i) 弥漫性大 B 细胞淋巴瘤 (DLBCL)，未另有说明 [包括生发中心 B 细胞 (GCB) 和活化 B 细胞 (ABC) 类型]

ii) 高级别 B 细胞淋巴瘤，伴有 MYC 和 BCL2 重排

iii) 高级别 B 细胞淋巴瘤，未另有说明

iv) T 细胞/组织细胞/富含大 B 细胞淋巴瘤 (THRLBCL)

v) Epstein-Barr 病毒 + DLBCL

  • 国际预后指数 (IPI) 评分 1 或 2，乳酸脱氢酶 (LDH) ≥ 1.3 x 正常上限 (ULN) 和/或定义为单病灶≥ 7 cm 的大块病变或 IPI ≥ 3。
