Molekulare Epidemiologie von Leukämie im Kindesalter (auch bekannt als „California Childhood Leukemia Study“) (CCLS)

Detaillierte Beschreibung
Bei dieser Studie handelt es sich um eine Fall-Kontroll-Studie über Leukämien im Kindesalter (alle Subtypen), die seit Mitte 1995 diagnostiziert wurden. Kinder, bei denen neu Leukämie diagnostiziert wurde, werden in die Studie aufgenommen. Kriterien für die Aufnahme in die Studie sind: unter 15 Jahre alt, keine vorherige Krebsdiagnose, Wohnsitz im Bundesstaat Kalifornien zum Zeitpunkt der Diagnose und Verfügbarkeit eines englisch- oder spanischsprachigen Elternteils oder Erziehungsberechtigten. Vor der Behandlung werden biologische Proben, einschließlich Knochenmark und peripheres Blut, zur Analyse im UCB-Labor von Dr. M. Smith entnommen. Das Labor wird Fluoreszenz-In-Situ-Hybridisierung (FISH) verwenden, um chromosomenspezifische Aneuploidie und Translokationen zu erkennen. Es ist bekannt, dass eine Reihe chromosomaler Translokationen, darunter t(9;22) und t(8;21), eine zentrale Rolle bei der Entstehung von Leukämie im Kindesalter spielen. Die molekulare Charakterisierung der Fälle mit Translokationen kann Aufschluss über den Zeitpunkt kritischer Expositionen und die Art des beteiligten ätiologischen Erregers geben.
Für jeden Einwilligungsfall wird eine Vergleichsperson (Kontrolle) rekrutiert. Für jeden Fall werden vier potenzielle Kontrollen nach dem Zufallsprinzip aus kalifornischen Geburtsurkundendateien ausgewählt und nach Geburtsdatum, Geschlecht, Rasse der Mutter, Hispanizität der Eltern und Wohnsitzland abgeglichen. Eine der vier Geburtsurkundenkontrollen wird nach dem Zufallsprinzip ausgewählt und für die Teilnahme an der Studie rekrutiert.
In einem ausführlichen persönlichen Interview werden verschiedene Fragen gestellt, darunter: Wohngeschichte; Berufs- und Haushaltsexpositionsgeschichten; Fortpflanzungsgeschichte der Mütter; Ereignisse während der Indexschwangerschaft und Entbindung; familiäre Krankheitsgeschichte; Gesundheitszustand und Impfgeschichte des Kindes, Kontakt zu anderen Kindern; Exposition von Mutter und Kind gegenüber Zigarettenrauch während der Schwangerschaft und seit der Geburt; Röntgenanamnese von Mutter und Kind.
Speichelproben werden sowohl von Fällen als auch von Kontrollen und ihren leiblichen Müttern entnommen. Die Speichelproben werden an das Studienbüro geschickt und im Labor für genetische Epidemiologie der UC Berkeley verarbeitet. DNA aus Fällen und Kontrollen wird mittels Polymerasekettenreaktion auf genetische Polymorphismen analysiert. Genetische Polymorphismen werden in zwei Glutathiontransferase-Genen, M1 und Tl, untersucht. Fallproben von peripherem Blut, Knochenmark und archiviertem Neugeborenenblut werden ebenfalls zum Nachweis der N-ras-Mutation verwendet.
Es werden drei Stufen einer Expositionsbewertung implementiert. Stufe 1 nimmt Fälle und Kontrollen auf und befragt sie, um Risikofaktoren zu identifizieren, einschließlich der Exposition gegenüber Chemikalien in Wohnräumen und am Arbeitsplatz. In Stufe 2 sind Fälle und Geburtsurkundenkontrollen, die ihren Wohnort nicht auf der Grundlage spezifischer Kriterien geändert haben, Teil einer Zuverlässigkeitsstudie, mit der festgestellt werden soll, ob bei einer visuellen Untersuchung mehrere selbst gemeldete Chemikalien, die zum Zeitpunkt der Befragung verwendet wurden, in der Wohnung gefunden wurden Monate nach dem Vorstellungsgespräch. Tier 3 zielt darauf ab, das Potenzial für Haushaltsexpositionen durch Staubproben auf den Bodenoberflächen zu dokumentieren. Ziel ist es herauszufinden, ob es Unterschiede in den Konzentrationen von Pestiziden, Metallen, polyaromatischen Kohlenwasserstoffen, Cotinin, polychlorierten Biphenylen und Ethylenthioharnstoff in den Häusern von Fällen und Kontrollen gibt. Darüber hinaus wird eine Fall-Fall-Analyse ermitteln, ob Fälle mit interessierenden chromosomalen Translokationen in Häusern mit höheren Konzentrationen an Zielverbindungen leben als Fälle ohne solche Translokationen. Diese Analysen werden feststellen, ob leukämische Kinder mit häufigen genetischen Veränderungen häufigen Expositionen ausgesetzt sind und ob diese genetischen Veränderungen zeitlich ungefähr gleich häufig auftreten. Abschließend werden wir untersuchen, ob sich Kinder mit und ohne Leukämie hinsichtlich der Anfälligkeit unterscheiden.

Beschreibung
Bei den Fällen muss es sich um Kinder im Alter von 0 bis 14 Jahren handeln, bei denen in einem der teilnehmenden Krankenhäuser neu Leukämie (jeglicher Art) diagnostiziert wurde. Sie müssen zum Zeitpunkt der Diagnose im Bundesstaat Kalifornien leben, bei ihnen wurde noch nie eine Krebserkrankung diagnostiziert und sie müssen einen Elternteil oder Erziehungsberechtigten haben, der Englisch oder Spanisch spricht.
Die Kontrollen werden anhand des Geburtsdatums, des Geschlechts, der Rasse der Mutter und der Hispanizität der Eltern des Fallkindes abgeglichen. Um teilnahmeberechtigt zu sein, dürfen sie keine Krebserkrankung haben, ein Elternteil oder Erziehungsberechtigter muss Englisch oder Spanisch sprechen und sie müssen im Bundesstaat Kalifornien leben.

Mithilfe eines Fall-Kontroll-Studiendesigns untersuchen wir die Rolle von Umwelt- und genetischen Faktoren bei der Ätiologie einer seltenen Krebserkrankung in einer gefährdeten und ethnisch vielfältigen Bevölkerung und nutzen dabei einen integrierten multidisziplinären Ansatz zur Verbesserung der Krankheitsklassifizierung und Expositionsbewertung.
