Tämän interventiokliinisen tutkimuksen tavoitteena on tutkia lievän terapeuttisen hypotermian käyttöä jäännöskuulon säilyttämiseen sisäkorvaistutekirurgiassa. Tärkeimmät kysymykset, joihin kokeilu pyrkii vastaamaan, ovat:
- Onko lievää terapeuttista hypotermiaa turvallista käyttää sisäkorvaistutuksen aikana?
- Onko lievä terapeuttinen hypotermia tehokas jäännöskuulon säilyttämisessä sisäkorvaistutuksen jälkeen?
Osallistujat saavat lievää terapeuttista hypotermiahoitoa sisäkorvaistutteiden leikkauksen aikana. Tutkijat vertaavat terapiaa saaneiden tuloksia kontrolliryhmän tuloksiin (henkilöt, jotka eivät saa terapiaa).

Sisäkorvaistutteet (CI) ovat muuttaneet dramaattisesti hoitoa ja ennustetta potilailla, joilla on vaikea tai syvä sensorineuraalinen kuulonmenetys. Sähköakustisen stimulaation (EAS) äskettäin käyttöönoton myötä yhdistetty akustinen vahvistus matalilla taajuuksilla korkeataajuisten sisäkorvaalueiden sähköstimulaation kanssa lupaa hyödyttää potilaita, joilla on jopa osittainen jäännöskuulo parantamalla vaikeita kuuntelutilanteita, kuten puhe melussa ja musiikki. arvostusta. Valitettavasti implanttileikkaukseen liittyvä trauma johtaa tulehdukseen, mekaanisiin ja verisuonivaurioihin sekä jäljellä olevien sensoristen karvasolujen (HC) ja hermosolujen katoamiseen. Tämän seurauksena yli 44 % EAS-potilaista menettää jäännöskuulon CI-leikkauksen jälkeen ja jäännöskuulo heikkenee ajan myötä yli 30 dB suurimmalla osalla potilaista. Tämä jäännöskuulon menetys CI-leikkauksen jälkeen on ollut ensisijainen este CI/EAS:n laajemmalle käytölle.
Tässä kliinisessä tutkimuksessa arvioidaan uutta lievää terapeuttista hypotermialaitetta tehokkaana hoitona, joka säilyttää jäännöskuulon CI:n jälkeen. Tämä järjestelmä ei vaadi muutoksia nykyisiin leikkaus- tai CI-laitteisiin, ja se aiheuttaa paikallisen, hallitun lievän hypotermian herkkiin sisäkorvan rakenteisiin. Prekliinisillä julkaistuilla tuloksilla tutkijat ovat jo osoittaneet, että paikallinen hypotermia CI-leikkauksen aikana on erittäin tehokas ja turvallinen jäännöskuulon suojelussa. Tässä tutkijat tutkivat sellaisen terapian käyttöönottoa, joka hyödyttää CI-potilaita parantamalla leikkauksen jälkeisiä audiologisia tuloksia. Laaja aiempi kaupallistamis- ja kehityskokemus, immateriaalioikeus ja insinööriosaamiseen yhdistettynä tutkijoiden otolaryngologian ja audiologian kliininen asiantuntemus takaavat tämän hoidon onnistuneen kliinisen ja kaupallisen tuloksen.
Kaiken kaikkiaan tämän kliinisen tutkimuksen tavoitteena on:
Vahvista järjestelmän ja laitekokoonpanon kliininen soveltuvuus, turvallisuus ja tehokkuus jäännöskuulon säilyttämiseen CI-leikkauksen jälkeen. Laitetta ja hypotermian antojärjestelmää testataan aikuisilla potilailla, joille tehdään CI-leikkaus Miamin yliopistossa. Kliinisen käännöksen osalta tutkijat pyrkivät osoittamaan, että hypotermian soveltaminen yhdistettynä nykyiseen sisäkorvaistutuksen hoitostandardiin jäännöskuulon säilyttämiseksi (steroidit) on tehokasta. Tutkijat olettavat, että hypotermiasovellus ei vaikuta negatiivisesti potilaiden CI-toimintaan (turvallisuus) ja parantaa varhaisia audiologisia tuloksia (tehokkuutta) verrattuna nykyiseen hoitotasoon CI-leikkauksissa. Tutkijat käyttävät kliinisesti merkittäviä leikkausta edeltäviä, intraoperatiivisia ja leikkauksen jälkeisiä toimenpiteitä potilasryhmissä, jotka saavat normaalia hoitoa ja hypotermiahoitoa.
Tätä kliinistä tutkimusta varten on rakennettu alustavat ja julkaistut prekliiniset tiedot sekä olemassa oleva järjestelmä ja laite. CI-leikkauksiin ehdotettu hoito- ja laitesuunnittelu edustaa ei-lääketieteellistä terapeuttista strategiaa jäännöskuulon säilyttämiseksi.

Kuvaus
Sisällyttämiskriteerit:
Potentiaaliset osallistujat tutkimukseen ovat potilaita, joilla on diagnosoitu vakava tai syvä kuulonmenetys ja jotka suunnittelevat sisäkorvaistutusta Miamin yliopistossa. Sisäkorvaistutteiden leikkaus, leikkausta edeltävä ja jälkihoito sekä implantin aktivointi ja ohjelmointi ovat osa osallistujien hoitotasoa. Jos osallistumiskriteerit täyttyvät (18-vuotiaat ja sitä vanhemmat, sama määrä miehiä ja naisia, syvä sensorineuraalinen kuulonmenetys (turvallisuustutkimuksessa n=6) ja jäännöskuulo (katso kriteerit alla) ennen leikkausta tehdystä ABR:stä (tehokkuustutkimus n). = 48), yhtä monta CI-laitetta kaikilta kolmelta valmistajalta ja englantia tai espanjaa puhuvia potilasta lähestyy tutkimushenkilöstö ja häntä pyydetään osallistumaan tutkimukseen.
