderste ledematen en geen voorgeschiedenis van ernstige rugpijn
- Engels sprekende
- Bereid om met beide apparaten een oefenprogramma te volgen van minimaal 30 minuten per dag, minimaal 5 dagen per week gedurende de volledige duur van het programma (6 maanden)
Uitsluitingscriteria:
- Geschiedenis van meer dan één keer per week vallen voorafgaand aan de beroerte
- Loopsnelheid: zelfgekozen snelheid (SSV) groter dan 1,2 m/s
- Onvermogen om de apparaten veilig te bedienen en er is geen hulp van een zorgverlener beschikbaar
- Morbide obesitas (body mass index >40 kg/m2)
- Reeds bestaande aandoeningen zoals ernstige hartaandoeningen, hartinfarct, congestief hartfalen, pacemaker, toevallen, overmatige dysesthetische pijn en ernstige pathologie van de onderste ledematen die de pasvorm of het gebruik van de schoen zouden verstoren.
- Ernstige tekorten in cognitie of communicatie
- Zwangere vrouwen (status bepaald door zelfrapportage).
- Comorbiditeit die het onderzoek verstoort (bijv. ernstige artritis of gewrichtsproblemen, voorgeschiedenis van rugletsel, neuromusculaire aandoeningen, epilepsie, enz.).
- Ernstige osteoporose (status bepaald door zelfrapportage).

De experimentele groep (A) krijgt een paar VF-schoenen en krijgt de opdracht schoenen aan beide voeten te dragen.
Deze groep zal worden geïnstrueerd om op harde oppervlakken te lopen om auditieve feedback van de VF-schoenen te krijgen.
Deze groepsgroep zal dagelijks worden gevolgd met behulp van actigrafie en zal wekelijks telefonische controlegesprekken krijgen van een lid van het onderzoeksteam om het aantal stappen, de looptijd, een schaal voor zelfgeïdentificeerde inspanning en waargenomen veiligheidsniveaus vast te leggen.
De controlegroep (B) krijgt een EVO voor gebruik op het paretische lidmaat.
Deze groep krijgt de instructie om op harde oppervlakken te lopen.

Objectieve maatstaf met 14 items, ontworpen om het statische evenwicht en het valrisico bij volwassen populaties te beoordelen.
Tot de beoordeelde items behoren onder meer het evenwicht in zit- en stapositie tijdens transfers, gewijzigde steunbasis, reiken, draaien, ogen open en gesloten.
Elk item krijgt een score van 0 tot 4 punten, met een maximum van 56 punten.

De SIS is een gevalideerde maatstaf voor de impact van een beroerte op het algehele fysieke en cognitieve functioneren.
Deze patiëntgebaseerde vragenlijst met 59 items beoordeelt acht domeinen van herstel na een beroerte: kracht, mobiliteit, communicatie, emotie, geheugen en denken, participatie, dagelijkse levensactiviteiten/instrumentele dagelijkse levensactiviteiten (ADL/IADL) en handfunctie.
Bij een aanvullende vraag moet de patiënt het herstel na een beroerte beoordelen op een schaal van 0 tot 100.
Deze maatregel instrueert proefpersonen om de vraag te beantwoorden op basis van de periode twee vier weken voorafgaand aan de vragenlijst
