Actieve MMP-8 in orale vloeistof voor de detectie van mond- en orofaryngeale kanker
Het doel van deze observationele studie is om te evalueren of de mondspoeltest met actieve matrix metalloproteïnase-8 (aMMP-8) kan helpen bij het opsporen van orale en orofaryngeale kankers. Onderzoekers maten de aMMP-8-niveaus in mondspoelmonsters verzameld bij 77 kankerdeelnemers en 50 niet-kankerdeelnemers in een klinische setting.
De hoofdvraag die dit onderzoek probeert te beantwoorden:
Kan de aMMP-8 mondspoeltest in realtime kankerdeelnemers nauwkeurig identificeren en onderscheiden van niet-kankerdeelnemers?

Het doel van deze prospectieve studie is om te evalueren of de mondspoeltest met actieve matrix metalloproteïnase-8 (aMMP-8) 72 orale en orofaryngeale kankers kan detecteren en differentiëren van 50 controledeelnemers.
Bij het bemonsteringsproces werd aan patiënten gevraagd om gedurende 30 seconden voor te spoelen met leidingwater. Na een minuut wachten werd hen gevraagd hun mond 30 seconden te spoelen met 5 ml gedestilleerd water uit de testkit en dit uit te spugen in een steriele maatbeker.
De monsteroplossing werd in de opening van de testcassette gegoten die op het testsysteem (ORAlyzer®) was geplaatst voor kwantitatieve resultaten. Na het verkrijgen van de kwantitatieve resultaten in 5-6 minuten werd de cassette onmiddellijk verwijderd om de kwalitatieve evaluatie uit te voeren (Periosafe®).
De expressie van aMMP-8 werd vergeleken met verschillende factoren, waaronder leeftijd, geslacht, rookstatus, parodontitis, tumorgrootte, knooppunten of distale metastasen, kankerstadium, histologische graad en tumorplaats. Er werd statistische analyse uitgevoerd om deze relaties te evalueren.

- Door biopsie bevestigde orale of orofaryngeale kanker geïdentificeerd door de volgende International Classification of Diseases (ICD-10) codes: C00-C06, C08-C10, C13 en C32 volgens de American Joint Committee on Cancer (AJCC) 8e editie
- Volwassen
Uitsluitingscriteria:
- Kanker ter plaatse
- Eerdere kankerdiagnose binnen vijf jaar voorafgaand aan het onderzoek
- Gebruik van tetracyclines op het moment van bemonstering
- Heeft de instructies van de fabrikant voorafgaand aan het mondspoelmonster niet gevolgd (d.w.z. tandenpoetsen vóór monstername)

aMMP-8-niveaus werden kwantitatief in realtime gemeten met behulp van snelle point-of-care (PoC) stoelkits (Periosafe®, Dentognostics GmbH, Solingen, Duitsland) en een kwantitatieve reader (Oralyzer®, Dentognostics GmbH, Solingen, Duitsland) om mondspoelmonsters van patiënten met mond- en orofarynxkanker beoordelen in vergelijking met gezonde proefpersonen. De drempel voor een positief kwantitatief aMMP-8-resultaat is vastgesteld op 20 ng/ml, waarbij waarden van ≥20 ng/ml als positief worden beschouwd en waarden van <19,9. ng/ml als negatief beschouwd. |
