The study included the following parts:
- Phase 1 - Dose-escalation safety lead-in in healthy participants.
- Phase 2 - Efficacy PoC in healthy participants considered at risk for gonorrhea.
The doses to be tested for efficacy would be selected based on the safety evaluation performed during the dose-escalation safety lead-in: in case more than one dose would show tolerability, the highest tolerated dose (HTD) and the dose below the highest tolerated (i.e., 2 doses) would be advanced in the efficacy PoC part of the study and compared versus placebo. In case only the lowest dose shows adequate tolerability, this would be the only dose tested in the PoC versus placebo.

Inclusiecriteria:
Opnamecriteria voor het inleidende gedeelte over veiligheid met dosisescalatie
- Deelnemers die naar het oordeel van de onderzoeker kunnen en zullen voldoen aan de eisen van het protocol.
- Geschreven of getuige/duim afgedrukte geïnformeerde toestemming verkregen van de deelnemer voorafgaand aan de uitvoering van een studiespecifieke procedure.
- Gezonde deelnemers zoals vastgesteld door medische geschiedenis, klinisch onderzoek en laboratoriumbeoordeling.
- Een deelnemer tussen en met 18 en 50 jaar oud op het moment van geïnformeerde toestemming.
- Vrouwelijke deelnemers die geen kinderen kunnen krijgen, kunnen aan het onderzoek deelnemen. Niet-vruchtbaar potentieel wordt gedefinieerd als premenarche, huidige bilaterale afbinding of occlusie van de eileiders, hysterectomie, bilaterale ovariëctomie of postmenopauze.
Vrouwelijke deelnemers in de vruchtbare leeftijd kunnen aan het onderzoek deelnemen als de deelnemer:
- adequate anticonceptie heeft toegepast gedurende 1 maand voorafgaand aan de toediening van de onderzoeksinterventie, en
- een negatieve zwangerschapstest heeft op de dag van toediening van de onderzoeksinterventie, en
- heeft ermee ingestemd om adequate anticonceptie voort te zetten gedurende de gehele behandelingsperiode en gedurende 1 maand na voltooiing van de studieinterventietoedieningsreeks.
Inclusiecriteria voor het onderdeel werkzaamheid PoC
- Deelnemers die naar het oordeel van de onderzoeker kunnen en zullen voldoen aan de eisen van het protocol.
- Geschreven of getuige/duim afgedrukte geïnformeerde toestemming verkregen van de deelnemer voorafgaand aan de uitvoering van een studiespecifieke procedure.
Gezonde deelnemers zoals vastgesteld door:
- Voor de 2 subsets van intensieve veiligheidsmonitoring (d.w.z. eerste 30 hiv-negatieve proefpersonen per groep, gevolgd door de eerste 8 hiv-positieve proefpersonen per groep): medische voorgeschiedenis, klinisch onderzoek en laboratoriumonderzoek.
- Voor alle overige deelnemers: medische geschiedenis, klinisch onderzoek.
- Risico lopen op gonokokkeninfecties op basis van seksuele gedragskenmerken: dit kunnen mannen zijn die seks hebben met mannen, pre-expositie profylaxe voor HIV-gebruikers, personen die deelnemen aan transactionele seks deelnemers met huidige of vroegere soa-diagnose, deelnemers op het moment van soa-screening of zoeken naar andere soa-diensten.
- Een deelnemer tussen en met 18 en 50 jaar oud op het moment van geïnformeerde toestemming. Transgender mannen en vrouwen, en andere gender non-conforme mensen die zichzelf identificeren als geen man of vrouw kunnen worden opgenomen in de studie, op basis van hun risicofactoren. Voor deze studie zullen ze worden opgevolgd op basis van hun biologische geslacht (geslacht bij de geboorte), seksuele geaardheid en genitale/seksuele anatomie
- Deelnemers die niet zwanger kunnen worden, kunnen aan het onderzoek deelnemen. Dit geldt ook voor transmannen die geen geslachtsbevestigende operatie aan hun geslachtsdelen hebben ondergaan.
