Kvalificerade deltagare för denna studie har en diagnos av lung DS-TB.
Detta är en fas 2b/c multicenter, öppen etikett, randomiserad, dosfindande studie, bestående av upp till 26 veckors behandlingsperiod.
Efter en screeningperiod på upp till 14 dagar kommer berättigade deltagare att randomiseras i studien.
Randomisering kommer att stratifieras genom närvaro av bilateral kavitation vid screening av röntgenstrålen (ja eller nej). Efter slutet av behandlingsperioden kommer deltagarna att följas till 12 månaders randomisering efter.

Inklusionskriterier:
- Kunna ge skriftligt, informerat samtycke innan försöksrelaterade procedurer eller behandlingar påbörjas, och kan, enligt utredarens åsikt, uppfylla alla krav i försöket.
- Manliga eller kvinnliga deltagare mellan 18 och 65 år (inklusive) vid screeningbesöket.
- Kroppsvikt ≥ 35,0 kg vid screeningbesöket.
- Nydiagnostiserad, rifampin- och isoniazidkänslig (på screeningprovet) lung-TB.
- Kan spontant producera sputum.
- Kvinnor i fertil ålder (FOCBP) måste gå med på att använda 2 olika godkända preventivmetoder eller förbli abstinenta under hela sitt deltagande i prövningen och i 12 veckor efter den sista dosen av IMP eller dosen av RHEZ.
- Manliga deltagare måste gå med på att använda 2 olika godkända preventivmetoder eller förbli abstinenta under hela sitt deltagande i prövningen och i 12 veckor efter den sista dosen av IMP eller RHEZ.
Exklusions kriterier:
- Deltagarna är kända eller misstänkta för att ha resistens mot rifampin, isoniazid, etambutol, pyrazinamid, DLM eller BDQ antingen bekräftat av laboratoriet, eller baserat på epidemiologisk historia, vid screening.
- Bevis på kliniskt signifikant metabolisk (till exempel inkluderande pågående eller aktuell hypokalemi [dvs kalium <3,5 mEq/dL vid screening]), gastrointestinal, neurologisk, psykiatrisk, endokrin eller lever (t.ex. hepatit B och C) sjukdom; malignitet; eller andra abnormiteter (andra än den indikation som studeras).
- Historik av, eller aktuell, kliniskt relevant kardiovaskulär störning såsom hjärtsvikt, kranskärlssjukdom, okontrollerad hypertoni, arytmi eller symptom som starkt tyder på ett sådant problem (till exempel synkope eller hjärtklappning), takyarytmi eller status efter hjärtinfarkt.
- Kända blödningsrubbningar eller familjehistoria av blödningsrubbningar.
- Alla sjukdomar eller tillstånd där användningen av DLM, BDQ, OPC 167832, rifampin, isoniazid, pyrazinamid eller etambutol är kontraindicerad.
- Eventuell tidigare behandling för M tuberkulos under de senaste 2 åren.
- All behandling med ett läkemedel som är aktivt mot M tuberkulos (t.ex. kinoloner) inom 3 månader före screening.
- Kliniska bevis på svår extrapulmonell TB (t.ex. miliär TB, buken TB, urogenital TB, artros TB, TB meningit).
- Bevis på lungsilikos, lungfibros eller annat lungtillstånd som utredaren anser som allvarligt (annat än TB). I synnerhet alla underliggande tillstånd som kan störa bedömningen av röntgenbilder, sputuminsamling eller tolkning av sputumfynd, eller på annat sätt äventyra försökspersonens deltagande i rättegången.
- Eventuellt nedsatt njurfunktion som kännetecknas av kreatininclearance/uppskattad glomerulär filtrationshastighet (eGFR) på < 60 ml/min/1,73 m2, eller nedsatt leverfunktion som kännetecknas av alanintransaminas eller aspartattransaminas > 2,0 × övre normalgränsen för det kliniska laboratoriereferensintervallet eller bilirubin > 2,0 × övre normalgränsen för det kliniska laboratoriereferensintervallet, vid screening.
- Screening av glukos (icke fastande) ≥ 200 mg/dL eller glykosylerat hemoglobin (HbA1c) ≥ 6,5 %.
- QTcF > 450 msek hos manliga deltagare (> 470 msek hos kvinnliga deltagare), atrioventrikulärt block II eller III, bifasikulärt block, vid screening eller aktuell eller historia av kliniskt signifikanta ventrikulära arytmier. Andra EKG-avvikelser, om de anses kliniskt signifikanta av utredaren.
- Deltagare som får någon av de förbjudna medicinerna (se avsnitt 6.5.1) inom de angivna perioderna eller som sannolikt kommer att kräva förbjuden samtidig behandling under försöket.
- Kvinnliga deltagare som ammar eller som har ett positivt graviditetstestresultat innan de fick den första dosen av IMP eller RHEZ på dag 1.
- Aktuell historia av betydande drog- och/eller alkoholmissbruk som sannolikt kommer att resultera i dålig efterlevnad av prövningskraven eller som skulle utgöra en risk för deltagarens välbefinnande under prövningens gång.
- Historik med aktuell hepatit eller bärare av hepatit B-ytantigen (HBsAg) och/eller antihepatit C-virus (HCV).
- Deltagare som testar positivt för kokain eller andra missbruksdroger (exklusive kända receptbelagda stimulantia och andra förskrivna mediciner och marijuana) vid screening exkluderas. Detekterbara nivåer av alkohol, marijuana, barbiturater eller opiater i drogscreeningen är inte uteslutande om, enligt utredarens dokumenterade åsikt, deltagaren inte uppfyller Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition-kriterierna för måttlig till svår missbruksstörning och det positiva testet signalerar inte ett kliniskt tillstånd som skulle påverka säkerheten för deltagaren eller tolkningen av försöksresultaten, och deltagande godkänns av den medicinska monitorn före behandling.
- Historik av att ha tagit ett prövningsläkemedel inom 30 dagar innan försöket påbörjades.
- En historia av svårigheter att donera blod.
- Donation av blod eller plasma inom 30 dagar före dosering.
- Historik om allvarliga psykiska störningar som, enligt utredarens uppfattning, skulle utesluta deltagaren från att delta i denna rättegång.
- All känd tidigare exponering för OPC-167832, DLM eller BDQ.
- Deltagare med betydande medicinska komorbiditeter som enligt utredarens uppfattning inte bör delta i prövningen.
- Deltagare med Karnofsky-poäng < 60 kommer att uteslutas från försöket.
- Deltagare testade positivt för aktivt allvarligt akut respiratoriskt syndrom coronavirus (SARS-CoV-2) infektion vid screening.
- Deltagare med HIV co-infektion som inte har en stabil antiretroviral regim bestående av tenofovir, emtricitabin/lamivudin, dolutegravir (dvs > 3 månader), eller som har en detekterbar virusmängd eller som har ett CD4-antal < 350 celler/mm3 kommer att vara utesluten från rättegången.

