Detaillierte Beschreibung
Berechtigte Teilnehmer für diese Studie diagnostizieren eine Lungen-DS-TB.
Dies ist eine multizentrische Phase-2b/c-Studie mit offenem Label, randomisierte, dosisfindende Studie, bestehend aus bis zu 26 Wochen der Behandlungszeit.
Nach einer Screening -Periode von bis zu 14 Tagen werden in der Studie berechtigte Teilnehmer randomisiert.
Die Randomisierung wird durch das Vorhandensein einer bilateralen Kavitation beim Screening der Röntgenaufnahme des Brustkorbs (Ja oder Nein) geschichtet. Nach dem Ende der Behandlungszeit werden die Teilnehmer bis 12 Monate nach der Randomisierung befolgt.

Beschreibung
Einschlusskriterien:
- Kann vor Beginn aller studienbezogenen Verfahren oder Behandlungen eine schriftliche, informierte Einwilligung erteilen und ist nach Ansicht des Prüfarztes in der Lage, alle Anforderungen der Studie zu erfüllen.
- Männliche oder weibliche Teilnehmer im Alter zwischen 18 und 65 Jahren (einschließlich) beim Screening-Besuch.
- Körpergewicht ≥ 35,0 kg beim Screening-Besuch.
- Neu diagnostizierte, Rifampin- und Isoniazid-anfällige Lungentuberkulose (auf der Screening-Probe).
- Kann spontan Auswurf produzieren.
- Frauen im gebärfähigen Alter (FOCBP) müssen der Anwendung von zwei verschiedenen zugelassenen Verhütungsmethoden zustimmen oder während ihrer gesamten Teilnahme an der Studie und für 12 Wochen nach der letzten IMP- oder RHEZ-Dosis abstinent bleiben.
- Männliche Teilnehmer müssen zustimmen, zwei verschiedene zugelassene Verhütungsmethoden anzuwenden oder während ihrer gesamten Teilnahme an der Studie und für 12 Wochen nach der letzten Dosis IMP oder RHEZ abstinent zu bleiben.
Ausschlusskriterien:
- Es ist bekannt oder vermutet, dass die Teilnehmer Resistenzen gegen Rifampin, Isoniazid, Ethambutol, Pyrazinamid, DLM oder BDQ haben, was entweder vom Labor bestätigt wurde oder beim Screening auf der epidemiologischen Vorgeschichte basiert.
- Hinweise auf eine klinisch signifikante Stoffwechselerkrankung (z. B. einschließlich anhaltender oder aktueller Hypokaliämie [d. h. Kalium < 3,5 mÄq/dl beim Screening]), gastrointestinaler, neurologischer, psychiatrischer, endokriner oder Lebererkrankung (z. B. Hepatitis B und C); Malignität; oder andere Anomalien (außer der untersuchten Indikation).
- Vorgeschichte oder aktuelle klinisch relevante Herz-Kreislauf-Erkrankungen wie Herzinsuffizienz, koronare Herzkrankheit, unkontrollierter Bluthochdruck, Arrhythmie oder Symptome, die stark auf ein solches Problem hinweisen (z. B. Synkope oder Herzklopfen), Tachyarrhythmie oder Status nach einem Myokardinfarkt.
- Bekannte Blutungsstörungen oder Blutungsstörungen in der Familie.
- Alle Krankheiten oder Zustände, bei denen die Verwendung von DLM, BDQ, OPC 167832, Rifampin, Isoniazid, Pyrazinamid oder Ethambutol kontraindiziert ist.
- Jede vorherige Behandlung von M-Tuberkulose innerhalb der letzten 2 Jahre.
- Jede Behandlung mit einem Arzneimittel gegen M-Tuberkulose (z. B. Chinolone) innerhalb der 3 Monate vor dem Screening.
- Klinischer Nachweis einer schweren extrapulmonalen Tuberkulose (z. B. miliäre Tuberkulose, abdominale Tuberkulose, urogenitale Tuberkulose, osteoarthritische Tuberkulose, Tuberkulose-Meningitis).
- Hinweise auf Lungensilikose, Lungenfibrose oder eine andere Lungenerkrankung, die vom Prüfer als schwerwiegend eingestuft wird (außer Tuberkulose). Insbesondere jede Grunderkrankung, die die Beurteilung von Röntgenbildern, die Entnahme von Sputum oder die Interpretation von Sputumbefunden beeinträchtigen oder auf andere Weise die Teilnahme des Probanden an der Studie beeinträchtigen könnte.
