Beschrijving
Inclusiecriteria:
Patiënten die zijn ingeschreven, zijn mannelijk of vrouwelijk, zijn ten minste 18 jaar oud en hebben de diagnose spastische hemiplegie en overmatige plantairflexie, waaronder het volgende:
- Diagnose van een beroerte of traumatisch hersenletsel meer dan 6 maanden voorafgaand aan deelname aan deze studie
- Verminderde AROM en PROM bij dorsaalflexie van de enkel
- Mogelijkheid om veilig 20 voet te lopen zonder een enkel-voet-orthese (EVO)
- Onvermogen om te lopen bij het eerste hielcontact
- Geen eerdere BTX-behandeling binnen 6 maanden
- R1 van -10° of hoger (met behulp van de Tardeiu-beoordelingsmethode)
- Gemiddelde Ashworth-schaaltest ≥3 voor plantairflexoren
Uitsluitingscriteria:
- Bell's verlamming
- Virale encefalitis
- Spierdystrofie
- Multiple sclerose
- Eerdere hielkoordverlenging of peestransferprocedure in de voet/enkel
- Proefpersonen met een vaste contractuur van de enkel
- Vrouwelijke proefpersonen die zwanger zijn (positieve zwangerschapstest in urine), die een baby hebben die ze borstvoeding geven, of in de vruchtbare leeftijd zijn en geen betrouwbare methode van anticonceptie toepassen
- Bloedingsstoornissen
- Diagnose van een ziekte die de neuromusculaire functie kan verstoren (bijv. myasthenia gravis, Lambert-Eaton myasthenisch syndroom, amyotrofische laterale sclerose)
- Onderwerpen die momenteel aminoglycoside-antibiotica, curare-achtige middelen of andere middelen gebruiken die de neuromusculaire functie kunnen verstoren
- Onderwerpen met ernstige zwakte of atrofie van de spieren in de doelgebieden van injectie
- Actieve systemische infectie of infectie op de injectieplaats
- Allergie of gevoeligheid voor botulinetoxine A

Alle patiënten zullen worden behandeld met de huidige zorgstandaard, waaronder onabotulinumtoxine

Experimenteel: Dynasplint
Patiënten in de experimentele Dynasplint-groep zullen worden behandeld met de huidige zorgstandaard, inclusief onabotulinumtoxine, en de enkeldorsiflexie Dynasplint gebruiken
|
Dynamisch spalken maakt gebruik van de protocollen van Low-Load Prolonged Stretch (LLPS) met gekalibreerde instelbare spanning om Total End Range Time (TERT) te verlengen en contractuur te verminderen.
De Dynasplint- of "experimentele" groep zal deze therapie toevoegen aan hun standaardbehandelingsregime
