Het behalen van PCI-tijddoelen wordt beïnvloed door meerdere factoren, zoals kenmerken van de patiënt, agentuurfactoren en elementen van de organisatiecultuur van Emergency Medical Services (EMS). Organisatiecultuur wordt gedefinieerd als een reeks gedeelde waarden, overtuigingen en aannames binnen een organisatie die van invloed zijn op hoe mensen binnen die organisatie zich gedragen. Verschillen in organisatiecultuur tussen ziekenhuizen zijn in verband gebracht met zowel cardiovasculaire mortaliteit als ziektespecifieke uitkomsten. Hoewel nog niet grondig bestudeerd in de preklinische omgeving, is het waarschijnlijk dat de organisatiecultuur bijdraagt aan verschillen in tempo en manier om interventies uit te voeren. Preklinische prestatieverantwoordelijkheid en -cultuur zijn besproken door experts in EMS-tijdschriften, maar zijn nooit formeel onderzocht.
EMS STEMI-protocollen die direct transport naar een PCI-compatibel ziekenhuis en pre-ziekenhuis PCI-centrumactivering omvatten, verbeteren de patiëntresultaten. Mislukte ECG-transmissie, vertraagde activering van het PCI-centrum en cardiogene shock hebben een negatieve invloed op de PCI-tijdstatistieken en patiëntresultaten. De impact van PCI-vertragingen in de landelijke omgeving is niet specifiek bestudeerd. Daarnaast zijn er factoren op bureauniveau, zoals ambulances per hoofd van de bevolking, het aantal satellietstations, kilometers snelweg, die waarschijnlijk van invloed zijn op het vermogen van de ambulancedienst om kortere PCI-tijden te bereiken voor de STEMI-patiënten waarvoor ze zorgen. Dit voorstel zal gemengde methoden gebruiken om voorheen niet-gemeten componenten van de organisatiecultuur, structuur, zorgprocessen en patiëntomgeving van landelijke EMS-bureaus te identificeren die waarschijnlijk de PCI-tijd en patiëntresultaten beïnvloeden. Bovendien zal dit project best practices identificeren die kunnen worden getest als nieuwe interventies en kunnen worden geïmplementeerd in landelijke EMS-agentschappen om de STEMI-tijdstatistieken te verbeteren en daardoor de morbiditeit en mortaliteit van patiënten te verminderen.

Register: Patiënten die zijn opgenomen in de registergegevens zullen een all-comers-populatie zijn van patiënten die lijden aan STEMI en tussen 2016-2019 door een van de 14 geïdentificeerde landelijke EMS-bureaus naar een primaire PCI zijn vervoerd.
Interviews met sleutelinformanten: deelnemers zijn EMS-directeuren, EMS Medical-directeuren, EMS-trainingsfunctionarissen, paramedici of EMT-bemanningspartners bij de best en slechtst presterende landelijke EMS-agentschappen, zoals bepaald door de STEMI-registratiegegevens.
Enquêtes onder belanghebbenden: De respondenten van de enquête zijn EMS-directeuren, EMS Medical-directeuren, EMS-trainingsfunctionarissen, paramedici of EMT-bemanningspartners bij een van de 14 lokale EMS-agentschappen die als landelijk zijn aangemerkt.

STEMI-register 2016-2019
Retrospectieve gegevens zullen worden verzameld om een goed gekarakteriseerd register te ontwikkelen van patiënten die van 2016-2019 zijn behandeld door een van de 14 lokale landelijke EMS-instanties en zijn vervoerd naar een faciliteit die in staat is om percutane coronaire interventie uit te voeren.
Dit register zal worden gebruikt om de tijd tot PCI-prestaties voor elk van de EMS-agentschappen te bepalen.
De tijd wordt aangepast voor de patiëntafstand tot een PCI-centrum met behulp van een lineair gemengd model met een willekeurig effect voor het centrum en een vast effect voor de afstand.
Dit proces maakt kwalitatieve methoden mogelijk om de organisatiecultuur, structuur en klinische processen te identificeren die van invloed zijn op de STEMI-zorg van de twee best en slechtst presterende landelijke EMS-agentschappen.

Interviews met sleutelinformanten
Na het identificeren van de twee best en slechtst presterende landelijke EMS-bureaus in het STEMI-register van 2016-2019, zullen sleutelmedewerkers van elk van die bureaus worden aangeworven om deel te nemen aan semi-gestructureerde interviews met sleutelinformanten.
De interviews zullen de huidige klinische zorg, de organisatiecultuur en mogelijkheden voor verbetering beoordelen.

Doel 1 zal worden bereikt door de ontwikkeling van een STEMI-gegevensregister voor 2016-2019, dat zal worden gebruikt om goed en slecht presterende landelijke EMS-agentschappen te identificeren.
Na het identificeren van deze agentschappen, zullen sleutelinformanten deelnemen aan interviews die zijn ontworpen om facilitators en belemmeringen voor het bereiken van STEMI-tijdstatistieken te beoordelen.
