Efficiëntie en kwaliteit bij postoperatieve pijntherapie na ontslag (EQUIPPED)

Na intramurale chirurgie krijgt meer dan 80% van de patiënten opioïden voorgeschreven voor gebruik na ontslag, maar tot 90% van de patiënten meldt overgebleven opioïden en slechts 16% maximaliseert niet-opioïde therapie.
Het voorgestelde onderzoek is bedoeld om een op de zorgverlener gerichte tool voor besluitvormingsondersteuning en een op de patiënt gerichte smartphone-app te testen om de hoeveelheid voorgeschreven en ingenomen opioïden na ontslag te verminderen, terwijl een effectieve behandeling van pijn na een operatie wordt gegarandeerd.

Voorafgaand aan ontslag uit het ziekenhuis zullen patiënten worden benaderd met betrekking tot interesse in het onderzoek met een inleidende brief waarin de inhoud en het doel van het onderzoek worden beschreven.
Na geïnformeerde toestemming worden patiënten gerandomiseerd om de consumentengezondheidsinformatica-app (UControlPain) te ontvangen in plaats van een app met alleen een gegevensverzamelingsfunctie (geen educatieve componenten).
Randomisatie wordt via REDCap uitgevoerd door de studiecoördinatoren.
Verder wordt informatie over welk type app is geïnstalleerd niet gedeeld met zorgverleners of patiënten.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Inclusiecriteria:
- >19-89 jaar oud
- Toegang tot een smartphone (iOS of Android)
- intramurale chirurgie waarbij minimaal een nacht in het ziekenhuis moet worden opgenomen met verwacht ontslag naar huis
Uitsluitingscriteria:
- Patiënten die binnen 30 dagen na eerdere ziekenhuisopname opnieuw in het ziekenhuis zijn opgenomen
- Zwangere patiënten
- Patiënten kunnen de Engelse taal niet lezen
- Patiënten ontslagen naar een post-acute zorginstelling
- Patiënten met contra-indicaties voor opioïden, paracetamol of NSAID's
- Patiënten die voldoen aan de AHRQ-definitie van langdurige opioïdentherapie (gebruik van opioïden op de meeste dagen >3 maanden) voorafgaand aan de operatie.

Opioïden gerapporteerd als genomen worden geregistreerd door type opioïde, eenheden en totale hoeveelheid die binnen de eerste 28 dagen na ontslag uit het ziekenhuis worden genomen.

Het door de patiënt gerapporteerde uitkomsten meetinformatiesysteem (Promis) werd gebruikt om de pijnintensiteit te meten, waarbij hogere scores duiden op meer intense pijn.
De promis werd vier keer na ontslag beoordeeld, eenmaal per week gedurende 28 dagen, en gemiddeld.
Op de promis 4-item schaalpijnintensiteitsscores duiden hogere scores op een hogere pijnintensiteit.
