Een beoordeling van chirurgische technieken voor de behandeling van cervicale spondylotische myelopathie
Het primaire doel van deze studie is om anterieure en posterieure chirurgische benaderingen bij de behandeling van CSM te vergelijken in termen van chirurgische complicaties en neurologische, functionele, ziektespecifieke en kwaliteit van leven uitkomstmaten.
Secundaire doelen zijn het kwantificeren van de mate van verandering voor en na de operatie met betrekking tot dezelfde uitkomstmaten; om te bepalen of er verschillen zijn in uitkomsten tussen posterieure chirurgische technieken (d.w.z. laminectomie met fusie of laminoplastie) en onderzoek de relatie tussen baseline MRI en baseline en follow-up neurologische en functionele resultaten.

Vernauwing van het wervelkanaal door osteofyten, verbening van het posterieure longitudinale ligament of uitpuilen van een grote centrale schijf kan het cervicale ruggenmerg samendrukken en de spinale zenuwwortels raken, resulterend in nekpijn en verschillende gradaties van neurologische symptomen en stoornissen.2 In ernstige gevallen kan dit leiden tot stenose van de cervicale wervelkolom, met symptomen van de bovenste motorneuronen in de onderste extremiteit en symptomen van de onderste motorneuronen in de bovenste extremiteit tot gevolg. Wanneer conservatieve maatregelen zoals tractie, halskraag en houdingsoefeningen neurologische progressie niet kunnen voorkomen, kan een operatie aangewezen zijn.
Er zijn verschillende chirurgische benaderingen en procedures beschikbaar, en de optimale behandelingskeuze blijft controversieel. Chirurgische procedures die zijn ontworpen om het ruggenmerg te decomprimeren en, in sommige gevallen, de wervelkolom te stabiliseren, zijn succesvol gebleken, maar er is een aanhoudend percentage patiënten dat niet verbetert met chirurgische interventie.3 Bovendien kunnen de mogelijke complicaties van een operatie voor CSM afhangen van de verschillende methoden van chirurgische behandeling. Historisch gezien werd cervicale laminectomie, een posterieure benadering, beschouwd als de standaard chirurgische behandeling van CSM. In de afgelopen 20 jaar wordt echter steeds meer erkend dat laminectomie zonder fusie niet geschikt is voor alle patiënten en kan leiden tot instabiliteit en misvorming. Vanwege de instabiliteit die wordt veroorzaakt door laminectomieën, zijn er alternatieve chirurgische benaderingen ontwikkeld, zoals anterieure benaderingen van de wervelkolom en laminoplastiek, die in de loop der jaren steeds populairder zijn geworden.3 Er moet rekening worden gehouden met een reeks factoren bij het beslissen welke chirurgische techniek moet worden gebruikt. Chirurgen worden vaak uitgedaagd om de meest geschikte techniek te bepalen, omdat er beperkte informatie is over de vraag of er een verschil is tussen chirurgische procedures in termen van klinische en radiografische resultaten, in postoperatieve complicaties en in functionele en kwaliteit van leven-uitkomsten. Behandelingsmethoden omvatten conservatieve en chirurgische behandeling. Bij chirurgisch behandelde patiënten kan een anterieure of posterieure benadering worden toegepast. Onder degenen die posterieur worden behandeld, zijn laminoplastie of laminectomie met fusie gebruikelijke chirurgische technieken. Met verschillende zorgstandaarden beschikbaar voor deze populatie, is een beter begrip van de overeenkomstige positieve en negatieve uitkomsten met betrekking tot klinische en patiëntgerichte uitkomsten gerechtvaardigd.

Inclusiecriteria:
- Patiënten ondergaan een operatie voor symptomatische cervicale spondylotische myelopathie met een of meer van de volgende symptomen: gevoelloze, onhandige handen, loopstoornissen, bilaterale armparasthesie, verschijnselen van L'Hermitte
- en een of meer van de volgende symptomen: motorische stoornissen in de corticospinale verdeling, atropie van intrinsieke handspieren, hyperflexie, positief teken van Hoffman, opgaande voetzoolreacties, spasticiteit van de onderste ledematen, brede onstabiele manier van lopen
Uitsluitingscriteria:
- Asymptomatische cervicale spondylotische myelopathie
- eerdere operatie voor CSM
- Actieve infectie
- Neoplastische ziekte
- Reumatoïde artritis
- Spondylitis ankylopoetica
- Trauma
- Gelijktijdige lumbale stenose
- Niet doorverwezen voor chirurgisch consult
- Deelnemen aan andere onderzoeken of waarschijnlijk niet aanwezig zijn bij follow-ups