- Jede Nierenfunktionsstörung, gekennzeichnet durch eine Kreatinin-Clearance/geschätzte glomeruläre Filtrationsrate (eGFR) von < 60 ml/min/1,73 m2 oder Leberfunktionsstörung, gekennzeichnet durch Alanintransaminase oder Aspartattransaminase > 2,0 × Obergrenze des Normalwerts des klinischen Laborreferenzbereichs oder Bilirubin > 2,0 × Obergrenze des Normalwerts des klinischen Laborreferenzbereichs beim Screening.
- Screening-Glukose (nicht nüchtern) ≥ 200 mg/dl oder glykosyliertes Hämoglobin (HbA1c) ≥ 6,5 %.
- QTcF > 450 ms bei männlichen Teilnehmern (> 470 ms bei weiblichen Teilnehmern), atrioventrikulärer Block II oder III, bifasikulärer Block, beim Screening oder aktuell oder in der Vorgeschichte klinisch signifikanter ventrikulärer Arrhythmien. Andere EKG-Anomalien, sofern sie vom Prüfer als klinisch bedeutsam erachtet werden.
- Teilnehmer, die innerhalb der angegebenen Zeiträume eines der verbotenen Medikamente (siehe Abschnitt 6.5.1) erhalten oder während der Studie wahrscheinlich eine verbotene Begleittherapie benötigen würden.
- Weibliche Teilnehmerinnen, die stillen oder ein positives Schwangerschaftstestergebnis haben, bevor sie am ersten Tag die erste Dosis IMP oder RHEZ erhalten.
- Aktuelle Vorgeschichte von erheblichem Drogen- und/oder Alkoholmissbrauch, der wahrscheinlich zu einer mangelhaften Einhaltung der Studienanforderungen führt oder das Wohlbefinden des Teilnehmers im Verlauf der Studie gefährden würde.
- Vorgeschichte einer aktuellen Hepatitis oder Träger des Hepatitis-B-Oberflächenantigens (HBsAg) und/oder des Anti-Hepatitis-C-Virus (HCV).
- Teilnehmer, die beim Screening positiv auf Kokain oder andere Drogen (mit Ausnahme bekannter verschreibungspflichtiger Stimulanzien und anderer verschriebener Medikamente und Marihuana) getestet wurden, werden ausgeschlossen. Nachweisbare Mengen an Alkohol, Marihuana, Barbituraten oder Opiaten im Drogenscreening sind kein Ausschluss, wenn der Teilnehmer nach dokumentierter Meinung des Prüfarztes die Kriterien des Diagnostic and Statistical Manual of Mental Disorders, 5. Auflage, für eine mittelschwere bis schwere Substanzgebrauchsstörung nicht erfüllt Der positive Test weist nicht auf einen klinischen Zustand hin, der die Sicherheit des Teilnehmers oder die Interpretation der Studienergebnisse beeinträchtigen würde, und der medizinische Monitor stimmt der Teilnahme vor der Behandlung zu.
- Vorgeschichte der Einnahme eines Prüfpräparats innerhalb von 30 Tagen vor Studienbeginn.
- Eine Vorgeschichte mit Schwierigkeiten beim Blutspenden.
- Spende von Blut oder Plasma innerhalb von 30 Tagen vor der Dosierung.
- Schwere psychische Störungen in der Vorgeschichte, die nach Ansicht des Prüfarztes den Teilnehmer von der Teilnahme an dieser Studie ausschließen würden.
- Jede bekannte frühere Exposition gegenüber OPC-167832, DLM oder BDQ.
- Teilnehmer mit erheblichen medizinischen Komorbiditäten, die nach Ansicht des Prüfarztes nicht an der Studie teilnehmen sollten.
- Teilnehmer mit einem Karnofsky-Score < 60 werden von der Studie ausgeschlossen.
- Teilnehmer, die beim Screening positiv auf eine aktive Infektion mit dem schweren akuten respiratorischen Syndrom Coronavirus (SARS-CoV-2) getestet wurden.
- Teilnehmer mit HIV-Koinfektion, die kein stabiles antiretrovirales Regime bestehend aus Tenofovir, Emtricitabin/Lamivudin, Dolutegravir (d. h. > 3 Monate) erhalten oder die eine nachweisbare Viruslast haben oder die eine CD4-Zahl < 350 Zellen/mm3 haben, werden davon betroffen sein von der Verhandlung ausgeschlossen.

