Molecular Epidemiology of Childhood Leukemia (Aka The California Childhood Leukemia Study) (CCLS)

Denne studien er en case-kontrollstudie av hendelse av barneleukemi (alle undertyper) diagnostisert siden midten av 1995. Barn som nylig er diagnostisert med leukemi er registrert i studien. Kriterier for inkludering i studien er: under 15 år, ingen tidligere kreftdiagnose, opphold i delstaten California på diagnosetidspunktet og tilgjengelighet av en engelsk- eller spansktalende forelder eller verge. Biologiske prøver før behandling, inkludert benmarg og perifert blod, innhentes for analyse i UCB-laboratoriet til Dr. M. Smith. Laboratoriet vil bruke Fluorescence In Situ Hybridization (FISH) for å oppdage kromosomspesifikk aneuploidi og translokasjoner. En rekke kromosomale translokasjoner, inkludert t(9;22) og t(8;21), er kjent for å være sentralt involvert i utviklingen av barneleukemi. Molekylær karakterisering av tilfellene med translokasjoner kan gi innsikt i tidspunktet for kritiske eksponeringer og arten av det involverte etiologiske middelet.
Ett sammenligningsobjekt (kontroll) rekrutteres for hver samtykkesak. For hvert tilfelle velges fire potensielle kontroller tilfeldig fra fødselsattestfiler i California og matches på fødselsdato, kjønn, mors rase, latinamerikanske foreldre og bostedsfylke. En av de fire fødselsattestkontrollene er tilfeldig valgt for å bli rekruttert til å delta i studien.
Et dyptgående personlig intervju stiller en rekke spørsmål, inkludert: bolighistorie; yrkes- og husholdningseksponeringshistorier; mødres reproduktive historie; hendelser under indeksgraviditet og fødsel; familiehistorie med sykdom; barnets helse og vaksinasjonshistorie, kontakt med andre barn; mors og barns eksponering for sigarettrøyk under graviditet og siden fødselen; mors og barns historie med røntgenstråler.
Spyttprøver er hentet fra både tilfeller og kontroller og deres biologiske mødre. Spyttprøvene sendes til studiekontoret og behandles i laboratoriet for genetisk epidemiologi ved UC Berkeley. DNA fra tilfeller og kontroller vil bli analysert ved polymerasekjedereaksjon for genetiske polymorfismer. Genetiske polymorfismer vil bli undersøkt i to glutationtransferasegener, M1 og Tl. Kasusprøver av perifert blod, benmarg og arkivert nyfødtblod vil også bli brukt for å oppdage N-ras-mutasjon.
Tre nivåer av en eksponeringsvurdering er under implementering. Nivå 1 registrerer og intervjuer saker og kontroller som søker å identifisere risikofaktorer, inkludert kjemisk eksponering i boliger og arbeidsplasser. I nivå 2 er saker og fødselsattestkontroller som ikke har skiftet bosted basert på spesifikke kriterier en del av en reliabilitetsstudie, som søker å finne ut om selvrapporterte kjemikalier brukt på intervjutidspunktet er funnet i hjemmet under en visuell undersøkelse flere måneder etter intervjuet. Tier 3 har som mål å dokumentere potensialet for husholdningseksponering ved å ta prøver av støv på gulvflatene. Målet er å identifisere om det er forskjeller i konsentrasjoner av plantevernmidler, metaller, polyaromatiske hydrokarboner, cotinin, polychorinated biphenyls og ethylenthiourea i hjemmene til tilfeller og kontroller. Videre vil en case-case-analyse identifisere om tilfeller med kromosomale translokasjoner av interesse bor i hjem med høyere konsentrasjoner av målforbindelser enn tilfeller som ikke har slike translokasjoner. Disse analysene vil avgjøre om leukemibarn med vanlige genetiske endringer opplever vanlige eksponeringer og om disse genetiske endringene har omtrent samme tidsmessige forekomst. Til slutt vil vi vurdere om barn med og uten leukemi er forskjellige med hensyn til mottakelighet.

Tilfellene må være barn i alderen 0-14 år som nylig er diagnostisert med leukemi (hvilken type som helst) ved et av de deltakende sykehusene. De må bo i delstaten California ved diagnose, aldri ha blitt diagnostisert med en tidligere kreftsykdom og ha en forelder eller verge som snakker engelsk eller spansk.
Kontrollene matches på saksbarnets DOB, kjønn, mors rase, foreldres latinamerikanske identitet. For å være kvalifisert må de ikke ha noen krefthistorie, ha en forelder eller verge som snakker engelsk eller spansk, og de må bo i delstaten California.
