由实质功能障碍引起的急性低氧性呼吸衰竭是免疫功能低下的血液病患者的主要并发症之一。 在这些患者队列中,经常需要机械通气以恢复氧合作用和正常碳酸血症。 由于每个正压通气方案都可能潜在地确定肺部并发症,由于压力和体积肺稳态和隔膜活动的改变,还必须评估隔膜功能,以免加重呼吸机引起的肺损伤 (VILI)。 VILI的主要靶点是肺间质和横膈膜。
肺间质经常涉及不同的损伤机制,这些机制源自诱导潮气量和呼气末正压 (PEEP)。 事实上,在辅助自主呼吸期间产生的大潮气量可能会配置非保护性通气方案和所谓的“pendelluft 现象”,即肺内空气从非依赖区域到依赖区域的固有流动而潮气量没有变化,可能会影响不充分PEEP 值。
正压通气也可能改变膈肌活动。 最近的数据表明,大约 65% 的接受机械通气的患者会出现膈肌功能障碍,这被认为是增厚部分的增强或减少。

本研究的目的是评估入住重症监护病房 (ICU) 的急性低氧性呼吸衰竭患者的基础膈肌活动,并记录 ICU 逗留期间与药物治疗优化相关的膈肌活动变化,如有必要,根据需要通气支持(使用 NAVA 协议提供有创或无创通气)。
本研究还打算记录每日膈肌增厚分数、每日动脉血气分析、无创通气失败频率、气管插管频率、机械通气时间、住院时间和住院死亡率。

纳入标准:
- 血液病患者的低氧性呼吸衰竭(PaO2 < 70 mmHg 或 P/F < 150)
排除标准:
- 入ICU前采用高流量鼻插管正压通气方案的患者
- 临床状况不稳定(使用升压药、急性冠脉综合征……)
- 拒绝治疗或知情同意
- 激动 (RASS ≥+2) 或缺乏合作 (Kelly Matthay ≥ 5)
- 多器官衰竭
