Molecular Epidemiology of Childhood Leukemia (Aka The California Childhood Leukemia Study) (CCLS)

Denne undersøgelse er en case-kontrol undersøgelse af hændelig børneleukæmi (alle undertyper) diagnosticeret siden midten af 1995. Børn, der er nyligt diagnosticeret med leukæmi, er optaget i undersøgelsen. Kriterier for medtagelse i undersøgelsen er: under 15 år, ingen tidligere kræftdiagnose, ophold i staten Californien på diagnosetidspunktet og tilgængelighed af en engelsk- eller spansktalende forælder eller værge. Biologiske præ-behandlingsprøver, inklusive knoglemarv og perifert blod, opnås til analyse i UCB-laboratoriet hos Dr. M. Smith. Laboratoriet vil bruge Fluorescence In Situ Hybridization (FISH) til at detektere kromosomspecifik aneuploidi og translokationer. En række kromosomale translokationer, herunder t(9;22) og t(8;21), er kendt for at være centralt involveret i udviklingen af børneleukæmi. Molekylær karakterisering af tilfældene med translokationer kan give indsigt i tidspunktet for kritiske eksponeringer og arten af det involverede ætiologiske agens.
Der rekrutteres et sammenligningsobjekt (kontrol) for hver samtykkesag. For hvert tilfælde er fire potentielle kontroller tilfældigt udvalgt fra Californiens fødselsattestfiler og matchet på fødselsdato, køn, moderens race, forældres latinamerikanske status og bopælsland. En af de fire fødselsattestkontroller er tilfældigt udvalgt til at blive rekrutteret til at deltage i undersøgelsen.
Et dybdegående personligt interview stiller en række spørgsmål, herunder: bolighistorie; erhvervsmæssig og husholdningseksponeringshistorier; mødres reproduktive historie; hændelser under indeksgraviditet og fødsel; familiehistorie med sygdom; barnets helbred og vaccinationshistorie, kontakt med andre børn; mors og barns eksponering for cigaretrøg under graviditet og siden fødslen; mors og barns historie med røntgenbilleder.
Spytprøver opnås fra både tilfælde og kontroller og deres biologiske mødre. Spytprøverne sendes til undersøgelseskontoret og behandles i laboratoriet for genetisk epidemiologi ved UC Berkeley. DNA fra tilfælde og kontroller vil blive analyseret ved polymerasekædereaktion for genetiske polymorfismer. Genetiske polymorfier vil blive undersøgt i to glutathiontransferasegener, M1 og Tl. Caseprøver af perifert blod, knoglemarv og arkiveret nyfødt blod vil også blive brugt til at påvise N-ras mutation.
Tre niveauer af en eksponeringsvurdering er ved at blive implementeret. Tier 1 tilmelder og interviewer sager og kontroller, der søger at identificere risikofaktorer, herunder kemisk eksponering i boliger og på arbejdspladsen. I niveau 2 er sager og fødselsattestkontroller, der ikke har skiftet bopæl ud fra specifikke kriterier, en del af en pålidelighedsundersøgelse, som søger at afgøre, om selvrapporterede kemikalier, der blev brugt på interviewtidspunktet, findes i hjemmet under en visuel undersøgelse flere måneder efter samtalen. Tier 3 har til formål at dokumentere potentialet for husholdningseksponeringer ved at tage prøver af støv på gulvfladerne. Målet er at identificere, om der er forskelle i koncentrationer af pesticider, metaller, polyaromatiske kulbrinter, cotinin, polychorinerede biphenyler og ethylenthiourinstof i hjemmene til sager og kontroller. Yderligere vil en case-case-analyse identificere, om tilfælde med kromosomale translokationer af interesse bor i hjem med højere koncentrationer af målforbindelser end tilfælde, der ikke har sådanne translokationer. Disse analyser vil afgøre, om leukæmibørn med almindelige genetiske ændringer oplever almindelige eksponeringer, og om disse genetiske ændringer har omtrent samme tidsmæssige forekomst. Til sidst vil vi vurdere, om børn med og uden leukæmi er forskellige med hensyn til modtagelighed.

Tilfældene skal være børn i alderen 0-14 år, nydiagnosticeret med leukæmi (enhver form) på et af de deltagende hospitaler. De skal bo i staten Californien ved diagnosen, aldrig have været diagnosticeret med en tidligere kræftsygdom og have en forælder eller værge, der taler engelsk eller spansk.
