Mecanismo del óxido nítrico en la disfunción del diafragma inducida por el ventilador con asistencia pulmonar con membrana extracorpórea
El objetivo de este ensayo clínico es explorar el efecto del tratamiento con NO sobre la función del diafragma después de la ventilación mecánica con asistencia de ECMO. Esperamos inscribir a un total de 80 pacientes que recibirán ventilación mecánica y ventilación mecánica combinada con tratamiento ECMO. Dependiendo de la indicación NO se les dará tratamiento o no. En el grupo de ventilación mecánica asistida por ECMO + NO, los sujetos serán asignados aleatoriamente a NO tratamiento mediante ventilador u oxigenador de membrana. Las principales preguntas que pretende responder son:
- Observar las influencias y los posibles efectos terapéuticos de diferentes métodos de inserción de NO a través de un ventilador o un oxigenador de membrana en la aparición y desarrollo de disfunción del diafragma inducida por ventilador (VIDD) durante la ventilación mecánica asistida por ECMO.
- Explorar los posibles mecanismos moleculares clave del tratamiento con NO sobre la aparición y el desarrollo de VIDD después de la ventilación mecánica asistida por ECMO.

Descripción
Criterios de inclusión:
- Edad ≥ 18 años;
- Intubación traqueal en nuestro servicio de urgencias;
- Tiempo de supervivencia esperado ≥ 72 horas;
- Los familiares aceptan participar y firman el formulario de consentimiento informado.
Criterios de exclusión:
- Mujeres embarazadas;
- Pacientes con hernia diafragmática u otras enfermedades y lesiones diafragmáticas conocidas;
- Pacientes con tratamiento de ventilación mecánica dentro de los 3 meses;
- Tumor torácico o abdominal que invade el diafragma;
- Una gran cantidad de ascitis eleva la posición del diafragma;
- Malformación del tracto respiratorio superior;
- Traumatismo que provoca colapso torácico, etc.

Función de diafragma
Periodo de tiempo: Iniciamos POCUS dentro de 1 hora (1 h) después de la ventilación mecánica y monitoreamos continuamente la función del diafragma a las 6 h, 12 h, 24 h, 48 h, 72 h, 96 h, 120 h, 144 h y 168 h respectivamente. POCUS se cancelará si el paciente muere.

Monitoreo POCUS continuo de la función del diafragma durante la ventilación mecánica, incluido el espesor del diafragma, la fracción de engrosamiento del diafragma, la excursión del diafragma, etc.
