Mechanisme van stikstofmonoxide bij door beademing geïnduceerde diafragmadysfunctie met hulp van extracorporale membraanlongen
Het doel van deze klinische studie is om het effect van NO-behandeling op de middenriffunctie na mechanische beademing met ECMO-ondersteuning te onderzoeken. We verwachten in totaal 80 patiënten in te schrijven die mechanische beademing en mechanische beademing in combinatie met ECMO-behandeling zullen krijgen. Afhankelijk van de indicatie krijgen zij al dan niet GEEN behandeling. In de ECMO-geassisteerde mechanische ventilatie + NO-behandelingsgroep worden de proefpersonen willekeurig toegewezen aan NO-behandeling via een beademingsapparaat of membraanoxygenator. De belangrijkste vragen die het wil beantwoorden zijn:
- Het observeren van de invloeden en potentiële therapeutische effecten van verschillende NO-inbrengmethoden via een beademingsapparaat of membraanoxygenator op het optreden en de ontwikkeling van beademingsgeïnduceerde diafragma (VIDD) dysfunctie tijdens ECMO-ondersteunde mechanische ventilatie.
- Onderzoek naar de mogelijke sleutelmechanismen van NO-behandeling voor het optreden en de ontwikkeling van VIDD na ECMO-geassisteerde mechanische ventilatie.

Inclusiecriteria:
- Leeftijd ≥ 18 jaar;
- Tracheale intubatie op onze afdeling spoedeisende hulp;
- Verwachte overlevingstijd ≥ 72 uur;
- Familieleden gaan akkoord met deelname en ondertekenen het formulier voor geïnformeerde toestemming.
Uitsluitingscriteria:
- Zwangere vrouwen;
- Patiënten met een hernia diafragmatica of andere bekende ziekten en verwondingen aan het middenrif;
- Patiënten met mechanische beademingsbehandeling binnen 3 maanden;
- Thoracale of abdominale tumor die het middenrif binnendringt;
- Een grote hoeveelheid ascites verhoogt de middenrifpositie;
- Misvorming van de bovenste luchtwegen;
- Trauma leidend tot instorting van de borstkas, enz.

Tijdsspanne: We starten POCUS binnen 1 uur (1 uur) na mechanische ventilatie en monitoren continu de membraanfunctie om respectievelijk 6 uur, 12 uur, 24 uur, 48 uur, 72 uur, 96 uur, 120 uur, 144 uur en 168 uur. POCUS wordt beëindigd als de patiënt overlijdt.