TEAEs are AEs with an onset date on or after the start of treatment. Serious AE are AEs that leads to death, is life threatening, requires or prolongs hospitalization, significant disability, congenital anomaly or birth defect, and other medically important serious advent. AEs were graded on a severity 3-point scale as follows: Mild: Discomfort noticed, but no disruption to daily activity.; Moderate: Discomfort sufficient to reduce or affect normal daily activity.;Severe: Inability to work or perform normal daily activity. Laboratory assessments included clinical chemistry (Albumin, Alkaline Phosphatase, Aspartate Aminotransferase, Total Bilirubin, Calcium, Creatinine, estimated Glomerular Filtration Rate (eGFR), Glucose, Fasting and Non-Fasting, Cholesterol, Inorganic Phosphorus, Magnesium, Potassium, Sodium, Triglycerides and Uric Acid), hematology (Activated Partial Thromboplastin Time, Prothrombin Time, Partial Thromboplastin Time, International Normalized Ratio (INR), Absolute CD4+ Count, Absolute Lymphocytes, Absolute Neutrophil Count, Hemoglobin, Platelet Count, White Blood Cell), and urinalysis (Urine Glucose, Blood and Protein).

Vital signs measurements included systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate, body weight, and body temperature.
Blood pressure (i.e., SBP, DBP) and heart rate were measured in the supine and sitting positions after the participant had been in each position for at least 3 minutes.
The participants were categorized based on the clinically relevant vital sign values as per DAIDS criteria pre-specified in SAP.
Each vital sign parameter was graded using the DAIDS criteria as follows: Grade 1 - Mild; Grade 2 - Moderate; Grade 3 - Severe; Grade 4 - Potentially Life Threatening; Grade 5 - Death.

Sputum LAM concentrations were measured on up to 3 samples collected at baseline and postbaseline at Week 8 and at end of treatment.
A participant was classified as having achieved negative sputum conversion in LAM if the participant has the first of 2 visits of at least 1 week apart with sputum LAM negative (below the lower limit of quantification) and without a positive sputum LAM result in between.
Numeric sputum LAM concentration data was converted to a binary variable using the rule: if the sputum LAM concentration was less than 10 picograms per milliliter (pg/mL) then it was interpreted as a negative result.
If the sputum LAM concentration was greater than or equal to 10 pg/mL then it was interpreted as positive result.
SCC for LAM was considered to have been achieved if there were negative results at two consecutive visits with non-missing results.

Acquired resistance was defined as a post-baseline resistant result at any timepoint after a baseline susceptible result.
Baseline was defined as Day -1, if Day -1 was missing then Day 1 was used, if day 1 was missing then Week 1 was used.
Resistance data was collected for streptomycin, isoniazid, rifampicin, ethambutol, pyrazinamide, bedaquiline and delamanid.
Susceptibility testing (DST) for anti-TB medications was performed on positive M tuberculosis isolates from Day -1 or Day 1 cultures, and on the end of treatment sputum specimen.
Percentage of participants who were susceptible at baseline and developed resistance to the indicated drug (class) at any post-baseline visits was summarized.

Time to Sputum Culture Conversion (SCC)

Sputum culture conversion occurs when a participant has the first of 2 consecutive visits of at least 7 days apart with sputum cultures negative and without a positive sputum culture result in between, as well as no positive sputum culture after the negative results.
If a participant had a positive MGIT culture result throughout the study period, then the participant was considered as not having achieved SCC within the period under consideration.
The time to SCC in this case was censored.
Time to SCC was calculated from date of first dose using MGIT cultures without the mitigation method measures.
