Het aanpakken van stress en onbewuste vooringenomenheid bij zorgverleners om de kwaliteit van de gezondheidszorg voor moeders te verbeteren
De in dit voorstel beschreven activiteiten zijn gericht op het aanpakken van stress en onbewuste vooroordelen bij zorgverleners om de kwaliteit van de gezondheidszorg voor moeders te verbeteren, met name in verband met de persoonsgerichte dimensies van de zorg, d.w.z.
zorg die respectvol is en inspeelt op de behoeften, voorkeuren en waarden van vrouwen.
De onderzoekers richten zich op stress bij zorgverleners en onbewuste vooroordelen, omdat deze belangrijke oorzaken zijn van zorg van slechte kwaliteit, die vaak niet worden aangepakt in interventies die zijn ontworpen om de kwaliteit van de gezondheidszorg voor moeders te verbeteren.
De onderzoekers zijn van plan (1) een interventie te ontwerpen die zorgverleners in staat stelt hun stress en onbewuste vooroordelen te identificeren en te beheersen; (2) de interventie testen om de haalbaarheid en aanvaardbaarheid ervan te beoordelen; en (3) het voorlopige effect van de interventie beoordelen op: (a) kennis, houding en gedrag van de aanbieder gerelateerd aan stress en onbewuste vooroordelen; en (b) stressniveaus voor aanbieders.

Slechte persoonsgerichte gezondheidszorg voor moeders (PCMHC) draagt bij aan de hoge sterfte onder moeders en pasgeborenen in Afrika bezuiden de Sahara (SSA), en verschillen in PCMHC zorgen voor verschillen in het gebruik van gezondheidszorg voor moeders.
Er bestaat echter weinig onderzoek naar de manier waarop PCMHC kan worden verbeterd en verschillen kunnen worden verminderd.
Met dit project proberen de onderzoekers deze leemte op te vullen.
Ze stellen voor om de stress en onbewuste vooroordelen van zorgverleners aan te pakken als fundamentele factoren die zowel slechte PCMHC als verschillen in PCMHC aandrijven.
Stress bij zorgverleners en onbewuste vooroordelen zijn belangrijk om rekening mee te houden, omdat: (1) zorgverleners in omgevingen met weinig middelen vaak onder zeer stressvolle omstandigheden werken; (2) onbewuste vooroordelen heersen in elke samenleving, inclusief SSA; en (3) deze factoren zijn elkaar versterkende oorzaken van zorg van slechte kwaliteit en verschillen in persoonsgerichte zorg.
In de eerste fase van het project (CPIPE1) voerden ze onderzoek uit om (1) de factoren te onderzoeken die verband houden met PCMHC en identificeerden ze stress bij de zorgverlener en onbewuste vooroordelen als belangrijkste bijdragende factoren.
Ze onderzochten ook de niveaus van stress bij leveranciers en onbewuste vooroordelen en de soorten stressoren en vooroordelen in Migori County, Kenia.
De resultaten van dat onderzoek zullen worden gebruikt om deze fase (CPIPE2) te informeren, waarvan de doelstellingen zijn: (1) het ontwerpen van een uit meerdere componenten bestaande theorie en op bewijs gebaseerde interventie die zorgverleners in staat stelt hun stress en onbewuste vooroordelen te identificeren en te beheersen; (2) de interventie testen om de haalbaarheid en aanvaardbaarheid ervan te beoordelen; en (3) het voorlopige effect van de interventie beoordelen op: (a) kennis, houding en gedrag van de aanbieder gerelateerd aan stress en onbewuste vooroordelen; en (b) stressniveaus bij de aanbieder met behulp van een pretest-posttest-controlegroepontwerp.
Ze zullen de resultaten van de pilot gebruiken om de interventie te verfijnen en een R01-voorstel te ontwikkelen voor een evaluatie op meerdere locaties met een grotere steekproef en een langere follow-up om de impact op PCMHC te beoordelen.
Deze studie zal waardevolle informatie opleveren ter ondersteuning van de kwaliteitsverbeteringsinspanningen voor PCMHC.
