Målet med denne intervensjonelle kliniske studien er å undersøke bruken av mild terapeutisk hypotermi for å bevare gjenværende hørsel ved cochleaimplantatkirurgi. Hovedspørsmålene forsøket tar sikte på å besvare er:
- Er mild terapeutisk hypotermi trygt å bruke under cochleaimplantasjon?
- Er mild terapeutisk hypotermi effektiv for å bevare gjenværende hørsel etter cochleaimplantasjon?
Deltakerne vil motta mild terapeutisk hypotermibehandling under cochleaimplantatkirurgi. Forskere vil sammenligne resultater fra de som mottar terapien med resultatene fra en kontrollgruppe (individer som ikke mottar terapi).

Cochleaimplantater (CI) har dramatisk endret behandlingen og prognosen for pasienter med alvorlig til dyp sensorineuralt hørselstap. Med den nylige bruken av elektroakustisk stimulering (EAS), lover den kombinerte akustiske forsterkningen ved lave frekvenser med elektrisk stimulering av høyfrekvente cochlea-regioner å være til fordel for pasienter med selv delvis gjenværende hørsel ved å forbedre vanskelige lyttesituasjoner som tale-i-støy og musikk verdsettelse. Dessverre resulterer traumer forbundet med implantatoperasjonen i betennelse, mekanisk og vaskulær skade og tap av gjenværende sensoriske hårceller (HC) og nevroner. Som et resultat mister mer enn 44 % av EAS-pasienter gjenværende hørsel etter CI-operasjon, og gjenværende hørsel forverres med mer enn 30 dB over tid hos de fleste pasienter. Dette tapet av gjenværende hørsel etter CI-kirurgi har vært den primære barrieren for en utvidet bruk av CI/EAS.
Denne kliniske studien vil evaluere en ny mild terapeutisk hypotermi-enhet som en effektiv terapi for å bevare gjenværende hørsel etter CI. Dette systemet krever ikke modifikasjon av gjeldende kirurgi eller CI-enheter og vil levere lokalisert, kontrollert mild hypotermi til de sensitive indre ørestrukturene. Med prekliniske publiserte resultater har etterforskerne allerede vist at lokalisert hypotermi under CI-kirurgi er svært effektivt og trygt for gjenværende hørselsbevaring. Her vil etterforskerne undersøke implementeringen av en terapi som gagner pasienter som gjennomgår CI ved å forbedre post-kirurgiske audiologiske utfall. Omfattende tidligere kommersialiserings- og utviklingserfaring, intellektuell eiendom og ingeniørekspertise kombinert med etterforskernes samarbeidspartneres kliniske ekspertise innen Otolaryngologi og Audiologi vil sikre det vellykkede kliniske og kommersielle resultatet av denne behandlingen.
Samlet sett er målet med denne kliniske studien:
Validere den kliniske anvendeligheten, sikkerheten og effektiviteten til systemet og enhetssammenstillingen for bevaring av gjenværende hørsel etter CI-kirurgi. Enheten og hypotermileveringssystemet vil bli testet på voksne pasienter som gjennomgår CI-operasjon ved University of Miami. For klinisk oversettelse har etterforskerne som mål å demonstrere at bruk av hypotermi kombinert med gjeldende standard for omsorg for cochleaimplantasjon for gjenværende hørselsbevaring (steroider) er effektiv. Etterforskerne antar at hypotermiapplikasjonen ikke vil ha en negativ innvirkning på CI-funksjonen hos pasienter (sikkerhet) og vil forbedre tidlige audiologiske utfall (effektivitet) sammenlignet med dagens standard for omsorg i CI-operasjoner. Utforskerne vil benytte klinisk relevante pre-kirurgiske, intraoperative og post-kirurgiske tiltak i grupper av pasienter som mottar standardbehandling og hypotermibehandling.
Foreløpige og publiserte prekliniske data, og et eksisterende system og enhet er konstruert for denne kliniske studien. Den foreslåtte terapi- og enhetsdesignen for CI-operasjoner representerer en ikke-farmasøytisk terapeutisk strategi for å bevare gjenværende hørsel.

Inklusjonskriterier:
Potensielle deltakere for studien er pasienter som har blitt diagnostisert med alvorlig til dyp hørselstap og planlegger å gjennomgå cochleaimplantasjon ved University of Miami. Cochleaimplantatkirurgien, pre- og post-kirurgisk behandling og aktivering og programmering av implantatet er en del av deltakernes standard for omsorg. Hvis inklusjonskriteriene er oppfylt (alder 18 år og eldre, like mange menn og kvinner, alvorlig sensorineuralt hørselstap (for sikkerhetsstudie n=6) og gjenværende hørsel (se kriteriene nedenfor) fra prekirurgisk ABR (for effektstudie n) =48), like antall CI-enheter fra alle 3 produsenter og engelsk- eller spansktalende pasienten vil bli kontaktet av studiepersonellet og vil bli bedt om å delta i studien.
Ekskluderingskriterier:
Forsøkspersoner under 18 år. Personer som ikke oppfyller funksjonelle hørselskrav.
