Een prospectieve, multi-center cohortstudie met één arm, gebaseerd op cfDNA-fragment-omics met deep learning, om het TuFEst-model te valideren voor de stadia-diagnose van borstkankerlaesies en lymfeklieren
Door het onderzoek van dit project verwachten we de cfDNA-fragment-omics vloeibare biopsietechnologie op basis van deep learning te bereiken, de nauwkeurigheid van het TuFEst-model te verifiëren bij het voorspellen van de tumorbelastingstatus van borstkankerlaesies en lymfeklieren bij nieuw gediagnosticeerde borstkankerpatiënten en patiënten die neoadjuvante therapie ontvangen, en een theoretische basis te bieden voor grootschalige klinische toepassing in de toekomst.

- Op basis van het eerder gevestigde TuFEst-model wordt de cfDNA-fragmentomics vloeibare biopsietechnologie gebaseerd op deep learning gebruikt om de tumorlaststatus van borstkankerlaesies en lymfeklieren te voorspellen, waardoor de nauwkeurigheid van de vroege diagnose van borstkanker wordt verbeterd: Deze studie zal bloedmonsters verzamelen en analyseren van borstkankerpatiënten in verschillende stadia, en gebruik maken van deep learning-gebaseerde cfDNA-fragmentomics vloeibare biopsietechnologie om tumorgerelateerde cfDNA-fragmenten te extraheren en een cfDNA-fragmentomics kenmerkenbibliotheek op te bouwen. Voorspellingen worden gemaakt op basis van het TuFEst-model. Vervolgens worden nauwkeurigheidsmatching en evaluatie uitgevoerd op basis van de voorspellingsresultaten en de werkelijke borstkankerlaesie en lymfeklier tumorlaststatus. De werkzaamheid van het TuFEst-model in de stadiumdiagnose van borstkanker wordt geverifieerd.
- Om de gevoeligheid, specificiteit, nauwkeurigheid en reproduceerbaarheid van het TuFEst-model te evalueren om de betrouwbaarheid ervan in klinische toepassing te bepalen: Deze studie zal een groter aantal bloedmonsters verzamelen van borstkankerpatiënten op basis van de eerdere retrospectieve cohort om de prestaties van het model in een groter prospectief cohort te beoordelen. Deze studie zal ook de toepassing van deze technologie in de monitoring van neoadjuvante therapie voor borstkanker onderzoeken, specifiek de toepassing ervan evalueren in de post-behandeling stadiumdiagnose van borstkanker, voorspelling van behandelingseffecten en monitoring van tumorrecidief.

Patiënten die zijn behandeld in het Second Affiliated Hospital of Zhejiang University School of Medicine (andere centra) vanaf de datum van ethische goedkeuring tot en met 31 december 2027
Beschrijving
Inclusiecriteria:
- Patiënten van 18 tot 70 jaar;
- Directe chirurgiegroep (Cohort 1): Radicale chirurgie werd uitgevoerd zonder neoadjuvante therapie;
- Neoadjuvante therapiegroep (Cohort 2): De initiële diagnose was invasieve borstkanker met bevestigde uitzaaiing in de oksellymfeklieren, en de patiënt was bereid om aan het einde van de behandeling radicale chirurgie te ondergaan;
- Plasma van patiënten tijdens de behandeling kan worden verkregen;
- Bereid zijn om het geïnformeerde toestemmingsformulier te ondertekenen. -
Exclusiecriteria:
- Zwanger zijn of borstvoeding geven;
- Patiënten bij wie de laesies al zijn verwijderd;
- In de 5 jaar voor insluiting geleden aan andere typen kwaadaardige tumoren met een duidelijke pathologische diagnose;
- In het jaar voor insluiting had de patiënt andere kwaadaardige tumoren die door beeldvorming werden vermoed, maar niet door pathologie werden bevestigd;
- Vermoede uitzaaiingen op afstand op beeldvorming, of potentiële lymfeklierlaesies die niet volledig door chirurgie kunnen worden genezen;
- In de afgelopen 30 dagen een infuusbehandeling met bloedproducten hebben ontvangen. -

Patients with newly diagnosed invasive breast cancer and biopsy-confirmed axillary lymph node metastasis without distant metastasis, who are scheduled to receive standard neoadjuvant systemic therapy followed by breast and axillary surgery .
Patients who complete or prematurely discontinue neoadjuvant therapy for clinical reasons but proceed to surgery are eligible.At least 100 participants are planned to be enrolled in Cohort 2.
