뇌졸중 환자의 하지 기능에 대한 고유수용성 신경근 촉진법과 수정된 제한 유도 운동 요법의 비교 효과
뇌졸중은 종종 약화, 불균형한 자세, 일상 활동의 독립성을 저해하는 운동성 감소로 특징지어지며, 전 세계적으로 장기적 장애의 주요 원인으로 확인되었습니다. 파키스탄에서는 뇌졸중 생존자의 수가 증가하고 있다는 보고가 있지만, 적절한 재활 치료에 대한 접근은 때때로 자원 부족과 인식 부재로 방해받고 있습니다. 보행 능력, 균형, 일상 기능 향상을 위해서는 하지 기능 회복이 필요하다고 여겨집니다. 수정 제한 유도 운동 치료(mCIMT)와 고유수용성 신경근 촉진법(PNF)은 운동 회복 향상을 위한 성공적인 물리치료 중재로 확인되었지만, 하지 기능에 미치는 영향에 대한 비교 데이터는 거의 없습니다.
이 연구의 목적은 PNF와 mCIMT의 효과를 검토하여 뇌졸중 후 하지 재활에 어떤 전략이 더 효과적인지 확인하는 것입니다. 연구 결과는 물리치료사에게 치료 선택을 위한 근거 기반 권고를 제공하여 뇌졸중 환자가 더 빠르게 회복하고 더 독립적으로 생활할 수 있도록 할 것으로 기대됩니다. 궁극적으로, 지역 사회 수준에서 장애 감소, 간병인 스트레스 완화, 뇌졸중 피해자의 삶의 질 전반적 향상 형태의 혜택이 예상됩니다.

설명
포함 기준:
만성 뇌졸중 환자 또는 뇌졸중 후 장기간 경과한 환자가 포함됩니다. 남성과 여성 모두 포함됩니다. 45세에서 75세 사이에 뇌졸중을 경험한 환자. 변형 랭킨 척도(mRS) 점수 2~3점으로 평가된 경도에서 중등도 장애.
영향을 받은 하지에서 시각 아날로그 척도 4점 이상의 통증.
제외 기준:
재발성 뇌졸중 인지 장애 (간이 정신 상태 검사 24점 미만) 말초 혈관 질환 (파킨슨병, 간질, 다발성 경화증 또는 척수 손상과 같은).
하지에 시술을 받지 않은 경우

실험적: PNF
먼저 10분 동안 핫팩과 TENS를 포함한 일반적인 준비 치료를 받아 이완을 촉진한 후, 기초 치료로서 능동적 관절가동범위 운동을 수행했습니다. 중재는 PNF 대각선 패턴, 리드믹 이니시에이션, 저항 훈련 및 다이나믹 리버설에 중점을 두었습니다.

초기에는 모든 참가자가 일반적인 이완 치료와 AROM을 포함한 기초 치료를 받게 됩니다.

모트리시티 지수(MI)는 뇌졸중 후 편마비 환자의 근력을 평가하기 위해 사용되는 순서 척도로, 1980년 데뮈리스(Demeurisse) 등이 처음 개발하였습니다.
상지의 세 가지 주요 움직임—핀치 그립(PG), 팔꿈치 굴곡(EF), 어깨 외전(SA)—과 하지의 세 가지 주요 움직임—발목 등쪽 굴곡(AD), 무릎 신전(KE), 고관절 굴곡(HF)—을 평가하며, 각각 의학연구위원회(MRC) 6점 척도로 등급을 매긴 후 수정된 가중 점수로 변환합니다.

뇌졸중 특이적 삶의 질(SS-QOL)은 뇌졸중 환자의 삶의 질을 평가하기 위해 설계된 표준화되고 신뢰할 수 있으며 질병 특이적인 설문지입니다.
12개 영역과 49개 항목으로 구성되어 있으며, 각 항목은 5점 리커트 척도로 점수가 매겨집니다.
영역에는 에너지, 언어, 이동성, 자기 관리, 가족 역할, 사회적 역할, 시력, 기분, 사고, 상지 기능, 업무 및 생산성이 포함됩니다.
SS-QOL은 널리 검증되었으며 뇌졸중 환자의 기능적 및 심리사회적 결과를 평가하는 효과적인 도구로 간주됩니다.뇌졸중 특이적 삶의 질(SS-QOL) 척도는 12개 영역에 걸쳐 49개 항목으로 구성됩니다.각 항목은 5점 리커트 척도(1-5)로 점수가 매겨집니다.
가능한 최소 총점은 49점이고 가능한 최대 총점은 245점입니다.점수가 높을수록 뇌졸중 환자의 삶의 질이 더 좋음을 나타냅니다.
