Dit is een retrospectieve observationele studie ontworpen als een doelgerichte trial-emulatie met gebruik van routinematig verzamelde elektronische gezondheidsgegevens uit het TriNetX US Collaborative Network. De studie heeft als doel om de 36-maandsrisico's van sterfte door alle oorzaken en belangrijke cardiorenale uitkomsten te vergelijken bij volwassenen met obesitas, diabetes type 2 en cardiovasculair-nier-metabool stadium 2-3 die een GLP-1-receptoragonist of een SGLT2-remmer starten en vervolgens verschillende vroege behandelingsintensiveringsstrategieën volgen. Primaire schattingsgrootheden zijn relatieve hazardratio's en absolute risicoverschillen na 36 maanden.
In aanmerking komende deelnemers zijn volwassenen van 20 jaar of ouder met obesitas en diabetes type 2 die voldoen aan de cardiovasculair-nier-metabool stadium 2-3 criteria. In aanmerking komen, blootstellingen en uitkomsten worden geoperationaliseerd met behulp van TriNetX elektronische gezondheidsgegevens, inclusief ICD-10-CM diagnosecodes, een body mass index van minimaal 27 kg/m2, een hemoglobine A1c van minimaal 6,5% en medicatieverstrekkingsgegevens. Uitsluitingscriteria omvatten eerder gebruik van relevante medicatieklassen binnen 6 maanden, ernstige cardiovasculaire ziekte of gevorderde nierziekte binnen 12 maanden, ernstige cardiovasculaire of renale gebeurtenissen binnen 6 maanden, en enige voorgeschiedenis van niet-diabetes type 2, hiv, bariatrische chirurgie of vaste orgaantransplantatie.
Drie onderling uitsluitende vroege behandelingsintensiveringsstrategieën worden vergeleken: vroege add-on SGLT2-remmertherapie, vroege add-on DPP-4-remmer- of sulfonylureumtherapie, en geen vroege add-on therapie. Voor add-on strategieën is de indexdatum de datum van add-on start binnen 90 dagen na start van achtergrondtherapie. Voor de geen vroege add-on strategie is de indexdatum het 90-dagen landmark na start van achtergrondtherapie.
Omdat behandelingstoewijzing niet gerandomiseerd is, wordt niet-willekeurige behandelingselectie aangepakt met behulp van multinomiale propensity-score matching met 1:1 nearest-neighbor matching zonder vervanging en een caliper van 0,2 keer de standaarddeviatie van de logit propensity score. Post-match covariaatbalans wordt geëvalueerd met gestandaardiseerde gemiddelde verschillen, en eventuele resterende onbalans wordt verder gecorrigeerd in de uitkomstmodellen. Follow-up begint op de indexdatum en gaat door tot het eerste van de volgende: de uitkomst van belang, overlijden, laatste geregistreerde contact, of 31 januari 2026, met intention-to-treat en per-protocol analyses uitgevoerd.
De primaire uitkomst is sterfte door alle oorzaken binnen 36 maanden. Secundaire uitkomsten omvatten belangrijke nadelige cardiovasculaire gebeurtenissen, gedefinieerd als cardiovasculaire arrestatie, myocardinfarct of beroerte, en belangrijke nadelige nierevenementen, gedefinieerd als eindstadium nierziekte, dialyseafhankelijkheid of -start, nierfalen of overlijden. Vergelijkende effecten worden geschat over 36 maanden voor paarsgewijze vroege add-on vergelijkingen, met vooraf gespecificeerde subgroepanalyses naar cardiovasculair-nier-metabool stadium, obesitas ernst, basale nierfunctie en basale cardiovasculaire comorbiditeit.

Inclusiecriteria:
- Volwassenen van 20 jaar of ouder.
- Obesitas, gedefinieerd als een body mass index (BMI) van 27 kg/m² of hoger.
- Diabetes mellitus type 2, gedefinieerd met behulp van elektronische gezondheidsdossiers, inclusief diagnosiscodes en/of hemoglobine A1c van 6,5% of hoger.
- Voldoen aan cardiovasculair-nier-metabool (CKM) stadium 2-3 criteria bij aanvang.
- Gestart met een GLP-1-receptoragonist of een SGLT2-remmer als achtergrondtherapie.
- Behandelstrategie geclassificeerd op basis van vroege toevoeging binnen 90 dagen na start van de achtergrondtherapie, of geen vroege toevoeging met index op het 90-dagen moment.
Exclusiecriteria:
- Eerder gebruik van GLP-1-receptoragonisten, SGLT2-remmers, DPP-4-remmers of sulfonylureumderivaten binnen 6 maanden voor cohortinvoer.
- Grote cardiovasculaire ziekte of revascularisatie binnen 12 maanden voor cohortinvoer.
- Gevorderde nierziekte binnen 12 maanden voor cohortinvoer, inclusief eindstadium nierziekte, dialyse of geschatte glomerulaire filtratiesnelheid minder dan 15 mL/min/1,73 m².
- Grote cardiovasculaire of renale gebeurtenissen binnen 6 maanden voor de index.
- Elke voorgeschiedenis van niet-type 2 diabetes, HIV-infectie, bariatrische chirurgie of vaste orgaantransplantatie.
- Ontbrekende kritieke baseline covariaten.

GLP-1 RA Base Therapy
