小细胞肺癌约占所有肺癌的 13%,其中三分之一的患者在诊断时为局限期 SCLC。 目前LS-SCLC的标准治疗是同步放化疗,对联合放化疗反应良好的患者进行预防性颅脑放疗,中位生存期15-23个月,5年生存率高达到 26%。
LS-SCLC 的最佳剂量/分数仍有争议。 对于具有快速倍增时间和高生长分数的特点的SCLC,也有证据表明延长或中断总辐射时间会加速再增殖而导致治疗失败和不良结果。在我们之前的研究中我们也发现总辐射时间可能在 LS-SCLC 的治疗中发挥重要作用,并且接受 >57 Gy 的高生物有效剂量(BED,包括时间因素)治疗的患者具有良好的局部控制和存活率。

纳入标准:
- 经组织学或细胞学证实的 SCLC。
- 男女不限,年龄18-70岁。
- ECOG 性能状态 0 到 2。
- 局限期 SCLC 被定义为疾病局限于一侧胸腔和肺门、纵隔或锁骨上淋巴结,无胸腔积液,可以安全地包含在可耐受的辐射野内。
- 没有先前的胸部放疗。
- 六个月内体重减轻小于或等于百分之五。
- FEV1 大于 0.75L。
- 无严重内科疾病,无脏器功能障碍。
- 既往无任何肿瘤病史。
- CT造影剂皮试阴性。
- 已接受 1-6 个周期的 VP16 加 DDP/卡铂。
- 自愿参加本研究,并由本人或其代理人签署知情同意书。 对学习程序有良好的依从性,能配合相关检查、治疗和随访。
排除标准:
- 其他肿瘤病史(预期寿命≥3个月的皮肤癌/乳腺癌/口腔癌/宫颈癌除外)。
- 多原发性肺癌。
- 任何不稳定的全身性疾病,包括活动性感染、未控制的高血压、不稳定型心绞痛、过去 3 个月内新观察到的心绞痛、充血性心力衰竭(纽约心脏协会 (NYHA) II 级或更高)、6 个月前心肌梗死发作入组,且有严重心律失常、肝、肾或代谢性疾病需要药物治疗者。 人类免疫缺陷病毒 (HIV) 感染。
- 怀孕或哺乳期妇女。
- 患有精神疾病的患者,被认为“不能完全理解本研究的问题”。
- 曾接受过其他化疗方案、任何放疗或 TKI。
