Biologisch afbreekbaar versus titanium spijkeren in onderarmschachtbreuken bij kinderen
Elastisch stabiele intramedullaire vernageling van onderarmschachtfracturen door titanium vergeleken met biologisch afbreekbare nagels: resultaten van een prospectief gerandomiseerd onderzoek bij kinderen met ten minste twee jaar follow-up
Er zijn nadelen aan Elastic Stable Intramedullary Nailing (ESIN) van onderarmschachtfracturen, zoals irritatie van zacht weefsel en de noodzaak om het implantaat te verwijderen. Er is een nieuwe mini-invasieve techniek van intramedullair spijkeren met biologisch afbreekbaar materiaal (BESIN) ontwikkeld. De nagels worden in de medullaire holte getikt en op hun plaats gelaten. Het doel van deze studie was om de BESIN-techniek te vergelijken met ESIN.
Dit is een prospectieve, gerandomiseerde, gecontroleerde klinische studie met patiënten (leeftijd 5-15 jaar) die operatieve behandeling nodig hadden voor onderarmschachtfracturen in twee pediatrische traumacentra in Finland. De patiënten werden gerandomiseerd om te worden behandeld met BESIN of ESIN. Dertien patiënten waren nodig voor elke groep, volgens poweranalyse, maar in totaal waren er 35 ingeschreven voor potentiële drop-outs. Biologisch afbreekbare polylactide-co-glycolide (PLGA) nagels (ActivaNail ®, Bioretec ltd, Finland) werden gebruikt bij 19 en titanium nagels (TEN ®, Synthes ltd, VS) bij 16 patiënten. Pijn en het bewegingsbereik (ROM) van onderarm, elleboog en pols waren primaire uitkomsten. Röntgenfoto's en mogelijke complicaties werden geanalyseerd van alle en magnetische resonantiebeelden (MRI) van een willekeurig geselecteerde subgroep in de BESIN-groep (N=13).

Beschrijving
Inclusiecriteria:
- die lijdt aan een onderarmschachtfractuur met één of beide botten
- kindpatiënt, leeftijd van 5 tot 15 jaar
- chirurgische fixatie nodig, fractuur is instabiel
- cast immobilisatie is niet rigide genoeg voor de fractuurbehandeling
Uitsluitingscriteria:
- open fracturen met significant letsel aan zacht weefsel
- pathologische fracturen
- als de patiënt eerder een fractuur of infectie in dezelfde onderarm heeft gehad
- patiënten met metabole botziekten, systematische ziekte
- patiënt gebruikt de medicatie die de botkwaliteit beïnvloedt
- weerstand tegen infectie
- breuken ouder dan 7 dagen
