Kryteria przyjęcia:
- Podpisana świadoma zgoda i autoryzacja HIPAA.
- Co najmniej 40 lat w chwili rejestracji (data podpisania świadomej zgody).
- Wada refrakcji obu oczu pacjenta, oparta na refrakcji iDesign wybranej do leczenia („4,0 mm Rx calc” przy 12,5 mm), musi być krótkowzroczna z astygmatyzmem lub bez astygmatyzmu do ekwiwalentu sferycznego (SE) -6,00 D, z cylindrem do -3,00 D i minimalna przedoperacyjna krótkowzroczność w oku niedominującym co najmniej tak duża, jak ich docelowe zatrzymanie krótkowzroczności.
- Przynajmniej jedno oko musi wymagać leczenia krótkowzroczności.
- Przewidywana grubość łożyska zrębu co najmniej 250 mikronów w oparciu o przedoperacyjną pachymetrię centralnej rogówki pomniejszona o maksymalną głębokość zabiegu do usunięcia (obliczoną przez iDesign plus zamierzoną grubość płatka).

- Ekwiwalent sferyczny: wielkość różnicy jest mniejsza niż 0,625 D.
- Cylinder: wielkość różnicy jest mniejsza lub równa 0,5 D.
- Tolerancja osi cylindrów: Jeśli cylinder manifestu wprowadzony do systemu iDesign lub cylinder iDesign wybrany do leczenia jest mniejszy niż 0,5 D, nie ma wymogu uzgodnienia osi. - Jednoczesne stosowanie ogólnoustrojowych (w tym wziewnych) leków, które mogą utrudniać gojenie, w tym między innymi: antymetabolitów, izotretynoiny (Accutane®) w ciągu 6 miesięcy leczenia i chlorowodorku amiodaronu (Cordarone®) w ciągu 12 miesięcy leczenia. UWAGA: Uważa się, że stosowanie kortykosteroidów wziewnych lub ogólnoustrojowych, zarówno chronicznych, jak i ostrych, w ciągu ostatnich 6 miesięcy niekorzystnie wpływa na gojenie, a osoby stosujące takie leki są wyraźnie wykluczone z kwalifikacji.

Historia wcześniejszej operacji wewnątrzgałkowej lub rogówki (w tym usunięcia zaćmy), czynna choroba lub nieprawidłowości oczne (w tym między innymi objawowe zapalenie powiek, nawracające nadżerki rogówki, zespół suchego oka, neowaskularyzacja > 1 mm od rąbka), odwarstwienie/naprawa siatkówki, klinicznie istotne zmętnienie soczewki, kliniczne dowody urazu, zmętnienie rogówki w centralnej części 9 mm i widoczne w topografii, przebyta operacja zeza, zez lub ryzyko rozwoju zeza lub z objawami jaskry lub skłonnością do jaskry z wąskim kątem przesączania lub podejrzeniem jaskry.
UWAGA: Pacjenci z jaskrą, niezależnie od schematu leczenia lub kontroli, lub IOP powyżej 21 mmHg podczas badania przesiewowego, są wyraźnie wykluczeni z kwalifikacji.
- Dowody na stożek rogówki, dystrofię lub nieregularność rogówki lub nieprawidłową topografię.

Change in driving scale score from preoperative to postoperative as measured by driving scale within PROWL-M was reported.
PROWL-M driving scale consists of three driving related items that were used to generate the driving scale score.
Each item response was converted to a 0 to 100 scale so that the lowest and highest possible scores were set at 0 (not functioning) and 100 (functioning) points.
A high score represents better functioning.
The composite driving scale score was calculated by averaging the scores of all the three items, resulting in a total score ranging from 0 to 100 where higher score represent better functioning.
Change in driving scale score was defined as the driving scale score at 12-month visit minus the driving scale score at preoperative visit.

Satisfaction with LASIK via PROWL-M questionnaire was assessed with 8 items.
Responses to individual items were linearly transformed to a 0 to 100 scale where 100 indicates best possible vision and satisfaction, while 0 indicates worst possible vision and satisfaction.
Each item score was averaged together to produce a score ranging from 0 to 100.
A higher score indicating greater satisfaction.
