자궁 경부 척추 골수증 치료를 위한 수술 기법의 평가 (CSM)
이 연구의 주요 목적은 외과적 합병증과 신경학적, 기능적, 질병 특이적 및 삶의 질 결과 측정 측면에서 CSM 치료에서 전방 및 후방 외과적 접근법을 비교하는 것입니다.
두 번째 목표는 동일한 결과 측정에 관한 수술 전후의 변화량을 정량화하는 것입니다. 후방 수술 기법(즉, 융합 또는 추궁 성형술을 사용한 후궁 절제술) 기준선 MRI와 기준선 및 후속 신경학적 및 기능적 결과 사이의 관계를 조사합니다.

상세 설명
골조직에 의한 척추관의 협착, 후종인대의 골화 또는 큰 중앙 디스크의 팽창은 경추 척수를 압박하고 척수 신경근을 압박하여 목 통증과 다양한 정도의 신경학적 증상 및 장애를 유발할 수 있습니다.2 심할 경우 경추의 협착증으로 이어져 하지의 상부 운동 신경 증상과 상지의 하부 운동 신경 증상이 나타날 수 있습니다. 견인, 경추 보호대 및 자세 운동과 같은 보존적 조치가 신경학적 진행을 예방하지 못하는 경우 수술이 필요할 수 있습니다.
다양한 외과적 접근과 시술이 가능하며 최적의 치료 선택은 여전히 논란의 여지가 있습니다. 척수를 감압하고 경우에 따라 척추를 안정시키기 위해 고안된 외과적 절차는 성공적인 것으로 나타났지만, 외과적 개입으로 호전되지 않는 환자의 비율이 지속적으로 존재합니다.3 또한 CSM 수술의 잠재적 합병증은 다양한 수술 관리 방법에 따라 달라질 수 있습니다. 역사적으로 후방 접근 방식인 자궁 경부 추궁 절제술은 CSM의 표준 수술 치료로 여겨져 왔습니다. 그러나 지난 20년 동안 융합 없는 추궁 절제술이 모든 환자에게 적합하지 않으며 불안정성과 기형을 초래할 수 있다는 인식이 점점 더 높아지고 있습니다. 추궁 절제술로 인한 불안정성으로 인해 척추 전방 접근법과 추궁 성형술과 같은 대체 수술 접근법이 개발되었으며 수년에 걸쳐 인기가 높아지고 있습니다.3 사용할 수술 기법을 결정할 때 다양한 요소를 고려해야 합니다. 임상 및 방사선학적 결과, 수술 후 합병증 비율, 기능 및 삶의 질 결과 측면에서 수술 절차 간에 차이가 있는지 여부에 대한 정보가 제한되어 있기 때문에 외과의는 가장 적절한 기술을 결정하는 데 어려움을 겪는 경우가 많습니다. 치료 방법에는 보존적 치료와 수술적 치료가 있습니다. 외과적으로 관리되는 환자의 경우 전방 또는 후방 접근법을 사용할 수 있습니다. 후방으로 관리되는 이들 중 후궁 성형술 또는 융합을 통한 추궁 절제술이 일반적인 수술 기법입니다. 이 인구에 대해 사용할 수 있는 몇 가지 치료 표준을 통해 임상 및 환자 중심 결과와 관련하여 해당하는 긍정적 및 부정적인 결과를 더 잘 이해해야 합니다.