TEAEs are AEs with an onset date on or after the start of treatment. Serious AE are AEs that leads to death, is life threatening, requires or prolongs hospitalization, significant disability, congenital anomaly or birth defect, and other medically important serious advent. AEs were graded on a severity 3-point scale as follows: Mild: Discomfort noticed, but no disruption to daily activity.; Moderate: Discomfort sufficient to reduce or affect normal daily activity.;Severe: Inability to work or perform normal daily activity. As pre-specified in statistical analysis plan (SAP),Division of AIDS (DAIDS v2.1) criteria was used and AEs graded as follows:Grade 1-Mild;Grade 2-Moderate;Grade 3-Severe;Grade 4-Life Threatening;Grade 5-Death. |

Laboratory assessments included clinical chemistry (Albumin, Alkaline Phosphatase, Aspartate Aminotransferase, Total Bilirubin, Calcium, Creatinine, estimated Glomerular Filtration Rate (eGFR), Glucose, Fasting and Non-Fasting, Cholesterol, Inorganic Phosphorus, Magnesium, Potassium, Sodium, Triglycerides and Uric Acid), hematology (Activated Partial Thromboplastin Time, Prothrombin Time, Partial Thromboplastin Time, International Normalized Ratio (INR), Absolute CD4+ Count, Absolute Lymphocytes, Absolute Neutrophil Count, Hemoglobin, Platelet Count, White Blood Cell), and urinalysis (Urine Glucose, Blood and Protein).
As pre-specified in statistical analysis plan (SAP), Division of AIDS (DAIDS) criteria was used and abnormalities were graded as follows:Grade 1 - Mild; Grade 2 - Moderate; Grade 3 - Severe; Grade 4 - Life Threatening; Grade 5 - Death.

Vital signs measurements included systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate, body weight, and body temperature.
Blood pressure (i.e., SBP, DBP) and heart rate were measured in the supine and sitting positions after the participant had been in each position for at least 3 minutes.
The participants were categorized based on the clinically relevant vital sign values as per DAIDS criteria pre-specified in SAP.
Each vital sign parameter was graded using the DAIDS criteria as follows: Grade 1 - Mild; Grade 2 - Moderate; Grade 3 - Severe; Grade 4 - Potentially Life Threatening; Grade 5 - Death.

Sputum LAM concentrations were measured on up to 3 samples collected at baseline and postbaseline at Week 8 and at end of treatment.
A participant was classified as having achieved negative sputum conversion in LAM if the participant has the first of 2 visits of at least 1 week apart with sputum LAM negative (below the lower limit of quantification) and without a positive sputum LAM result in between.
Numeric sputum LAM concentration data was converted to a binary variable using the rule: if the sputum LAM concentration was less than 10 picograms per milliliter (pg/mL) then it was interpreted as a negative result.
If the sputum LAM concentration was greater than or equal to 10 pg/mL then it was interpreted as positive result.
SCC for LAM was considered to have been achieved if there were negative results at two consecutive visits with non-missing results.

Acquired resistance was defined as a post-baseline resistant result at any timepoint after a baseline susceptible result.
Baseline was defined as Day -1, if Day -1 was missing then Day 1 was used, if day 1 was missing then Week 1 was used.
Resistance data was collected for streptomycin, isoniazid, rifampicin, ethambutol, pyrazinamide, bedaquiline and delamanid.
Susceptibility testing (DST) for anti-TB medications was performed on positive M tuberculosis isolates from Day -1 or Day 1 cultures, and on the end of treatment sputum specimen.
Percentage of participants who were susceptible at baseline and developed resistance to the indicated drug (class) at any post-baseline visits was summarized.

Sputum culture conversion occurs when a participant has the first of 2 consecutive visits of at least 7 days apart with sputum cultures negative and without a positive sputum culture result in between, as well as no positive sputum culture after the negative results.
If a participant had a positive MGIT culture result throughout the study period, then the participant was considered as not having achieved SCC within the period under consideration.
The time to SCC in this case was censored.
Time to SCC was calculated from date of first dose using MGIT cultures without the mitigation method measures.
