假设:
- 在接受 VTE 预防的高风险下肢骨折患者中,与接受 LMWH 的患者相比,接受 ASA 的患者出血并发症的发生率较低。

方法/结果:

- 重要器官的致命性出血(腹膜后、颅内、眼内、脊柱内)
- 临床上明显出血伴 Hb 下降 > 2g/dL 或需要 > 2U 输血
- 需要再次手术的伤口引流或血肿
- 深部手术部位感染的诊断
- VTE 事件定义为任何有症状的近端 DVT(在股骨或腘血管中)或 PE(中央、节段或亚节段)的复合。 所有 VTE 事件都将使用多平面 CT 扫描或正式的静脉双相检查进行确认。
- VTE 预防治疗、VTE 事件和出血相关并发症的费用。
数据采集:
符合纳入/排除标准的患者将被前瞻性地随机分配到两个治疗组之一。 将使用块随机化。 患者将接受分配的 VTE 预防,并跟踪他们的指数住院和出院后 3 个月的 VTE 事件和出血并发症。 将在指数住院期间、出院后 2 周和 3 个月前瞻性收集结果数据,并在 50% 和 100% 招募时对结果进行盲法分析和解释。
数据分析:

- 所有在 1 级创伤中心接受过以下任何一种或多种损伤的患者:骨盆骨折(手术或非手术)、髋臼骨折(手术或非手术)或任何手术肢体骨折(靠近跖骨/腕骨)
- 年龄大于等于18岁
排除标准:
- 入院前接受预先存在的混杂预防或治疗性抗凝(不包括抗血小板药物)的患者或受伤后接受超过一剂 LMWH 的患者
- 先前存在凝血病的患者
- 过去 6 个月内有 VTE 病史的患者
- 怀孕的患者
- 中枢神经系统或脊髓损伤患者可能需要进一步神经外科干预,排除阿司匹林抗凝治疗
- 活动性出血患者不能使用抗凝药
- 随机分组时肌酐清除率受损 <30ml/min
- 肝素诱导的血小板减少症或 ASA 或 NSAID 过敏史
