详细说明
目的 探讨反向头低脚高位 (RTP) 呼气末正压 (PEEP) 滴定操作对机械通气肥胖患者呼吸和血流动力学参数的影响。
设计随机对照试验。 参与者 30 名机械通气的肥胖患者被平均随机分配为 A 组,接受反向特伦德伦堡位置 (RTP) + 呼气末正压 (PEEP) 滴定,而 (B) 组:接受反向特伦德伦堡位置 (RTP)。
干预 60 例机械通气肥胖患者在术后减肥手术后立即接受一次 PEEP 和 RTP,然后通过手术呼吸机脱机,以评估动脉血气反应,然后进入重症监护病房 ICU 房间。
结果 主要结果包括呼吸参数(PaO2/FiO2 比率、PaCO2、Cstat),次要结果包括血流动力学参数(收缩压 (SBP)、舒张压 (DBP)、平均动脉压 (MAP)) .

描述
纳入标准:
本研究中的受试者具有以下标准:
- 在手术室接受上腹部手术(减肥)和全身麻醉的机械通气肥胖患者。
- BMI(30-34.9)的肥胖患者。
- 年龄40-50岁。
- 两性。
- 非吸烟者。
- 没有影响心脏和肺部的疾病史。
排除标准:
具有以下任何一项的受试者被排除在研究之外:
- 头部严重受伤。
- 增加颅内压和可能容易发展为病理性颅内压升高的人。
- 急性肺水肿。
- 气压伤/容积伤。
- 未引流的气胸/血胸。
- 连枷胸。
- 低血压(收缩压低于 80 mmHg)。
- 氧饱和度小于90%。
