Zuurstoftherapie is van onbetwist belang bij de zorg voor ernstig zieke patiënten om secundaire complicaties in verband met hypoxemie te voorkomen. Hoewel het routine is, kan de praktijk van overmatige overoxygenatie schadelijk zijn. Er werd een expertpanel bijeengeroepen en er ontstond een sterke consensus om normoxemie aan te pakken bij een zuurstofverzadigingsbereik (SpO2) van 90-96%, een arterieel zuurstofbereik (PaO2) van 60-100 mmHg (indien van toepassing) en een fractie van ingeademde zuurstof (FiO2) van 21% voor mechanisch beademde patiënten of kamerlucht voor niet-mechanisch beademde patiënten.
Specifiek doel: Het doel van deze studie is om de effectiviteit te bepalen van een multimodale educatieve interventie om het extra zuurstofgebruik bij ernstig gewonde patiënten te verminderen. Onderzoekers zullen ook de veiligheid en klinische effectiviteit van het meer gerichte gebruik van zuurstoftherapie evalueren.
Hypothesen: Klinische inspanningen om een multimodale onderwijsinterventie door te voeren zullen:
- Verbeter het deel van de tijd dat wordt doorgebracht binnen de streefnormoxemiedrempels (zuurstofverzadiging [SpO2] 90-96% en/of arteriële zuurstof [PaO2] 60-100 mmHg [indien van toepassing]
- Beperk het gebruik van overmatige aanvullende zuurstof
- Verminder blootstelling aan hyperoxemie zonder een substantiële toename van hypoxemische episodes of bijwerkingen

Inclusiecriteria:
- Acuut gewonde patiënten die voldoen aan de criteria voor opname in het staats- of nationale traumaregister
- Opname op de chirurgische/trauma-IC binnen 24 uur na aankomst in het ziekenhuis
Uitsluitingscriteria:
- Leeftijd <18 jaar
- Gevangenen
- Bekende zwangerschap
- Overgeplaatste patiënten die niet via de spoedeisende hulp zijn opgenomen

Geen tussenkomst: Pre-implementatie
De controlegroep (pre-implementatie) bestaat uit traumapatiënten die zijn opgenomen op de chirurgische/trauma-ICU tijdens de controleperiode van het getrapte implementatieproces op de locatie (maximaal 22 maanden).

Experimenteel: Post-implementatie gericht op normoxemie bij trauma-ICU
De interventiegroep (post-implementatie) bestaat uit patiënten die zijn opgenomen op de chirurgische/trauma-IC tijdens de gerichte normoxemie-interventieperiode van het getrapte implementatieproces (tot 25 maanden).

Post-implementatie van gerichte normoxemie door middel van zuurstoftitratie voor individuele patiënten.
Interventies voor de behandeling van hypoxemie volgen de gebruikelijke lokale praktijk.
Interventies voor de behandeling van hyperoxemie (SpO2 >96% of PaO2 >100 mmHg) omvatten een neerwaartse titratie van FiO2 (of aanvullende zuurstof voor niet-mechanisch beademde patiënten) binnen een tijdsbestek op basis van lokale locatievoorkeuren, doorgaans in stappen van niet meer dan 0,10 totdat de beoogde oxygenatie in het normoxemiebereik is bereikt (inclusief kamerlucht [geen aanvullende zuurstof] voor niet-mechanisch beademde patiënten).
