Beschrijving
Inclusiecriteria:
- Man of niet-zwanger/geen borstvoeding Vrouw ≥ 18 jaar. Vrouwen in de vruchtbare leeftijd moeten een negatieve zwangerschapstest hebben (binnen 7 dagen na de procedure);
- Gedocumenteerde stabiele angina pectoris Canadian Cardiovascular Society Classification (CCSC) 1-4, onstabiele angina pectoris met gedocumenteerde ischemie (Braunwald I-II) of gedocumenteerde stille ischemie;
- Linkerventrikelejectiefractie (LVEF) ≥ 25%;
- Patiënt is een aanvaardbare kandidaat voor PTCA, stenting en opkomende coronaire bypassoperatie (CABG);
- Patiënt is bereid geïnformeerde toestemming te geven en zich te houden aan vervolgbezoeken en testschema;
- Doellaesie is een de novo laesie in een natief kransslagadervat;
- Initiële stenose is ≥ 50% en < 100% volgens visuele schatting of kwantitatieve coronaire angiografie (QCA);
- Referentievatdiameter (RVD) is ≥ 2,5 en ≤ 3,25 voorafgaand aan predilatatie;
- Doellaesie is ≤18 mm lang en kan in zijn geheel worden behandeld met niet meer dan 1 enkele Lutonix-katheterballon en 1 enkele BMS;
- Guidewire kan de laesie passeren en voorafgaand aan registratie in een distaal vat worden geplaatst;
- Inschrijving toegestaan na succesvolle behandeling van 1 niet-onderzoekslaesie in een enkel ander niet-onderzoeksvat (niet in hetzelfde vasculaire gebied als de onderzoekslaesie). Succesvolle behandeling wordt gedefinieerd als ≤ 30% reststenose met trombolyse bij myocardinfarct (TIMI) Graad III flow en geen bewijs van dissectie.
Uitsluitingscriteria:
- Geschiedenis van een beroerte in de afgelopen 6 maanden;
- Voorgeschiedenis van myocardinfarct (MI) of trombolyse binnen 72 uur na randomisatie;
- Voorafgaande vasculaire brachytherapie;
- Oncontroleerbare allergieën voor proceduremedicatie, materialen of contrastmiddel;
- angiografisch bewijs van trombus of dissectie in het doelbloedvat;
- Interventie van een andere coronaire laesie ≤ 60 dagen voor indexproceduredag of geplande volgende indexprocedure;
- Doellaesie is gepland om te worden behandeld met iets anders dan PTCA en stent (d.w.z. snijballon, atherectomie, vasculaire brachytherapie (VBT), enz.);
- Doellaesie bevindt zich in de linkerhoofdader en heeft overmatige verkalking of kronkeligheid of omvat vertakkingsziekte van vat ≥ 2,5 mm;
- Bekende gevoeligheid of heeft paclitaxel of een ander antimitogeen middel gekregen binnen 12 maanden voorafgaand aan de behandeling van het doelbloedvat;
- Patiënt heeft een eerdere ingreep (PTCA, stent, enz.) van het doelkransvat;
- Elke medische aandoening, naar de mening van de onderzoekers, die de patiënt zou moeten uitsluiten van de studie of patiënt heeft een levensverwachting van minder dan 24 maanden;
- Bekend creatinekinase-MB (CKMB) > 2x bovengrens van normaal (ULN) of positief troponine;
- Creatinine > 2,0 mg/dl;
- Leukocyten < 3500/ml;
- Bloedplaatjes < 100.000 ml of > 750.000 ml;
- momenteel warfarine gebruikt of moet hervatten;
- De patiënt is gecontra-indiceerd voor plaatjesaggregatieremmende therapie of moet worden teruggetrokken voor een geplande procedure;
- De proefpersoon neemt momenteel deel aan een ander onderzoek naar een geneesmiddel of apparaat.
