RELIANCE är en USA-baserad pragmatisk klinisk prövning finansierad av Patient-Centered Outcomes Research Institute (PCORI) för att jämföra långtidsanvändning av roflumilast vs. azitromycin hos upp till 3 200 patienter. Den är avsedd att stödja sjukhusinsatser för att minska risken för sjukhusinläggning av alla orsaker och minska för tidiga dödsfall hos individer med kronisk obstruktiv lungsjukdom (KOL) som har varit inlagd på sjukhus föregående år för en KOL-exacerbation. COPD Patient Powered Research Network (PPRN) och anslutna utredare kommer att genomföra prövningen på platser i USA.
Både roflumilast och azitromycin har visat sig minska risken för KOL-exacerbationer jämfört med placebo. Det har dock inte gjorts en direkt jämförelse mellan de två läkemedlen. Så de relativa skadorna och fördelarna med de två läkemedlen är okända. Berättigade patienter kommer att randomiseras (1:1) för att antingen få ett recept på roflumilast eller ett recept på azitromycin, och kommer att följas i minst 6 och upp till 36 månader. Det primära effektmåttet är det kombinerade resultatet av sjukhusvistelse eller dödsfall av alla orsaker; de sekundära effektmåtten inkluderar för tidigt avbrytande av behandlingen, patientrapporterade biverkningar och fysisk, social och emotionell hälsa. Patienter kommer att registreras på deltagande kliniska platser och uppföljningsdata kommer att samlas in via en patientportal online eller via ett callcenter. Baslinje- och resultatdata kommer också att samlas in från webbplatsens medicinska journaler och administrativa/skadedatabaser.
Pragmatisk, non-inferioritetsprövning som använder en intention-to-treat-analys för att utvärdera om daglig azitromycin är icke-sämre än daglig roflumilast hos patienter med hög risk för KOL-exacerbationer. Utredarna kommer att randomisera enskilda patienter för att få recept på roflumilast eller azitromycin (förhållande 1:1), stratifierade efter plats och aktuell rökstatus (ja/nej).

Inklusionskriterier:
- Patient och behandlande läkare som överväger intensifiering av behandlingen med roflumilast eller azitromycin för att minska risken för KOL-exacerbationer
- Ålder ≥ 40 år
- Nuvarande eller tidigare rökare i minst 10 pack-år
- Diagnos av svår KOL och associerad kronisk bronkit
- Inlagd på sjukhus med diagnosen KOL-exacerbation under de senaste 12 månaderna
- Nuvarande mediciner inkluderar inhalerad långverkande muskarinantagonist (LAMA), långverkande beta-agonist (LABA) /LAMA eller inhalerade kortikosteroider (ICS) /LABA
- Engelsk eller spansktalande
Exklusions kriterier:
- Kan inte eller vägrar ge informerat samtycke;
- Avböjer att ange personnummer eller sjukförsäkringsnummer (i tillämpliga fall)
- Anamnes med intolerans mot azitromycin eller roflumilast som patienten eller patientens behandlande läkare anser vara tillräckligt allvarlig för att undvika båda behandlingsalternativen;
- Nuvarande behandling med långtidsbehandling (mer än 30 dagar) roflumilast eller azitromycin (tidigare behandling med 1 eller fler doser av azitromycin eller roflumilast är inte ett uteslutningskriterium, så länge som patienten och läkaren söker behandlingsintensiveringsalternativ och är villiga att använd azitromycin eller roflumilast, enligt randomiserad behandlingsuppgift.)
- Känd överkänslighet mot azitromycin, erytromycin, alla makrolid- eller ketolidantibiotika;
- Historik av kolestatisk gulsot/leverdysfunktion i samband med tidigare användning av azitromycin
- Måttligt till gravt nedsatt leverfunktion (Child-Pugh B eller C)
- Nuvarande graviditet
- Alla andra uteslutningar som fastställts av läkaren enligt läkarens kliniska praxis
