Descrição
Critério de inclusão:
- Hospitalizado com infecção por coronavírus (SARS-CoV-2).
- Participantes masculinos ou femininos de 1 a <18 anos de idade.
- Requer oxigênio suplementar e tem achados de imagem do tórax para confirmar doença respiratória devido a COVID-19 dentro de 72 horas após a entrada no estudo e inscrição.
- Oxigênio suplementar, incluindo, entre outros: cânula nasal, máscara, dispositivos de alto fluxo, CPAP/BiPAP, ventilação mecânica invasiva e ECMO.
Critério de exclusão:
- Estão recebendo tratamentos biológicos (como inibidores do Fator de Necrose Tumoral [TNF], inibidores de interleucina, terapias direcionadas para células T ou células B, interferon ou inibidores de Janus quinase (JAK)); ou estão recebendo outros imunossupressores de forma que, na opinião do investigador, a participação no estudo colocaria o participante em risco inaceitável de imunossupressão.
Nota: É necessário um período de washout antes da triagem.
- Estão recebendo fortes inibidores do transportador de ânions orgânicos 3 (OAT3) (como probenecida) que não podem ser descontinuados no início do estudo.
- Ter diagnóstico de tuberculose ativa (TB) atual ou, se conhecida, TB latente tratada por menos de 4 semanas com terapia antituberculose apropriada de acordo com as diretrizes locais (apenas pela história, sem necessidade de testes de triagem).
- Suspeita de infecção bacteriana, fúngica, viral ou outra infecção grave e ativa (além de COVID-19) que, na opinião do investigador, possa constituir um risco ao tomar o produto sob investigação.
- Ter recebido qualquer vacina viva dentro de 4 semanas antes da triagem ou pretender receber uma vacina viva durante o estudo. Nota: O uso de vacinas não vivas (inativadas) é permitido para todos os participantes.
- Necessita de ventilação mecânica invasiva, incluindo oxigenação por membrana extracorpórea (ECMO) no início do estudo.
- Diagnóstico atual de malignidade ativa que, na opinião do investigador, pode constituir um risco ao tomar o produto sob investigação.
- Tem histórico de tromboembolismo venoso (TEV) (trombose venosa profunda [TVP] e/ou embolia pulmonar [EP]) ou considerado alto risco de TEV (TVP/EP).
- Alta antecipada do hospital ou transferência para outro hospital (ou outra unidade), que não seja um local de estudo dentro de 72 horas após a entrada no estudo.
- Tem neutropenia (contagem absoluta de neutrófilos <1000 células/microlitros).
- Têm linfopenia (contagem absoluta de linfócitos <200 células/microlitros).
- Têm alanina aminotransferase (ALT) ou aspartato aminotransferase (AST) >5 vezes AAULN.
- Taxa de filtração glomerular estimada (eGFR) (Modificação da Dieta na Doença Renal [MDRD]) <40 mililitros/minuto/1,73 metros quadrados.
- Tem hipersensibilidade conhecida ao baricitinibe ou a qualquer um de seus excipientes.
- Esteja atualmente inscrito em qualquer outro estudo clínico envolvendo um produto de investigação ou qualquer outro tipo de pesquisa médica considerada não compatível científica ou medicamente com este estudo. Observação: o participante não deve estar inscrito (iniciado) em outro ensaio clínico para o tratamento de COVID-19 ou SARS CoV-2 até o dia 28.
- Estão grávidas, ou pretendem engravidar ou amamentar durante o estudo.
- Estão, na opinião do investigador ou patrocinador, em risco de imunossupressão ou de outra forma inadequados para inclusão no estudo.
- Está usando ou usará dispositivo de purificação extracorpórea de sangue (EBP) para remover citocinas pró-inflamatórias do sangue, como um dispositivo de absorção ou filtragem de citocinas, por exemplo, CytoSorb®.
- São, na opinião do investigador, improváveis de sobreviver por pelo menos 48 horas após a triagem.

Duration of ICU stay through Day 28 was defined as the total number of days spent in the ICU through Day 28, with days following death counted up to Day 28.
For participants alive at Day 28, duration was calculated as (Date of ICU discharge - Date of ICU admission) + 1.
For participants who died in the ICU on or before Day 28, duration was calculated as (Date of death - Date of ICU admission + 1) + (Date of Day 28 - Date of death).
