Studie van APRV en ICP bij patiënten met ernstig traumatisch hersenletsel
De onderzoekers zullen een observationele cross-over studie uitvoeren. De onderzoekers streven ernaar om 50 deelnemers te rekruteren met ernstig traumatisch hersenletsel (TBI) die intracraniale druk (ICP) monitoring vereisen tijdens hun verblijf op de Neuro Trauma ICU in het R Adams Cowley Shock Trauma Center.
Over het algemeen zullen deelnemers tijdens de onderzoeksperiode gemiddeld ongeveer 6-8 uur worden gevolgd. De onderzoekers voorzien geen behoefte aan langdurige monitoring tijdens hun ziekenhuisverblijf of posthospitaalperiode.

Elke deelnemer krijgt bij opname in eerste instantie een primaire vorm van mechanische beademing zoals bepaald door de behandelend trauma-intensivisten. 12-18 uur na rekrutering begint de continue monitoring van de ICP en hemodynamische status van de deelnemers om de basisgegevens van de deelnemers gedurende 30 minuten te verzamelen.
Vervolgens ondergaan de deelnemers gedurende 2 uur mechanische beademing met een laag ademvolume (LOTV), die dient als controlemodus van de beademing, voordat er gedurende 30 minuten wordt teruggeschakeld naar de primaire beademingsmodus. Vervolgens worden patiënten gedurende 2 uur op Airway Pressure Release Ventilation (APRV) geplaatst. Tijdens het ontvangen van APRV worden de intracraniale druk en hemodynamische status van deelnemers continu gecontroleerd en geregistreerd voor vergelijking en analyse. Na 2 uur APRV worden patiënten teruggeschakeld naar hun vorige beademingsmodus en worden er gedurende nog eens 30 minuten meer gegevens verzameld.

- Mechanisch geventileerd
- Klinisch stabiel zoals bepaald door de aanwezige Critical Care

Airway Pressure Release Ventilation (APRV) is een vorm van mechanische beademing die schakelt tussen hoge (PHigh) en lage (PLow) continue positieve luchtwegdruk terwijl spontane ademhaling in beide fasen mogelijk is.
Alveolaire rekrutering en oxygenatie vinden plaats tijdens PHigh, terwijl ventilatie plaatsvindt tijdens korte releases naar PLow.

Na inschrijving en het verzamelen van de intracraniële druk en hemodynamische status gedurende 30 minuten ondergaan de deelnemers mechanische beademing met een laag teugvolume (LOTV), die dient als controlemodus van de beademing.
LOTV wordt het meest gebruikt voor traumapatiënten met longletsel.
LOTV levert zuurstof in kleinere hoeveelheden, zonder de longen te overbelasten
