Effectiviteit van een gecombineerd trainingsprogramma voor intradialyse op het niveau van invaliditeit en fysieke prestaties. (PTH-CKD0113)
Werkzaamheid van een intradialyse-gecombineerd trainingsprogramma voor spierkracht en cardiorespiratoire capaciteit op het niveau van invaliditeit en fysieke prestaties bij patiënten met chronisch nierfalen op een niervervangende therapie-eenheid.

Doel Het evalueren van de impact van het intradialytische fysieke trainingsprogramma op het niveau van invaliditeit en fysieke prestaties bij patiënten die hemodialyse ondergaan.
Materialen en methoden 18 patiënten die hemodialyse ondergingen, van wie de leeftijd varieerde van 54 tot 84 (10 vrouwen), werden in het onderzoek opgenomen. Negen van deze patiënten namen deel aan een gecombineerd trainingsprogramma, dat 12 weken duurde, de trainingssessies vonden plaats binnen elke dialysesessie, de werksessie omvatte een opwarmperiode, aerobe oefeningen met pedalen of krachtoefeningen op basis van vrij gewicht afhankelijk van de trainingsfase. De Borg-schaal en de hartslagreserve werden gebruikt om de trainingsintensiteit te regelen. Het invaliditeitsniveau werd aan het begin en einde van het onderzoek geëvalueerd door middel van de World Health Organization Disability Assessment Schedule II, de zes minuten looptest (6MWT), quadricepskracht, Kt/V, serumspiegels van kalium en creatinine.
Uitkomsten Eén proefpersoon die tot de experimentele groep behoorde, voltooide de training niet vanwege problemen met de veneuze toegang tijdens de eerste maand van de training. 8 patiënten in de experimentele groep vertoonden significante verbeteringen in de 6MWT, quadricepskracht en afname in het niveau van invaliditeit. In de controlegroep werden geen veranderingen waargenomen. Er werden geen significante veranderingen gevonden in de serumspiegels van kalium en creatine.
Conclusies Een intradialytisch fysiek trainingsprogramma zorgde voor een substantiële vermindering van het invaliditeitsniveau en een verbetering van de aërobe capaciteit en spierkracht. De studie werd gefinancierd door het uitbreidingskantoor van de Austral University of Chile.

Beschrijving
Inclusiecriteria:
- Patiënten die een hemodialyseproces ondergaan via een arterioveneuze fistel in de dialyse-eenheid Valdivia.
- Die zijn tussen de 60-80 jaar.
- Therapie Dialyse gedurende een periode van meer dan 3 maanden.
- Dien 24 of meer punten in op de Mini-mental state-test.
Uitsluitingscriteria:
- Onvermogen om bestellingen te begrijpen.
- Dyspnoe in rust.
- Een maand voor de operatie opgenomen in het ziekenhuis.
- Ziekten Cerebrovasculaire voorbijgaande ischemische aanvallen.
- Amputatie van de onderste ledematen (enkelaanwezigheid) dat deze orthopedische prothese.
- Syndroom Acuut coronair.
- Congestief hartfalen of ernstige longontsteking.
- Patiënten met ongecontroleerde diabetes farmacologisch of niet binnen de juiste glucosenormen voor training.
- Patiënten met ongecontroleerde hypertensie farmacologisch of niet binnen de juiste bloeddruknormen voor training.
- Aanhoudende hyperkaliëmie.
- Angina in rust.
- Longziekte die bijwerkingen veroorzaakt.
