传统上,除了明显的癌症外,结肠镜检查发现的所有息肉都已被切除。 据认为,内窥镜医师无法可靠地区分小型(<10 毫米直径)增生性息肉与腺瘤性息肉 [1]。 因此,大量的增生性息肉被不必要地切除。 这会导致患者接受不必要的息肉切除术,从而增加风险,并通过处理更多组织病理学标本的成本增加卫生服务的成本。 每个样本的成本约为 58 英镑。
在过去十年中,通过使用新型色素内镜和计算“虚拟色素内镜”技术,已经解决了区分小增生性息肉和小腺瘤性息肉的挑战。
色素内窥镜检查涉及使用染色或对比染料来突出胃肠道粘膜中的微小表面图案。 染料通过内窥镜的工作通道应用于结肠粘膜。 日本内窥镜医师多年来一直使用吸收到上皮细胞中的甲酚紫和亚甲蓝等重要染色剂。 Kudo 等人的工作表明,通过将这些染色与放大内窥镜结合使用,可以精确检查和分类结肠粘膜病变的微小“凹坑模式”。 所描述的凹坑是粘膜隐窝的表面开口。 Kudo 表明,表面凹坑模式可以高精度地预测病变的组织学 [2-4],从而能够区分腺瘤性和增生性息肉。 对活体染色剂潜在有害影响的担忧增加了另一种色素内窥镜检查染料靛胭脂的普及 [5]。 Indigocarmine 不被细胞摄取,而是位于结肠壁上。 近十年前发表的东亚、美国和欧洲的研究表明,通过结合靛蓝胭脂红和放大内镜,可以将肿瘤性结肠息肉(腺瘤)与非肿瘤性增生性息肉区分开来,准确率为 68-96% [6-10]。 通过同样的方法,表面图案也可以识别小的早期结直肠癌[11]。 随后的研究表明,靛胭脂无需光学放大即可充分区分腺瘤性和增生性息肉 [12-15]。
在过去的几年中,已经开发了密切检查结肠内病变的新方法。 数字成像技术或“虚拟色素内窥镜”系统已被内窥镜制造商开发为色素内窥镜染料的“按钮式”替代品,使用起来可能更简单、更快捷。 这些新技术中第一个可用的是奥林巴斯生产的窄带成像 (NBI) 系统。 内窥镜内的滤光片选择出窄带宽的蓝光和绿光,从而减少红光。 这些波长仅穿透粘膜的表层,突出表面图案和微血管系统。 与增生性息肉相比,腺瘤性息肉具有更多且突出的微血管。 该系统已被证明可有效区分小型增生性和腺瘤性息肉,准确度为 91 - 94% [16-23]。
Fujinon 开发的 Fujinon Intelligent Color Enhancement (FICE) 系统使用后处理器电子滤色器技术,可以查看一系列具有不同波长的光的滤镜图像。 在小息肉的表征中已经取得了与 NBI 类似的结果 [13、24、25]。
最近,宾得开发了一种更进一步的虚拟色素内窥镜系统 i-Scan。 i-Scan 识别相邻像素之间的反差区域(即亮边暗)并增强这种反差。 此外,在与 FICE 类似的方法中,对光谱的红色、绿色和蓝色分量进行后处理器处理可用于增强血管和表面结构。
来自德国的一项研究表明,i-Scan 提高了可弯曲乙状结肠镜检查期间对病变的检测,并且可以准确预测组织学。 需要有关 i-Scan 在评估结肠息肉方面的临床疗效的更多数据,特别是在英国人群和肠癌筛查人群中,这两种人群迄今为止都没有进行过研究。
通过使用这些新技术,可以实时准确地预测小息肉的组织学。 一旦以高可信度做出体内诊断,小的增生性息肉可能会被丢弃,而不是送去进行组织学分析,或者留在原位 [22]。 这在降低患者息肉切除术的频率和随之而来的风险方面具有潜在益处,并且还可以降低病理学成本。 英国和美国关于在基线检查中检测到腺瘤后重复监测结肠镜检查的指南是基于腺瘤的数量、大小和组织学 [26, 27]。 目前,一旦收到任何已切除息肉的组织学报告,就会确定这些“重新检查间隔”。 通过准确的体内诊断,可以在完成基线结肠镜检查后立即设置这些间隔。
