Reduced Intensity Conditioning (RIC)-regime voor patiënten met niet-kwaadaardige aandoeningen (RIC)
Dit is een fase II-pilootstudie om de implantatie en toxiciteit van patiënten met niet-kwaadaardige ziekten te evalueren met behulp van een conditioneringsregime met verminderde intensiteit in de setting van allogene transplantatie voor niet-kwaadaardige ziekten. Beenmerg- of navelstrengbloed is acceptabel als stamcelbron.
Onlangs zijn conditioneringsregimes met verminderde intensiteit (RIC) gebruikt voor zowel volwassen patiënten met leukemieën als pediatrische patiënten met niet-kwaadaardige ziekten. Deze regimes worden beter verdragen, wat resulteert in minder transplantatiegerelateerde morbiditeit en mortaliteit. Stabiel gemengd chimerisme, hoewel onvoldoende voor de uitroeiing van leukemieën, kan voldoende zijn om patiënten met niet-kwaadaardige ziekten te genezen.

Er zijn twee conditioneringsregimes in dit protocol voor kinderen >6 maanden. Alemtuzumab (Campath), Fludarabine en Melphalan worden gebruikt. De regimes verschillen door de timing en dosering van Alemtuzumab (Campath). De twee timings zijn distaal en intermediair.
- Distale campath wordt 22 dagen voorafgaand aan de allogene transplantatie gestart.
- Tussenliggende campath wordt gestart 14 dagen voorafgaand aan allogene transplantatie.
Het conditioneringsregime voor kinderen met immunodeficiënties <6 maanden laat melfalan weg en vervangt twee dagen busulfan. Dit regime wordt met succes gebruikt in het Verenigd Koninkrijk en is succesvol geweest bij een baby van 3 maanden oud in het Children's Hospital of Philadelphia (CHOP) die werd geënt met een haplo-identieke donor.

- Leeftijd >6 maanden - 25 jaar
Ziekten die in aanmerking komen voor Distale Alemtuzumab:
- Immunodysregulatie polyendocrinopathie enteropathie X-linked (IPEX) syndroom
- Sikkelcelziekte
- Thalassemie majeur
- Beenmergfalen
Ziekten die in aanmerking komen voor intermediair Alemtuzumab
- Hemofagocytische lymfohistiocytose andere macrofaagactiveringssyndromen, ernstige Langerhans-histiocytose
- Ernstige gecombineerde immuundeficiëntie, adenosinedeaminasedeficiëntie, algemene variabele immunodeficiëntie
- Wiskott-Aldrich-syndroom
Orgaancriteria:
- Cardiaal: echocardiogram verkortingsfractie >27%
- Nier: serumcreatinine minder dan 1,5 keer de bovengrens van normaal voor leeftijd
- Lever: leverfunctietesten moeten minder dan 5 keer de bovengrens van normaal zijn
- Geen actieve infecties

Experimenteel: Reduced-Intensity Conditioning (RIC) HSCT
Participants receive reduced-intensity conditioning with alemtuzumab-, fludarabine-, and melphalan-based regimens, varying by timing of alemtuzumab (distal or intermediate).
Regimens are based on established protocols used in non-malignant disorders.
Alemtuzumab is administered prior to other agents to reduce risk of declining donor chimerism.
Participants then undergo allogeneic stem cell transplantation with standard post-transplant care.

A pre-transplant conditioning approach designed to provide sufficient immunosuppression to enable donor cell engraftment while minimizing regimen-related toxicity compared to myeloablative conditioning.
Regimens are adapted based on patient age and underlying disease characteristics using the following agents: Campath, Fludarabine, Melphalan, Cyclosporine, Cellcept (MMF).
