Karakterisering van extracellulaire blaasjes uit het navelstrengbloed van extreem premature pasgeborenen en hun correlatie met ernstige morbiditeit en mortaliteit
Deze studie heeft als doel de rol van extracellulaire vesikels (EV's) bij extreem premature zuigelingen te begrijpen, die geboren zijn vóór 28 weken zwangerschap. EV's zijn kleine deeltjes die door cellen worden afgegeven en die belangrijke informatie over de toestand van het lichaam bevatten. Bij extreem premature zuigelingen functioneren bloedvaten mogelijk niet goed, wat leidt tot ernstige gezondheidsproblemen zoals bloedingen in de hersenen, longletsel of ernstige infecties.
Onderzoekers geloven dat het analyseren van EV's in het navelstrengbloed van deze zuigelingen kan helpen voorspellen welke baby's een hoger risico lopen op het ontwikkelen van deze complicaties. Door de grootte, het aantal en het type EV's te bestuderen, hoopt het team vroege markers te identificeren die artsen kunnen begeleiden bij het bieden van betere zorg.
De studie zal navelstrengbloed verzamelen van 30 in aanmerking komende zuigelingen die geboren zijn in het CHU van Montpellier. Bloedmonsters worden verwerkt om plaatjesarm plasma te isoleren, dat EV's bevat. Dit plasma wordt opgeslagen in een biobank, waardoor toekomstig onderzoek naar EV's en hun rol bij extreme vroeggeboorte mogelijk wordt. EV's worden vervolgens in het laboratorium geanalyseerd om hun kenmerken en eventuele verbanden met ernstige gezondheidsproblemen te beoordelen.
De bevindingen van deze studie kunnen het begrip van circulatieproblemen bij extreem premature zuigelingen verbeteren, helpen bij het identificeren van vroege voorspellers van ernstige complicaties, en betere monitoring- en behandelingsstrategieën informeren. De oprichting van een plasmabiobank biedt ook een waardevolle bron voor toekomstig onderzoek om de zorg en resultaten voor deze kwetsbare populatie te verbeteren.

Inclusiecriteria:
- Moeder ouder dan 18 jaar, in staat Frans te spreken en te begrijpen
- Pasgeborene jonger dan 28 weken zwangerschap, geboren en opgenomen in het Universitair Ziekenhuis van Montpellier
- Navelstrengaderbloed onmiddellijk na de geboorte afgenomen (uit het segment tussen de navelstrengklem en de placenta), met een volume van 10 ml (dat kan worden teruggebracht tot 3 ml als afname moeilijk is) in een EDTA-buis.
- Ouderlijke niet-oppositie tegen de studie verkregen vóór de monstername
Exclusiecriteria:
- Doodgeboren kind
- Afnamefalen: onvermogen om het monster af te nemen of de eerste centrifugatie te starten meer dan 3 uur na de geboorte
- Algemene regelgevingscriteria: onvermogen om ouderlijke niet-oppositie te verkrijgen, gebrek aan sociale zekerheidsdekking, personen onder wettelijke voogdij, of deelname aan een ander lopend onderzoek met een actieve uitsluitingsperiode

Extreem vroeg geboren pasgeborenen (ELGAN's) geboren in het Montpellier Ziekenhuis vóór 28 weken zwangerschap, die werden opgenomen op de neonatale eenheid, bij wie navelstrengbloed werd afgenomen bij de geboorte en die geen ernstige morbiditeit (vroege shock, intraventriculaire bloeding of pulmonale bloeding) of overlijden vóór ontslag hebben ervaren.
