이 중재적 임상 연구의 목표는 달팽이관 이식 수술에서 잔존 청력 보존을 위한 가벼운 치료 저체온요법의 사용을 조사하는 것입니다. 재판에서 답변하려는 주요 질문은 다음과 같습니다.
- 달팽이관 이식 중에 가벼운 치료용 저체온요법을 사용해도 안전한가요?
- 가벼운 치료적 저체온요법은 인공와우 이식 후 잔존 청력을 보존하는 데 효과적입니까?
참가자는 달팽이관 이식 수술 중에 가벼운 치료적 저체온 요법을 받게 됩니다. 연구자들은 치료를 받은 사람들의 결과와 대조군(치료를 받지 않은 개인)의 결과를 비교할 것입니다.

상세 설명
인공와우(CI)는 중증 내지 심도 감각신경성 난청 환자의 치료와 예후를 극적으로 변화시켰습니다. 최근 전기 음향 자극(EAS)의 출현으로 고주파 달팽이관 영역의 전기 자극과 저주파 음향 증폭을 결합하면 소음 속 어음 및 음악과 같은 어려운 청취 상황을 개선하여 부분 잔존 청력이 있는 환자에게도 도움이 될 수 있습니다. 감사. 불행하게도 임플란트 수술과 관련된 외상은 염증, 기계적 및 혈관 손상, 남아 있는 감각 유모 세포(HC) 및 뉴런의 손실을 초래합니다. 결과적으로, EAS 환자의 44% 이상이 CI 수술 후 잔여 청력을 상실하고 대부분의 환자에서 잔여 청력은 시간이 지남에 따라 30dB 이상 악화됩니다. CI 수술 후 잔여 청력 손실은 CI/EAS의 확장된 적용에 대한 주요 장벽이었습니다.

전반적으로 본 임상시험의 목적은 다음과 같습니다.
CI 수술 후 잔존 청력 보존을 위한 시스템 및 장치 어셈블리의 임상적 적용성, 안전성 및 효능을 검증합니다. 장치와 저체온증 전달 시스템은 마이애미 대학교에서 CI 수술을 받는 성인 환자를 대상으로 테스트될 예정입니다. 임상적 해석을 위해 연구자들은 잔류 청력 보존(스테로이드)을 위한 인공와우 이식에 대한 현재 표준 치료와 결합된 저체온요법의 적용이 효과적이라는 것을 입증하는 것을 목표로 합니다. 연구자들은 저체온요법 적용이 환자의 CI 기능(안전성)에 부정적인 영향을 미치지 않으며 CI 수술의 현재 표준 치료와 비교하여 초기 청각학적 결과(효능)를 향상시킬 것이라고 가정합니다. 연구자들은 표준 치료 및 저체온 요법을 받는 환자 그룹에서 임상적으로 관련된 수술 전, 수술 중 및 수술 후 조치를 활용할 것입니다.
이 임상 시험을 위해 예비 및 발표된 전임상 데이터, 기존 시스템 및 장치가 구축되었습니다. CI 수술을 위해 제안된 치료법과 장치 설계는 잔존 청력을 보존하기 위한 비약물적 치료 전략을 나타냅니다.

포함 기준:
연구에 대한 잠재적인 참가자는 심각도 내지 심도 청력 상실 진단을 받고 마이애미 대학에서 인공와우 이식을 받을 계획을 갖고 있는 환자들입니다. 인공와우 수술, 수술 전후 관리, 임플란트 활성화 및 프로그래밍은 참가자의 표준 치료의 일부입니다. 포함 기준(18세 이상, 남성과 여성의 동일한 수, 심도 감각신경성 청력 손실(안전성 연구 n=6) 및 수술 전 ABR(효능 연구 n=6)의 잔존 청력(아래 기준 참조))이 충족되는 경우 =48), 3개 제조업체 모두에서 동일한 수의 CI 장치를 사용하고 영어 또는 스페인어를 구사하는 환자에게 연구 인력이 접근하여 연구에 참여하도록 요청합니다.
제외 기준:
18세 미만의 대상. 기능적 청력 요구 사항을 충족하지 않는 피험자.