Deelnemers in de vruchtbare leeftijd kunnen aan het onderzoek deelnemen als de deelnemer:
- adequate anticonceptie heeft toegepast gedurende 1 maand voorafgaand aan de toediening van de onderzoeksinterventie, en
- een negatieve zwangerschapstest heeft op de dag van toediening van de onderzoeksinterventie, en
- heeft ermee ingestemd om adequate anticonceptie voort te zetten gedurende de gehele behandelingsperiode en gedurende 1 maand na voltooiing van de studieinterventietoedieningsreeks.
Uitsluitingscriteria:
Medische aandoeningen Veiligheidsinleiding voor dosis-escalatie
- Elke klinisch significante hematologische/biochemische laboratoriumafwijking.
- Elke andere klinische aandoening die, naar de mening van de onderzoeker, een extra risico voor de deelnemer kan vormen vanwege deelname aan het onderzoek.
- Voorgeschiedenis van een reactie/overgevoeligheid die waarschijnlijk wordt verergerd door een onderdeel van de onderzoeksinterventie(s).
- Elke bevestigde vermoedelijke immunosuppressieve/immunodeficiënte aandoening, gebaseerd op medische geschiedenis en lichamelijk onderzoek.
- Overgevoeligheid voor latex.
- Acute/chronische klinisch significante pulmonale, cardiovasculaire, lever-/nierfunctieafwijkingen, zoals vastgesteld door lichamelijk onderzoek/laboratoriumtests.
- Terugkerende geschiedenis/ongecontroleerde neurologische aandoeningen of toevallen.
- Geschiedenis van invasieve meningokokkenziekte. Werkzaamheid PoC deel: hiv-negatieve intensieve veiligheidsmonitoring, hiv-negatieve volledige inschrijving en voor alle overige deelnemers
- Gonokokkeninfectie geïdentificeerd binnen 14 dagen voorafgaand aan randomisatie.
- Elke andere klinische aandoening die, naar de mening van de onderzoeker, een extra risico voor de deelnemer kan vormen vanwege deelname aan het onderzoek.
- Geschiedenis van een reactie of overgevoeligheid die waarschijnlijk wordt verergerd door een onderdeel van de onderzoeksinterventie(s).
- Elke bevestigde of vermoede immunosuppressieve/immunodeficiënte aandoening, gebaseerd op medische geschiedenis en lichamelijk onderzoek.
- Bekende seropositiviteit voor HIV-infectie, ongeacht viremie en aantal CD4-cellen
- Overgevoeligheid voor latex.
- Acute/chronische klinisch significante pulmonale, cardiovasculaire, lever-/nierfunctieafwijkingen, zoals vastgesteld door lichamelijk onderzoek/laboratoriumtests.
- Terugkerende geschiedenis/ongecontroleerde neurologische aandoeningen of toevallen.
- Geschiedenis van invasieve meningokokkenziekte.
De hierboven beschreven uitsluitingscriteria en het volgende uitsluitingscriterium zijn alleen van toepassing op hiv-positieve deelnemers (subgroep voor intensieve veiligheidsmonitoring en volledige inschrijving van hiv-positieve deelnemers):
Seropositiviteit voor hiv-infectie als:
- Aantal CD4-cellen < 350 cellen/mm3 in de afgelopen 6 maanden
- viral load > 50cp/ml in de laatste 6 maanden
- deelnemer neemt > 3 maanden geen antiretrovirale therapie (ART) of is in de afgelopen 3 maanden overgestapt van een andere ART.
Voor beide subgroepen van intensieve veiligheidsmonitoring (eerste 30 hiv-negatieve proefpersonen per groep en eerste 8 hiv-positieve proefpersonen per groep) zijn deze uitsluitingscriteria van toepassing:
Elke klinisch significante hematologische/biochemische laboratoriumafwijking.
Eerdere/gelijktijdige therapie Van toepassing op zowel het inleidingsgedeelte voor de veiligheid van de dosis-escalatie als het PoC-gedeelte
- Gebruik van een ander onderzoeks-/niet-geregistreerd product dan de onderzoeksinterventie(s) gedurende de periode vanaf 30 dagen vóór de eerste dosis/het geplande gebruik ervan tijdens de onderzoeksperiode.
