Beschrijving
Inclusiecriteria:
- Leeftijd ≥ 50 jaar op de datum van toestemming.
- LVEF >50% zoals bepaald op beeldvormend onderzoek gemeten in de laatste 6 kalendermaanden voorafgaand aan of op de toestemmingsdatum.
- In het ziekenhuis opgenomen voor HF in de afgelopen 12 kalendermaanden voorafgaand aan de toestemmingsdatum.
- Gestabiliseerde patiënten, gedefinieerd als zijnde van alle IV-therapieën af gedurende ten minste 24 uur voorafgaand aan de toestemmingsdatum.
- Paroxysmaal AF gediagnosticeerd in de afgelopen 6 kalendermaanden of vroeg aanhoudend AF, gedefinieerd als een AF-episode die langer dan 7 dagen maar minder dan 6 kalendermaanden duurt voorafgaand aan de toestemmingsdatum.
- Geïndiceerd voor orale antistolling volgens de huidige richtlijnen (CHA2DS2-VASc ≥ 2 bij mannen of ≥ 3 bij vrouwen op het moment van toestemming)*
- Patiënt met CIED in situ op het moment van toestemming of gepland om ICM-implantatie te ondergaan voor AF-behandeling binnen 2 kalendermaanden na randomisatie.
Uitsluitingscriteria:
- Eerdere katheter of chirurgische ablatie van AF op enig moment in het verleden.
- Langdurige aanhoudende AF (>1 jaar voorafgaand aan de toestemmingsdatum) of permanente AF.
- Chronische longziekte waarvoor thuiszuurstof of orale/IV steroïdtherapie nodig was in de afgelopen 12 kalendermaanden voorafgaand aan de toestemmingsdatum.
- Actieve infectie op het moment van toestemming, maar kan later na effectieve behandeling opnieuw worden overwogen voor inschrijving.
- Linker atriale grootte > 50 mm zoals gemeten door middel van echo in de afgelopen 6 kalendermaanden voorafgaand aan de toestemmingsdatum.
- Onbehandelde obstructieve slaapapneu ooit in het verleden.
- Myocardinfarct in de afgelopen 3 kalendermaanden voorafgaand aan de toestemmingsdatum.
- Coronaire bypassoperatie (CABG) in de afgelopen 3 kalendermaanden voorafgaand aan de toestemmingsdatum.
- Beroerte in de afgelopen 3 kalendermaanden voorafgaand aan de toestemmingsdatum.
- Intracardiale trombus die het vermogen om AF-ablatie te ondergaan uitsluit op het moment van toestemming, maar kan later na effectieve behandeling opnieuw worden overwogen voor inschrijving.
- Infiltratieve cardiomyopathie (sarcoïde, amyloïde) op enig moment in het verleden.
- Actieve myocarditis op het moment van toestemming, maar kan later na effectieve behandeling opnieuw worden overwogen voor inschrijving.
- Hypertrofische cardiomyopathie op enig moment in het verleden.
- Bekende pericardiale vernauwing.
- Ongecontroleerde hypertensie (SBP > 160 mmHg) op het moment van toestemming, maar kan later na effectieve behandeling opnieuw worden overwogen voor inschrijving.
- Onbehandelde hypothyreoïdie of hyperthyreoïdie, maar kan later na effectieve behandeling opnieuw worden overwogen voor inschrijving.
- Zwangerschap of borstvoeding.
- Valvulaire AF of aanwezigheid van een kunstklep.
- Dialyse ondergaat of een gevorderde nierfunctiestoornis heeft (eGFR <30 ml/min/m2), gedefinieerd als 3 keer de bovengrens van ASAT of ALAT gedurende de afgelopen 6 kalendermaanden.
- Contra-indicatie voor antistolling.
- Stofwisselingsstoornissen (bijv. nier-/leverfalen, verstoring van de elektrolytenbalans, enz.), het verbieden van studie (EP) studie en ablatie.
- Cognitieve beperking.
- Levensverwachting < 1 jaar na toestemmingsdatum.
- Niet bereid om te voldoen aan alle door het studieprotocol vereiste tests.
- Geïnformeerde toestemming niet willen of kunnen geven.

Proefpersonen ondergaan vroege RF-ablatie van AF met behulp van CARTO 3 en een Thermocool ST SF-ablatiekatheter
|
Ablatie van AF is standaardzorg bij de behandeling van patiënten met paroxismaal of aanhoudend AF.
De meest voorkomende triggers die AF initiëren, komen voort uit longaderen (PV) en succesvolle elektrische isolatie van PV's kan de last van AF aanzienlijk verminderen.
Daarom is het belangrijkste doel bij patiënten die AF-ablatie ondergaan het bereiken van PV-isolatie.
