移植物和自体血管中动脉粥样硬化病变的进展导致心绞痛复发、长期生存率不变以及重复 CABG 手术的疗效降低。 Johnson 在对 3,105 名 CABG 术后患者进行为期 11 年的随访时报告说,大约 15% 的术前心绞痛患者在 1 年时心绞痛复发;随后每年又有 6% 的人患上心绞痛。 在这项研究中,复发性心绞痛患者的死亡率是无心绞痛患者的两倍。 CABG 的 VA 合作试验的长期随访表明,手术组的存活率最初似乎优于药物组,但这种益处减少了大约 10 年。 这可能与移植物阻塞和自身冠状动脉粥样硬化的进展有关。 这些患者的再次手术具有更高的手术死亡率风险,而且结果也不那么令人印象深刻。 据估计,1984 年所有 CABG 手术中约有 5% 是再次手术,并且这一比例在未来十年内将翻一番。 因此,防止移植物闭塞和自体血管动脉粥样硬化进展的措施如果成功,将通过降低死亡率、发病率和接受再次手术的患者数量而具有重要的临床和经济意义。
除了阿司匹林和双嘧达莫在 CABG 术后患者中的试验表明术后第一年内移植物闭合显着减少外,没有对这些患者进行干预的大型系统研究。 移植物闭塞已被证明与高 LDL 胆固醇和低 HDL 胆固醇相关,动脉粥样硬化的严重程度还被证明与吸烟和凝血因子水平升高有关。 因此,降低低密度脂蛋白胆固醇(通过饮食和药物)和华法林抗血栓治疗是合理的干预选择。

设计叙述:
多中心、双盲、随机、对照试验。 所有潜在参与者在随机分组前接受了为期一个月的积极华法林治疗。 只有对华法林表现出极小反应并且服用超过 90% 的处方药的参与者才会被随机分配。 实施了降低血清胆固醇的饮食调整、锻炼计划和戒烟计划。 患者被随机分配到四个治疗组的 2 X 2 析因设计中:积极降低 LDL-C 洛伐他汀 40 至 80 mg/d,必要时使用考来烯胺 8 mg/d 和 LDL-C 60 至 85 mg/d分升;使用洛伐他汀 2.5 至 5 mg/d 适度降低 LDL-C,根据需要使用考来烯胺,以达到 130 至 140 mg/dl 的 LDL-C;华法林 1 至 4 毫克/天,以达到 1.8 至 2.0 的 INR;和华法林安慰剂。 所有参与者都被随访了五年,最后进行了选择性冠状动脉和移植血管造影术。 主要血管造影终点是每位患者的主要 SVG 显示管腔直径显着减少(0.06 毫米或更大)的比例。 对 750 名参与者进行了生物行为研究。

CABG 后生物行为研究检查了在五个临床中心登记的 759 名冠状动脉搭桥患者(269 名女性和 490 名男性)的队列。 社会人口和医疗数据是通过访谈和医疗图表获得的。 术前通过访谈和问卷调查确定那些条件允许进行术前评估的患者的健康相关生活质量和心理社会数据,并在招募男性和女性患者的医院的患者中进行比较。