- Eerdere en geplande vaccinatie met een op OMV gebaseerd Neisseria meningitidis groep B-vaccin (bijv. Bexsero, MeNZB-vaccin of MenBvac op elk moment voorafgaand aan de eerste dosis en gedurende de gehele onderzoeksperiode.
- Geplande toediening/toediening van een vaccin waarin het studieprotocol niet voorziet in de periode die begint 15 dagen voor de eerste dosis en eindigt 15 dagen na de laatste dosis vaccintoediening*.
- Toediening van langwerkende immuunmodificerende geneesmiddelen gedurende de periode vanaf 6 maanden voorafgaand aan de eerste dosis studieinterventie/geplande toediening op elk moment tijdens de studieperiode.
- Toediening van immunoglobulinen/alle bloedproducten/plasmaderivaten gedurende de periode vanaf 3 maanden vóór de toediening van de eerste dosis van de onderzoeksinterventie/geplande toediening tijdens de onderzoeksperiode.
- Chronische toediening (gedefinieerd als meer dan 14 dagen in totaal) van immunosuppressiva/andere immuunmodificerende geneesmiddelen gedurende de periode vanaf 3 maanden voorafgaand aan de eerste studieinterventiedosis(s). Zie hierboven voor langwerkende immuunmodificerende geneesmiddelen. Voor corticosteroïden betekent dit prednison-equivalent ≥ 20 mg/dag voor volwassen deelnemers/ ≥ 0,5 mg/kg/dag. Geïnhaleerde en lokale steroïden zijn toegestaan.
Het volgende uitsluitingscriterium is alleen van toepassing op het PoC-gedeelte:
•Chronisch/langdurig gebruik van systemische antibiotica met een werking tegen Neisseria gonorrhoeae.
- Eerdere/gelijktijdige klinische onderzoekservaring die van toepassing is op zowel het veiligheidsinleidend deel met dosisescalatie als het PoC-deel Gelijktijdig deelnemen aan een ander klinisch onderzoek, op enig moment tijdens de onderzoeksperiode, waarin de deelnemer is/zal worden blootgesteld aan een onderzoeks-/ een niet-onderzoeksinterventie.
Andere uitsluitingen die van toepassing zijn op zowel het leidende deel veiligheid dosis-escalatie als het PoC-deel
- Zwangere/zogende teef.
- Vrouw die van plan is zwanger te worden/van plan is de anticonceptievoorzorgen stop te zetten binnen 1 maand na voltooiing van de studieinterventietoedieningsreeks.
- Elk studiepersoneel / hun directe gezinsleden, familieleden / gezinsleden.
- Leefstijloverwegingen die de uitvoering van het onderzoek kunnen verstoren/extra risico's kunnen vormen voor de rechten en het welzijn van de deelnemers.

An AE is any untoward medical occurrence (an unfavourable/unintended sign - including an abnormal laboratory finding), symptom, or disease (new or exacerbated) in a clinical study participant that is temporally associated with the study intervention.
The AE may or may not be considered related to the study intervention.
A participant is considered to have withdrawn from the study if no new study procedure has been performed or no new information has been collected for them since the date of withdrawal/last contact.
Any = occurrence of the event regardless of intensity grade.

The incidence rate (y/PT) of confirmed gonorrhea cases with and without Chlamydia Trachomatis co-infection, expressed in terms of 100 person-years, was calculated as the number of confirmed gonorrhea cases with and without Chlamydia Trachomatis co-infection (y) in a group, over the sum of individual person-times at risk (PT) in the same group, and multiplied by 100.

Symptomatic gonorrhea cases which were later confirmed gonorrhea cases are reported.
Symptomatic gonorrhea cases are defined as participants with symptoms suggestive of gonorrhea at the infected anatomical site which were later confirmed by a positive NAAT at the anorectal and/or urogenital location.
The incidence rate (y/PT) of confirmed gonorrhea cases, expressed in terms of 100 person-years, was calculated as the number of confirmed gonorrhea cases (y) in a group, over the sum of individual person-times at risk (PT) in the same group, and multiplied by 100.
