Observação da segurança, tolerabilidade e eficácia da terapia com adalimumabe (Humira) em um grande grupo de pacientes em condições clínicas diárias ao longo de 2 anos.

Pacientes com artrite reumatóide que iniciaram o tratamento com adalimumabe em um ambiente clínico normal de acordo com o rótulo do produto foram documentados.
O período de observação de acompanhamento foi de 2 anos e focado em informações de segurança e manutenção da eficácia durante um ambiente clínico normal.
O acompanhamento dos participantes foi feito por meio de visitas regulares ao consultório em intervalos determinados pela prática clínica de rotina ou conforme recomendado pelas diretrizes nacionais.

Amostra comunitária: pacientes com artrite reumatoide residentes na Alemanha.
Os regulamentos alemães estabelecem que todos os pacientes são elegíveis para estudos não intervencionais; não há exclusões.
Pacientes adultos (≥ 18 anos de idade) com AR que estavam se preparando para iniciar a terapia com adalimumabe de acordo com o rótulo do produto eram elegíveis para inclusão no estudo.
Os critérios de elegibilidade abaixo refletem o rótulo aprovado conforme declarado no Resumo Alemão das Características do Produto (SPC) para Humira.
Descrição
Critério de inclusão:
- Artrite reumatóide ativa moderada a grave em adultos com resposta insuficiente a medicamentos antirreumáticos modificadores da doença, incluindo metotrexato
- Artrite reumatóide ativa moderada a grave em adultos que não foram tratados anteriormente com metotrexato. Em caso de incompatibilidade com o metotrexato, Humira pode ser utilizado em monoterapia
Critério de exclusão:
- Hipersensibilidade contra a droga ou um dos outros ingredientes
- Tuberculose ativa ou outras infecções graves (p. sepse e infecções oportunistas)
- Insuficiência cardíaca moderada a grave

Vinte e oito contagens de articulações sensíveis, 28 contagens de articulações inchadas, a taxa de sedimentação de eritrócitos (ESR) e a avaliação do paciente da atividade global da doença (em uma escala analógica visual [VAS] de 0 a 10 cm) estão incluídos no escore DAS28.
Se o valor ESR estiver faltando, o valor da proteína C-reativa (CRP) pode ser substituído.
As pontuações no DAS28 variam de 0 a 10; pontuações mais altas indicam mais atividade da doença.
Uma pontuação DAS28 >5,1 indica alta atividade da doença, uma pontuação DAS28 <3,2 indica baixa atividade da doença e uma pontuação DAS28 <2,6 indica remissão clínica.

Uma resposta terapêutica individual DAS28-Dcrit é definida como uma diminuição (melhoria) no DAS28 desde a linha de base de ≥ 1,8.
