Apparaat voor intra-operatieve therapeutische hypothermie voor gehoorbehoud bij cochleaire implantatie
Het doel van deze interventionele klinische studie is het onderzoeken van het gebruik van milde therapeutische hypothermie voor het behoud van het restgehoor bij cochleaire implantaatchirurgie. De belangrijkste vragen die de proef wil beantwoorden zijn:
- Is milde therapeutische hypothermie veilig voor gebruik tijdens cochleaire implantatie?
- Is milde therapeutische hypothermie effectief bij het behouden van het restgehoor na cochleaire implantatie?
Deelnemers krijgen milde therapeutische hypothermietherapie tijdens een cochleaire implantaatoperatie. Onderzoekers zullen de resultaten van degenen die de therapie krijgen, vergelijken met die van een controlegroep (personen die geen therapie krijgen).

Cochleaire implantaten (CI) hebben de behandeling en prognose van patiënten met ernstig tot zeer ernstig perceptief gehoorverlies dramatisch veranderd. Met de recente opkomst van elektro-akoestische stimulatie (EAS) belooft de gecombineerde akoestische versterking bij lage frequenties met elektrische stimulatie van hoogfrequente cochleaire gebieden voordelen te bieden aan patiënten met zelfs een gedeeltelijk restgehoor door moeilijke luistersituaties zoals spraak in lawaai en muziek te verbeteren. waardering. Helaas resulteert trauma geassocieerd met de implantaatchirurgie in ontstekingen, mechanische en vasculaire schade en verlies van resterende sensorische haarcellen (HC) en neuronen. Als gevolg hiervan verliest meer dan 44% van de EAS-patiënten restgehoor na een CI-operatie en verslechtert het restgehoor bij de meerderheid van de patiënten met meer dan 30 dB in de loop van de tijd. Dit verlies aan restgehoor na een CI-operatie is de belangrijkste barrière geweest voor een uitgebreide toepassing van CI/EAS.
Deze klinische proef zal een nieuw mild therapeutisch hypothermieapparaat evalueren als een effectieve therapie om restgehoor na CI te behouden. Dit systeem vereist geen aanpassing van de huidige operatie- of CI-apparaten en zorgt voor gelokaliseerde, gecontroleerde milde hypothermie van de gevoelige structuren van het binnenoor. Met preklinisch gepubliceerde resultaten hebben de onderzoekers al aangetoond dat gelokaliseerde hypothermie tijdens CI-chirurgie zeer effectief en veilig is voor het behoud van restgehoor. Hier zullen de onderzoekers de implementatie onderzoeken van een therapie die ten goede komt aan patiënten die CI ondergaan door de postoperatieve audiologische resultaten te verbeteren. Uitgebreide eerdere commercialiserings- en ontwikkelingservaring, intellectuele eigendom en technische expertise, gekoppeld aan de klinische expertise van de onderzoekers op het gebied van Otolaryngologie en Audiologie, zullen het succesvolle klinische en commerciële resultaat van deze therapie garanderen.
Over het geheel genomen is het doel van deze klinische proef:
Valideer de klinische toepasbaarheid, veiligheid en werkzaamheid van het systeem en de apparaatconstructie voor het behoud van het restgehoor na een CI-operatie. Het apparaat en het hypothermietoedieningssysteem zullen worden getest bij volwassen patiënten die een CI-operatie ondergaan aan de Universiteit van Miami. Voor klinische vertaling willen de onderzoekers aantonen dat de toepassing van hypothermie in combinatie met de huidige zorgstandaard voor cochleaire implantatie voor restgehoorbehoud (steroïden) effectief is. De onderzoekers veronderstellen dat de toepassing van hypothermie geen negatieve invloed zal hebben op de CI-functie bij patiënten (veiligheid) en de vroege audiologische uitkomsten (werkzaamheid) zal verbeteren in vergelijking met de huidige zorgstandaard bij CI-operaties. De onderzoekers zullen klinisch relevante pre-operatieve, intra-operatieve en post-operatieve maatregelen gebruiken bij groepen patiënten die de standaardzorg en hypothermietherapie krijgen.
Voor deze klinische proef zijn voorlopige en gepubliceerde preklinische gegevens en een bestaand systeem en apparaat ontwikkeld. De voorgestelde therapie en het ontwerp van het apparaat voor CI-operaties vertegenwoordigen een niet-farmaceutische therapeutische strategie om restgehoor te behouden.

Inclusiecriteria:
Potentiële deelnemers aan het onderzoek zijn patiënten bij wie ernstig tot zeer ernstig gehoorverlies is vastgesteld en die van plan zijn een cochleaire implantatie te ondergaan aan de Universiteit van Miami. De cochleaire implantaatchirurgie, pre- en postoperatieve zorg, en activering en programmering van het implantaat maken deel uit van de zorgstandaard van de deelnemers. Als aan de inclusiecriteria wordt voldaan (leeftijd 18 jaar en ouder, gelijk aantal mannen en vrouwen, ernstig perceptief gehoorverlies (voor veiligheidsonderzoek n=6) en restgehoor (zie onderstaande criteria) van pre-chirurgische ABR (voor werkzaamheidsonderzoek n=6) =48), een gelijk aantal CI-apparaten van alle drie de fabrikanten en Engels- of Spaanssprekenden, zal de patiënt door het onderzoekspersoneel worden benaderd en worden gevraagd om aan het onderzoek deel te nemen.
Uitsluitingscriteria:
Onderwerpen jonger dan 18 jaar. Proefpersonen die niet voldoen aan de functionele gehoorvereisten.
