EN FASE I/II DOSESEKALASJONSSTUDIE SOM BRUKER TREDIMENSJONELL KONFORMAL STRÅLETERAPI FOR ADENOKARSINOMA I PROSTAA
BAKGRUNN: Strålebehandling bruker høyenergirøntgenstråler for å skade tumorceller og kan være en effektiv behandling for prostatakreft.
FORMÅL: Fase I/II studie for å studere effektiviteten av strålebehandling ved behandling av pasienter som tidligere har ubehandlet stadium II eller stadium III prostatakreft.

MÅL: I. Bestem maksimal tolerert dose av 3-dimensjonal konform strålebehandling til prostatakjertelen og umiddelbart omkringliggende vev hos pasienter med stadium II eller III adenokarsinom i prostata. II. Bestem den normale vevstoksisitetsraten for dette regimet i endetarmen og blærene til disse pasientene. III. Bestem lokal kontroll ved klinisk og patologisk undersøkelse og ved PSA-bestemmelser hos pasienter behandlet med dette regimet. IV. Bestem fjernsvikt og total overlevelse for pasienter behandlet med dette regimet.
OVERSIKT: Dette er en doseeskaleringsstudie. Pasientene er stratifisert i henhold til 1 av følgende 3 prognostiske grupper: Gruppe 1: T1b-c eller T2a-b med kombinert PSA/Gleason (CPG) score 15 og under gruppe 2: T1b-c eller T2a-b med CPG-score over 15, eller en hvilken som helst T2c med PSA mindre enn 70 ng/ml Gruppe 3: T3 med prostataspesifikt antigen (PSA) mindre enn 70 ng/ml Pasienter får 3-dimensjonal (3-D) konform strålebehandling 5 dager i uken i 7,6, 8,2 eller 8,8 uker. Kohorter på 120-270 pasienter mottar økende doser av 3-D konform strålebehandling inntil maksimal tolerert dose (MTD) er bestemt. MTD er definert som dosen før den der 20 % av pasientene opplever dosebegrensende toksisitet. Pasientene følges hver 3. måned i 1 år, hver 4. måned i 1 år, hver 6. måned i 3 år, og deretter årlig.
PROSJEKTERT PASSERING: Totalt 549-801 pasienter (120-204 pasienter for første dosenivå, 237-321 pasienter for andre dosenivå og 192-276 pasienter for tredje dosenivå) vil bli påløpt for denne studien innen 4,75 til 5,25 år.

SYKDOMS KARAKTERISTIKA: Tidligere ubehandlet adenokarsinom i prostata T1b-c eller T2a-b med en Gleason-score på mer enn 5 og PSA større enn 4 ng/mL ELLER Enhver T3 PSA under 70 ng/mL innen 3 uker før inntreden og minst 10 dager etter prostatabiopsi Ingen biopsi påvist regional nodal involvering Ingen fjernmetastaser
